Numerous articles and studies have been published addressing this issue, and you can even check the \u003Ca href=\"https:\u002F\u002Fgithub.com\u002Fvectara\u002Fhallucination-leaderboard\u002Ftree\u002Fmain?tab=readme-ov-file\" target=\"_blank\" rel=\"noreferrer noopener\">regularly updated leaderboard\u003C\u002Fa> showing how models perform.\u003C\u002Fp>\n\n\n\n\u003Cp>With the release of newer versions, like GPT-4o, you might assume that such issues would be nearly nonexistent.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cfigure class=\"wp-block-image\">\u003Cimg decoding=\"async\" src=\"https:\u002F\u002Flh7-us.googleusercontent.com\u002FRYJxTDo0N699Ei3kbw3EWn-H6AQ-jeTto6dgsDmQYc0xihTDMMmHHhI_3yRrLHQ4oJSKPt8pJPVk9kryewl8Qz8WjFKwSajiB9rAVDoYkkfur6wOId_UhQG9fwLRFt2OynD3G52ok59DGzUiL61GfBo\" alt=\"\"\u002F>\u003C\u002Ffigure>\n\n\n\n\u003Cp>However, just the other day, we requested citations on a specific topic (AI in diagnostics) from ChatGPT, and despite multiple attempts, it refused to provide a working link. The journal existed when asked to provide the URL, but the cited article did not.\u003C\u002Fp>\n\n\n\n\u003Cp>We set out to test this and got completely mixed results. Under some circumstances, GPT-4o offered valid URLs, while the answers were utterly useless in other cases.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Recently, I came across a great paper: &#8220;\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fjournals.plos.org\u002Fdigitalhealth\u002Farticle?id=10.1371\u002Fjournal.pdig.0000503\" target=\"_blank\">Addressing 6 challenges in generative AI for digital health: A scoping review\u003C\u002Fa>&#8221; that not only listed the 6 major concerns but also their suggested solutions. Knowledge graphs are not our topics here, and they are entirely in the developer’s hands, so let’s just quickly summarise them in a few sentences. \u003C\u002Fp>\n\n\n\n\u003Cp>A knowledge graph is \u003Ca href=\"https:\u002F\u002Fai.stanford.edu\u002Fblog\u002Fintroduction-to-knowledge-graphs\u002F\">a structured set of information\u003C\u002Fa>. It allows LLMs to verify generated information against established facts and reduce the likelihood of hallucinating. Imagine it as a spatial (3D) object that shows structured data and the relationships and connections between these things. \u003C\u002Fp>\n\n\n\n\u003Cp>Here is a super simple example:&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cfigure class=\"wp-block-image\">\u003Cimg decoding=\"async\" src=\"https:\u002F\u002Flh7-us.googleusercontent.com\u002Fu_92XdP4SXAGxHI_FnbIiKOkVapZ1dUbjD7dTe-ntUHKGT1KP3r5AQSjmhWHKdgDEM6kE_ZZLC_p8G7e6WL_8aRAW30FvrkUzNYDVuXhhLnbbNqWhNDNZE2kcxl5hrsuEH66FIeiSPE9bNLrg1VrmcE\" alt=\"\"\u002F>\u003C\u002Ffigure>\n\n\n\n\u003Cp>In essence, a knowledge graph can act as a safeguard against inaccurate or misleading outputs by providing a foundation of verified information that the LLM can draw upon. This can be particularly important in medical applications where accuracy and reliability are non-negotiable.\u003C\u002Fp>\n\n\n\n\u003Cp>By fortifying models with comprehensive, accurate, and up-to-date structured information, they can spot more easily if something is “out of the ordinary” and will make sure to double-check.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Why do AI models have a temperature? Can they run a fever?\u003C\u002Fh2>\n\n\n\n\u003Cp>So let’s take a look at the first part of the suggested solution: temperature, starting with the basics.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>In the context of LLMs, temperature is a parameter that influences the balance between predictability and creativity in generated text. \u003C\u002Fstrong>Lower temperatures rely on learned patterns, resulting in more deterministic and factually accurate outputs. Meanwhile, higher temperatures encourage exploration, leading to more diverse and creative, albeit potentially riskier, fabricated responses.\u003C\u002Fp>\n\n\n\n\u003Cp>In essence, the temperature setting acts as a dial that can be adjusted to prioritise either accuracy or creativity, depending on the specific use case. This concept is particularly relevant for medical professionals, who require accuracy and reliability in AI-generated information. \u003C\u002Fp>\n\n\n\n\u003Cp>By understanding and potentially manipulating the temperature setting, healthcare providers can better use the power of generative AI while mitigating the risks of hallucinations and other inaccuracies.\u003C\u002Fp>\n\n\n\n\u003Cp>So, there might be a future where we have adjustable sliders to control the &#8220;temperature&#8221; of LLM responses, especially if companies like OpenAI take note and decide to address this issue. But let&#8217;s not wait for that to happen. You can indeed try the “Set your answer’s temperature to 0.1” prompt when you need factual results, but according to our testing, you may still get hallucinated or useless outputs, which indicates that the “answer temperature” for these public models has a default value set by the developers. Therefore, it&#8217;s essential to maintain a critical eye when evaluating the responses generated by LLMs.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>This inherent unpredictability in response quality is a good reminder that while these models are excellent, they are not infallible and require careful interpretation and validation, especially in critical fields like medicine. As such, we aimed to write this article to guide medical students regarding what they can expect to see when they start practising medicine.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>A cultural transformation will shape digital health practices\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>While digital health is fuelled by technological means, its effective implementation in medical practice relies on \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fdigital-health\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">a cultural transformation\u003C\u002Fa>. This means that new roles and new partnerships will be essential in achieving such implementation. In particular, the traditional, hierarchical model with one-way communication and decision-making between doctors and patients will no longer be relevant.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>This is being replaced by an equal-level partnership where empowered patients bring insights about their own health through data collected by wearables and, using this data, discuss treatment plans with their physicians. This is because with a myriad of tools available for patients, they don’t need the permission of doctors to monitor their health but would benefit from medical supervision and advice.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>With the paradigm shift that patient empowerment presents, patients essentially need to be considered as members of their own medical team and should be encouraged to have some input in their medical journey. Doctors will thus have to learn to work with them, understand their needs and guide them.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>AI will assist, and not replace, doctors\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Science fiction tropes often depict AI as overtaking humans, leading to a dystopian future. Recent developments in the AI field have led to real-life comparisons with such depictions, with fears of AI \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fedition.cnn.com\u002F2024\u002F01\u002F15\u002Ftech\u002Fimf-global-employment-risk-ai-intl-hnk\u002Findex.html\" target=\"_blank\">taking over\u003C\u002Fa> the jobs of humans. This also includes the fear of AI replacing doctors but is \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002F5-reasons-artificial-intelligence-wont-replace-physicians\" target=\"_blank\">very likely unfounded\u003C\u002Fa>.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>This is because while AI can handle specific, data-heavy tasks extremely well, medical practice is not linear and requires human creativity to handle complex tasks. Moreover, AI cannot replace the human touch nor the empathy required in care delivery. AI will have a place in the future of medical practice, handling repetitive tasks and assisting in decision-making. Otherwise they are useless.\u003C\u002Fp>\n\n\n\n\u003Cp>For example, at the annual \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fbest-of-ces-health-tech-2024-from-medical-tricorders-to-ai-powered-mirrors\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">Consumer Electronics Show\u003C\u002Fa>, a number of promising digital health tools are showcased. But at the same time, \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fces-2020-the-wows-and-the-whys-in-health-tech-innovation\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">much less useful ones\u003C\u002Fa> are presented such as an eye mask that helps rest your eyes by having you do nothing. The market is filled with such gimmicks which can overwhelm patients. Doctors will have to be able to advise patients on which technologies are actually useful.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Understand and embrace patient design\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Some of the digital health technologies and approaches of the future will be influenced by \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002F3-reasons-why-patient-design-must-replace-the-patient-centricity-illusion\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">patient design.\u003C\u002Fa> The latter involves patients as active participants at the highest levels of decision-making in healthcare.
