Policy-makers around the world started to realize\nthe incredible potential in genomics for population health in the last 3-5\nyears, but there are huge question marks whether they can manage the use of this\nincredibly useful pool of data in an appropriate framework, with well-thought-out\nmeans, for the right purposes, meaning for the well-being of humans and\ncommunities in the present and the future. Here, we launched an article series\nto look at the countries with the most experience. Let’s start with a truly\npositive example: the fairytale from the digital republic: Estonia.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Your whole genome sequenced as a holiday special for $200?\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Currently, anyone can order a DNA test for full genome\nsequencing on their smartphone and pay with virtual wallets, Apple or Google\nPay, for example, while sitting on the couch in their living room. 15 years\nago, not a single ingredient of the previous sentence existed: no virtual\nwallets, no smartphones, and no sequenced genome. Okay, perhaps your couch\nwould have been the same. The point is that technological progress has reached\nincredible heights in an absurdly short time – and that will likely accelerate\nin the years to come.\u003C\u002Fp>\n\n\n\n\u003Cp>Take whole-genome sequencing. \u003Cstrong>While deciphering the secrets of the human DNA for the first time through the completion of the state-funded \u003C\u002Fstrong>\u003Ca href=\"https:\u002F\u002Fwww.genome.gov\u002F12011238\u002Fan-overview-of-the-human-genome-project\u002F\">\u003Cstrong>Human Genome Project\u003C\u002Fstrong>\u003C\u002Fa>\u003Cstrong> took 15 years, today you can order a full genome sequencing test for hundreds of dollars and get a comprehensive view about your genetic background within weeks\u003C\u002Fstrong>. As a pioneer, \u003Ca href=\"https:\u002F\u002Fwww.veritasgenetics.com\u002F\">Veritas Genetics\u003C\u002Fa> has been \u003Ca href=\"http:\u002F\u002Fwww.xconomy.com\u002Fboston\u002F2016\u002F10\u002F17\u002Fveritas-genetics-lands-30m-for-1000-genome-sequencing\u002F\">hailed\u003C\u002Fa> as the company to be able to sequence, analyze and interpret whole human genome data for less than $1,000. The company even offered \u003Ca href=\"https:\u002F\u002Fwww.wired.com\u002Fstory\u002Fwhole-genome-sequencing-cost-200-dollars\u002F\">the test for $200 as a holiday special last year\u003C\u002Fa>. \u003C\u002Fp>\n\n\n\n\u003Cfigure class=\"wp-block-image\">\u003Cimg loading=\"lazy\" decoding=\"async\" width=\"512\" height=\"288\" src=\"https:\u002F\u002F1712507217.rsc.cdn77.org\u002Fwp-content\u002Fuploads\u002F2018\u002F11\u002F051_genomsequence_v2-512x288.png\" alt=\"\" class=\"wp-image-22404\" srcset=\"https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2018\u002F11\u002F051_genomsequence_v2-512x288.png 512w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2018\u002F11\u002F051_genomsequence_v2-370x208.png 370w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2018\u002F11\u002F051_genomsequence_v2-768x432.png 768w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2018\u002F11\u002F051_genomsequence_v2.png 870w\" sizes=\"auto, (max-width: 512px) 100vw, 512px\" \u002F>\u003C\u002Ffigure>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Governments on the quest for genomic data\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>As you can get unique insights into your past through ancestry and peek into your future through your health risks alongside the potential to change your fate, \u003Cstrong>\u003Ca href=\"https:\u002F\u002Fjournals.plos.org\u002Fplosbiology\u002Farticle?id=10.1371\u002Fjournal.pbio.1002195#pbio.1002195.ref024\">researchers estimate\u003C\u002Fa> that it is possible that as many as 25 percent of the people in developed nations and half of that in less-developed countries \u003C\u002Fstrong> \u003Cstrong>(comparable to the current worldwide distribution of Internet users)\u003C\u002Fstrong> \u003Cstrong>will have their genomes sequenced by 2025, while the world’s population is projected to top 8 billion – thus, it means between 100 million and 2 billion people. \u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>That’s already such an amount of sensitive health data that cannot be left out of sight for regulators and policymakers either – especially since it has the power to reveal information about the past and to influence the future\u003C\u002Fstrong>. Thus, in the last years, governments are contemplating sequencing large portions of their populations: \u003Ca href=\"https:\u002F\u002Fjournals.plos.org\u002Fplosbiology\u002Farticle?id=10.1371\u002Fjournal.pbio.1002195\">both England and Saudi Arabia have announced plans to sequence 100,000 of their citizens\u003C\u002Fa>, and researchers in both the US and China aim to sequence 1 million genomes in the next few years. In Ireland, Genomics Medicine Ireland had \u003Ca href=\"https:\u002F\u002Fwww.the-scientist.com\u002Fbio-business\u002Flearning-from-icelands-model-for-genetic-research-31435\">raised more than $40 million\u003C\u002Fa> by November 2016 to launch its own program to develop diagnostics and drugs informed by sequencing Irish DNA. In April 2018, news outlets surfaced that one of the biggest states in India, \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Frise-in-population-genomics-local-government-in-india-will-use-blockchain-to-secure-genetic-data\">Andhra Pradesh, will secure the DNA base of 50 million citizens through the blockchain\u003C\u002Fa>. The government \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Frise-in-population-genomics-local-government-in-india-will-use-blockchain-to-secure-genetic-data\u002F\">signed a Memorandum of Understanding\u003C\u002Fa> with a German genomics and precision medicine start-up, Shivom, which will first launch a trial to determine the viability of their technology for moving from a proactive to a preventive approach in medicine, and towards precision health. How safe would sensitive DNA information remain in the hands of governments or private companies? How safe would it be for citizens to let \u003C\u002Fstrong>\u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fhow-autocracies-could-misuse-digital-health-innovations\">\u003Cstrong>authorities know about their innermost biological secrets alongside their health risks\u003C\u002Fstrong>\u003C\u002Fa>\u003Cstrong> and future prospects? How advisable would it be to rather entrust private companies with this information?\u003C\u002Fstrong> \u003C\u002Fp>\n\n\n\n\u003Cp>As the time has come to figure out how to best\nregulate the privacy and security issues around genetic and genomic data –\npreferably as soon as possible -, in this article series, we looked around how\nvarious countries with specific institutional settings and social backgrounds\ntried to manage the issues around genomics. What we aim to see through these\narticles is how countries with various population sizes, institutional\nsettings, start-up scenes, and attitude towards digital health technologies\nattempt to handle genomic information. Who is the initiator of technological\nprogress: a strong entrepreneurial scene or the government? How big is the\npopulation size and how homogenous are ancestry lineages? (That could be\nimportant when it comes to the question whether it is possible to extrapolate\ngenetic information to relatives.) How strong is the institutional setting in\npreserving privacy rights, in counteracting economic or market interests or the\ninterests of one particular political group?\u003C\u002Fp>\n\n\n\n\u003Cp>These are extremely important questions when it comes\nto the future of genomics and whether or not we manage to use this incredibly\nuseful pool of data for the right purposes, meaning for the well-being of\nhumans and communities in the present and the future. \u003C\u002Fp>\n\n\n\n\u003Cp>Thus, at first, let’s look at the tiny digital republic, Estonia with a strong institutional setting, government-supported tech scene, and enthusiasm towards genetics in healthcare.\u003C\u002Fp>\n\n\n\n\u003Cfigure class=\"wp-block-image\">\u003Cimg loading=\"lazy\" decoding=\"async\" width=\"512\" height=\"384\" src=\"https:\u002F\u002F1712507217.rsc.cdn77.org\u002Fwp-content\u002Fuploads\u002F2019\u002F09\u002FGenome-Security-512x384.jpg\" alt=\"\" class=\"wp-image-24946\" srcset=\"https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2019\u002F09\u002FGenome-Security-512x384.jpg 512w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2019\u002F09\u002FGenome-Security-768x576.jpg 768w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2019\u002F09\u002FGenome-Security.jpg 1440w\" sizes=\"auto, (max-width: 512px) 100vw, 512px\" \u002F>\u003Cfigcaption class=\"wp-element-caption\">Source: www.wired.com\u003C\u002Ffigcaption>\u003C\u002Ffigure>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Solutions from the digital republic &#8211; Estonia\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Estonia is a tiny republic with 1.3 million residents sitting in the Baltic region of Europe. The country recognized early at the dawn of the IT revolution, already in the 1990s, that their \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fdigital-healthcare-in-estonia\u002F\">chances to get out of poverty lay in two megatrends – IT and genetics\u003C\u002Fa>. During the years, the country has built up an entire digital republic – first of all with incentives coming from the government. The normal services that the government is involved with &#8211; legislation, voting, education, justice, banking, taxes, policing, and, naturally, healthcare &#8211; have been digitally linked across one platform, X-Road. Citizens can vote through their laptops, sign contracts with their digital signature, or use their chip-IDs when surfing around in business and land registry – knowing that their data is secured through the blockchain and open to everyone. By having \u003Ca href=\"https:\u002F\u002Fwww.spiegel.de\u002Finternational\u002Fbusiness\u002Festonian-president-kersti-kaljulaid-our-government-is-more-innovative-a-1264358.html\">78.1 percent of public bureaucracy digitized\u003C\u002Fa>, the country also saves around 2 percent of its gross domestic product.\u003C\u002Fp>\n\n\n\n\u003Cp>Concerning the healthcare system, one of the most important goals is to build an ecosystem where genetic data-based, personalized and preventive solutions are at the center. The personalized medicine project has already been ongoing since 2002, the date of the foundation of the Estonian Genome Center, but it is going to take years and years to fully reap its benefits, remarked Priit Tohver, Junior Doctor and Digital Services Advisor at the Ministry of Social Affairs in Estonia to The Medical Futurist.\u003C\u002Fp>\n\n\n\n\u003Cfigure class=\"wp-block-image\">\u003Cimg decoding=\"async\" src=\"https:\u002F\u002F1712507217.rsc.cdn77.org\u002Fwp-content\u002Fuploads\u002F2019\u002F05\u002F084_estonia_v2-512x288.png\" alt=\"healthcare in Estonia\" class=\"wp-image-23942\"\u002F>\u003C\u002Ffigure>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>The Estonian recipe: legal framework, blockchain, and\ntrust\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>The latest step in implementing the personalized\nmedicine project was that on 20 March 2018, \u003Ca href=\"https:\u002F\u002Ffuturism.com\u002Fgenetic-testing-estonia\u002F\">Estonia launched the first stage of a national\nstate-sponsored genetic testing and information service\u003C\u002Fa> providing 100,000 of its 1.3 million residents with\ninformation on their genetic risk for certain diseases. Now, approximately 10\npercent of the population has their genetic data in the Estonian biobank, \u003Ca href=\"https:\u002F\u002Fwww.forbes.com\u002Fsites\u002Fyiannismouratidis\u002F2019\u002F01\u002F30\u002Festonia-houses-the-biggest-biobank-in-europe\u002F#522d01c3227a\">the biggest one in Europe\u003C\u002Fa>, and that number will reach 20 percent or even more\nby the end of this year, added Tonu Esko, Vice Director of the Estonian Genome\nCenter. He added that since 1,500 people join the Estonian biobank every single\nweek, he hopes that the entire population’s genetic profile can be drawn up\nwithin the next ten years, or even the next 5-7 years.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>And how come that the Estonian citizens are willing and eager to give their most personal data, their health, and genetic information into the hands of the state voluntarily? Are they not aware of the privacy risks or not worried that someone could steal the data and misuse it? The answer is that the government created such a regulatory framework alongside with technologies of trust that make the average citizen believe that their data is in safe hands.\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>Blockchain-based technology is just one, although a\nvery important element in the system. In March 2017, the Baltic country’s \u003Ca href=\"http:\u002F\u002Fpwc.blogs.com\u002Fhealth_matters\u002F2017\u002F03\u002Festonia-prescribes-blockchain-for-healthcare-data-security.html\">eHealth Authority signed a deal\u003C\u002Fa> with \u003Ca href=\"https:\u002F\u002Fguardtime.com\u002F\">Guardtime\u003C\u002Fa>, an Estonian company developing K.S.I. blockchain technology, to secure\nthe health records of Estonians. When The Medical Futurist asked Ivo Lohmus\nfrom Guardtime \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fwhat-the-hell-is-blockchain-what-does-it-mean-for-healthcare-and-pharma\">to explain how the blockchain works\u003C\u002Fa>, he said it is to be imagined as a shared book of\nrecords, or in more technical terms, a distributed database that’s designed in\nsuch a smart way that whatever is added to this database, that’s immutable. As\nif it’s carved into stone. What makes it more secure is that the blockchain\ndoes not directly deal with the data. Through the cryptographic process, a\nunique identifier, a hash, is created, which functions similarly to the biological\nfingerprint. While you can identify anyone based on his or her fingerprint, you\ncannot “reconstruct” the whole person.\u003C\u002Fp>\n\n\n\n\u003Cp>Moreover, beyond having secure identities through the ID card, the mobile ID, and the smart ID, the Estonian constructors of the new digital system were also very thoughtful when it came to establishing the regulatory and legal framework. Tonu Esko said that in 2000, the Estonian parliament approved the \u003Ca href=\"https:\u002F\u002Fwww.riigiteataja.ee\u002Fen\u002Feli\u002F531102013003\u002Fconsolide\">Human Genes Research Act\u003C\u002Fa> that regulates genetic research, the activities of the biobank, but more importantly, it protects citizens against any discrimination by their employer, by health insurance or law enforcement units. That means if any Estonian learns that they carry an Alzheimer or Parkinson’s disease mutation, an employer or a health insurance company doesn’t have the right to know that, or in case of obtaining this information in any legal way, it’s illegal to discriminate on this basis – to change risk scores or employer status, for example.\u003C\u002Fp>\n\n\n\n\u003Cp>And finally, not only did the government put in place a carefully thought-out legal system and the blockchain, but it also cares a lot about implementing it – gaining the general trust of citizens. That means, for example, that attempted data breach or misuse has consequences. Both Ivo Lohmus and Priit Tohver mentioned that in some cases, nurses or doctors looked up health data for patients – in one case a famous person’s health information – that they were not supposed to look at. When the \u003Ca href=\"https:\u002F\u002Fwww.genome.gov\u002F12011238\u002Fan-overview-of-the-human-genome-project\u002F\">Human Genome Project\u003C\u002Fa> was completed in 2006, and the DNA double spiral uncovered its secrets for the very first time, scientists agreed that it&#8217;s just as a revolutionary moment for mankind as Neil Armstrong stepping on the Moon. \u003Ca href=\"https:\u002F\u002Fghr.nlm.nih.gov\u002Fprimer\u002Fhgp\u002Fdescription\">Researchers said\u003C\u002Fa> that learning about the functions of genes and proteins will have a major impact in the fields of medicine, biotechnology, and life sciences.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">Although \u003Cstrong>the technical conditions, \u003C\u002Fstrong>\u003Cstrong>\u003Ca href=\"https:\u002F\u002Fpdfs.semanticscholar.org\u002F0e38\u002F7bd00952c8450deefcdbcdebf5c946c20f54.pdf\">the time and the cost of sequencing genomes were reduced by a factor of 1 million in less than 10 years\u003C\u002Fa>\u003C\u002Fstrong>\u003Cstrong>, the revolution lags behind. Before you start worrying, I can assure you: it will come\u003C\u002Fstrong>. When the price of genome sequencing reaches that of a blood test – it is already dropping below $1000 – and the data analysis will happen according to layers (and special pricing according to the various batches of information!), we will see how many areas of our lives genetic information impacts.&nbsp;\u003Cstrong>It affects pharmaceutical decisions, lifestyle and dietary choices, disease prevention plans, the human aging process and life expectancy in general.\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">However, the latter has been questioned over time for the advantage of environmental factors. I have learned what medication would have a side effect on me; what major conditions I’m at risk for; why I have certain features such as the inability to run marathons and what I can expect from the future. As you could imagine, that’s NOT among my favorite activities. What could I do to pass the time? I went to my primary care physician to tell her that I expect such a sample (side note: she hasn’t gone through the results yet, I’ll let you know as soon as she does so) and asked her to help me in interpreting the results. Besides, I also checked what kind of reports they will send me. That’s fascinating and weird at the same time.\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">At first, I was baffled by the size of the document and I was afraid that I’ll have \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fhow-to-overcome-being-cyberchondriac\">cyberchondriac\u003C\u002Fa> tendencies. However, the report supports interpretation with several “crutches”: they emphasize the likelihood of the risks, they include various illustrations and scales to visualize the results, and they also divide the outcome based on the prevalence of evidence. Thus, there is a difference between having a risk based on one study or fifty – more evidence certainly gives more weight to a risk and it helps the tester to put the results into perspective.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">Bearing these in mind, I went through the report. Although I already knew some of the results from previous genetic tests, the majority of the findings was entirely new for me, and I understood a lot more about myself. In some instances, I even found the explanation for my inclination towards activities. For example, I read sentences like your genes indicate you are likely to excel at power-based sports and training, but less likely to be good at endurance sporting activities. For all my life, I have been aware of the fact that I’m not a long-distance runner, but now also understand why I had the tendency to choose sprints and why I’m better at running fast for short distances. Nevertheless, on the same page, I was also glad to read that I don’t have sickle cell trait or that I’m not predisposed to an abnormal amount of exercise-induced fatigue.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">I was also informed that I have a moderate risk for melanoma, which I already knew from previous genetic tests. They informed me that tanning beds or sunburns are highly avoidable for me – although I did that pretty much for my entire life. Another previously known fact about myself was my predisposition to thrombosis. They detailed that I have a Factor V Leiden mutation and a Factor V R2 mutation. It argues that despite general sentiments, medications do not have the same effect on people. Some doctors already highly \u003Ca href=\"https:\u002F\u002Fwww.pharmgkb.org\u002Fguideline\u002FPA166104949\">recommend genetic testing before any prescription of, e.g.\u003C\u002Fa> Warfarin, a type of anti-blood clotting drug takes place. Dante Labs also tests for Warfarin, and it turned out I have a slight resistance towards that.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">Nevertheless, I received information about how my body metabolizes more than a hundred drugs, which is incredibly useful. For example, my cholesterol level is usually higher than the reference value, however, I would definitely have severe side effects if I took prescription pills for high levels of cholesterol. That means a risk for cardiomyopathy, for example. Moreover, the test revealed that I have a risk to develop lactose intolerance as an adult – and I already suspected that, and now the report confirmed it.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">However, I had no idea before that I have a high risk for knee arthritis and moderate ones for hip and wrist arthritis, so I should rather not gain weight during the years and I should not wear high heels quite often. That latter came as a real shocker. But jokes aside, I’m glad that I know about these risks alongside with the risk for atrial fibrillation, cardiovascular disease, colorectal cancer or brain aneurysm.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">\u003Cstrong>All-in-all, I was very curious and fascinated to read the overview. I also thought that as a geneticist, I will not have any problem even with the most stringent medical explanation. However, when I went through the entire report, I had to face the truth: I need a genetic counselor to make the right conclusions\u003C\u002Fstrong>. It turned out they cooperate with the \u003Ca href=\"https:\u002F\u002Fwww.dnafeed.com\u002F\">DNAfeed.com\u003C\u002Fa> team, but they don’t have an advisor on their own. When you go to the site, you provide your e-mail address and upload the raw data, then a smart algorithm analyzes it and compares it to the latest scientific findings, and in the end, you have the chance to talk your results through with a specialist.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">\u003Cstrong>I had \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-genomic-data-challenges-of-the-future\">many privacy concerns about whether or not I should share my genomic information\u003C\u002Fa>\u003C\u002Fstrong>\u003Cstrong> with the team. I started to do my research, and it turned out that the service is HIPAA-compliant, information sharing is protected and encrypted, so I took a deep breath and uploaded my sample\u003C\u002Fstrong>. And then I asked. Tons of questions.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">I was curious whether I had any significant risk that I should definitely be aware of and take into consideration when making lifestyle choices. I wanted to know whether I had certain risks against which I could individually act or whether I possess a genetic factor that I should always keep in the back of my mind.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">The clinician calmed me by saying I don’t have any mutations in any of the known genes that may substantially increase my risk of hereditary cancer, for example, BRCA. The counselor mentioned the slightly increased risks for melanoma and colorectal cancer, but also explained the steps to take to prevent them: proper skin protection from sun exposure and consider periodic skin checks with a doctor; getting the recommended screening with colonoscopies to catch any potential tumors early.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">Moreover, the physician explained that I have a Factor V Leiden mutation, which is a specific gene mutation that results in thrombophilia. That gives me a slightly increased tendency to form abnormal blood clots that can block blood vessels. Nevertheless, he also added the much-needed context: this is a common mutation that 3-8% of people may have, and most people with this mutation never develop clots.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">That’s the primary reason why I decided for genetic counseling. You need to hear from a clinician that if you have a gene mutation or SNP, how likely it is going to cause you any problems later on and how likely its prevalence is in the population. And as scientific research proved, that has a lot more impact on a person’s life expectancy, for example, than genetics.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">Thus, as a next step, I will pack up my health sensors and trackers, go to the specialists and make a plan on how to avoid my genetic risks.
