Only with their help would it become possible to create a healthcare system that is futuristic even decades after the first plans were drawn. Digital health strategy “around the pill”\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Rather than focusing on traditional drug manufacturing and marketing, pharma companies will put more emphasis on new approaches relying on technology to appeal more to providers and payers. “Around the pill” is more than the production and the sale of drugs: it is about \u003Cstrong>\u003Cem>developing a drug and attaching a digital health technology to it\u003C\u002Fem>\u003C\u002Fstrong>.\u003C\u002Fp>\n\n\n\n\u003Cp>These are often patient-support programs, that are often non-clinical solutions, that can boost patient outcomes and benefit the entire health system. These initiatives create win-win situations, patients receiving more than just a pill, while pharma companies can build on the data and the feedback they receive – and the likely loyalty of patients who appreciate the extra care. If done well.\u003C\u002Fp>\n\n\n\n\u003Cp>Good solutions however are not easy to make. There are only a handful of good examples, one of these is that of \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002F4-examples-of-merging-gaming-digital-health\u002F\" target=\"_blank\">mySugr\u003C\u002Fa>. The startup’s approach for diabetes management is a gamified approach, wherein they reimagined diabetes as a Tamagotchi-like monster that can be tamed. By completing challenges, earning points and receiving personalized insights, the app \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fdiabetestimes.co.uk\u002Fwebinar-will-help-people-with-diabetes-to-build-resilience-post-pandemic\u002F\" target=\"_blank\">incentivises patients\u003C\u002Fa> to keep their glucose level at a desirable one. The idea was so good pharma giant Roche acquired mySugr in 2017 and kept the team to continue growth. The company went on to pair the app with its existing Accu-Chek Guide glucose meter to create the \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.mysugr.com\u002Fen\u002Fdiabetes-bundle\u002F\" target=\"_blank\">mySugr Bundle\u003C\u002Fa>.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">3. Digital pills\u003C\u002Fh2>\n\n\n\n\u003Cp>Digital pills, medications with embedded electronic circuits can be good solutions for specific patients with specific conditions. These refer to ingestible medications with embedded electronic circuits rather than smartphone logging apps.\u003C\u002Fp>\n\n\n\n\u003Cp>For example, such pills could help with medication adherence in people taking medicines regularly. The first pill approved by the FDA was \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.abilifymycite.com\u002F\" target=\"_blank\">Ablify Mycite\u003C\u002Fa> (by since-dissolved pharma startup \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fdigital-health-ventures-that-flew-too-close-to-the-sun\" target=\"_blank\">Proteus\u003C\u002Fa>), a drug that was aimed at helping psychiatric conditions like bipolar disorder and schizophrenia. As a patient swallows the pill, the acidic environment in the stomach activates the pill&#8217;s sensor, which thereafter begins to send Bluetooth signals to an external patch. It will then notify the smartphone app that the pill was taken. Regulatory agencies are unprepared for the waves of digital innovation.\u003C\u002Fp>\n\n\n\n\u003Cp>We could notice it with the appearance of the \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fliving-with-an-artificial-pancreas\u002F\">#wearenotwaiting \u003C\u002Fa>Twitter movement for patients suffering from diabetes. They started to use the so-called artificial pancreas without FDA or any other approval because the patients had the impression it works and they needed it. With the rapid innovation of digital health solutions, regulators need a more rapid response in order to get ahead of possibly dangerous DIY solutions.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Ca href=\"https:\u002F\u002Fwww.nature.com\u002Farticles\u002Fs41746-023-00873-0\" target=\"_blank\" rel=\"noreferrer noopener\">Generative AI has recently joined\u003C\u002Fa> this unprecedented regulatory discussion.\u003C\u002Fp>\n\n\n\n\u003Cp>Another example of regulations going after innovation is the case of direct-to-consumer genetic testing companies. They appeared approximately ten years ago on the market. However, in 2014 the FDA restricted 23andme’s operations saying that the health information services were not clear or complete enough for customers. And it is especially important in case of such sensitive information as money or healthcare data.\u003C\u002Fp>\n\n\n\n\u003Cp>In case of money, trust and credibility have long been established by central intermediaries, such as banks and other types of middlemen. You transfer money via your online bank account knowing it&#8217;s safe and secure because you trust the financial institution behind it. And in the case of online transactions, there has been another problem for which trustworthy intermediaries have meant the solution for years. The problem of duplications. When you send an e-mail with an attached cat photo, the image will automatically be copied. So, how do you make sure that doesn’t happen with your money? That the well-earned dollars that you spend on books will actually disappear from your bank account and appear on Amazon’s. Blockchain’s time-sensitive nature allows any data to move around in that particular format only once in the network. The blocks are impossible to change; only new entries can be added to the network. This is critical in case of health data. Just imagine what might happen if someone could change a patient’s blood type in the health record system without anyone noticing it.\u003C\u002Fp>\n\n\n\n\u003Cp>Moreover, Ivo Lohmus said that many times, as in the case of the K.S.I blockchain which is used for the Estonian medical records system, the blockchain does not directly deal with the data. Through the cryptographic process, a unique identifier of the data, a hash, is created, which functions similarly to the biological fingerprint. While you can identify anyone based on his or her fingerprint, you cannot “reconstruct” the whole person. And finally, as the blockchain is based on the consensus of network participants, access to data can be linked to permission.\u003C\u002Fp>\n\n\n\n\u003Cp>Beyond ensuring authentication and credibility, blockchain also \u003Ca href=\"https:\u002F\u002Fmedium.com\u002Fblockchain-review\u002Fhow-does-the-blockchain-work-for-dummies-explained-simply-9f94d386e093\">brings unprecedented security benefits\u003C\u002Fa>. Hacking attacks that commonly impact large, centralized intermediaries like banks would be virtually impossible to pull off on the blockchain. For example — if someone wanted to hack into a particular block in a blockchain, a hacker would not only need to hack into that specific block, but all of the proceeding blocks going back the entire history of that blockchain. According to the IBM Institute for Business Value blockchain study, new adopters of the technology expect the greatest blockchain benefits across time, cost, and risk in three areas: clinical trial records, regulatory compliance, and medical\u002Fhealth records.\u003C\u002Fp>\n\n\n\n\u003Cp>It is capable of transforming the entire system of medical records, just as Estonia has already done. In March 2017, the Baltic country’s \u003Ca href=\"http:\u002F\u002Fpwc.blogs.com\u002Fhealth_matters\u002F2017\u002F03\u002Festonia-prescribes-blockchain-for-healthcare-data-security.html\">eHealth Authority signed a deal\u003C\u002Fa> with \u003Ca href=\"https:\u002F\u002Fguardtime.com\u002F\">Guardtime\u003C\u002Fa> to secure the health records of over a million Estonians. \u003Ca href=\"https:\u002F\u002Fpatientory.com\u002F\">Patientory\u003C\u002Fa>, a start-up helping hospitals to secure their patient data while enabling patients to follow the fate of their own data, has urged the British government “to get behind a blockchain-enabled national IT health system” at the time of the \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-nhs-ransomware-attack-data-privacy-in-digital-health-part-one\u002F\">NHS ransomware attack\u003C\u002Fa>. If there were political will and comprehensive financial support, other countries could also follow Estonia’s as well as Dubai’s lead. The latter has also started to test blockchain technology for securing its electronic medical records.\u003C\u002Fp>\n\n\n\n\u003Cp>Due to its time-sensitive nature, blockchains shift the lens from disparate bits of information held by a single owner, to the lifetime history of an asset. Instead of big data, capturing long-term data becomes more easily possible. In many developing countries in Asia, Africa, and South America, counterfeit drugs comprise between 10 percent and 30 percent of the total medicines on sale.\u003C\u002Fp>\n\n\n\n\u003Cp>Cindy Greatrex, Vice President of The National Alliance of Research Associates Programs remarked that for combatting fake pharmaceuticals a solution needs to be employed that stops the counterfeits from contaminating the supply chain. The best solution is to track pharmaceuticals so that digital systems linked to medication moving in the physical world are established. This is important because when you have a unique digital reference to a drug and a physical copy of that drug, it is much harder to erase or duplicate one without the other.\u003C\u002Fp>\n\n\n\n\u003Cp>Blockchain offers security through transparency. It might work as follows: barcode-tagged drugs could be scanned and entered into secure digital blocks whenever they change hands. This ongoing real-time record could be viewed anytime by authorized parties and even patients at the far end of the supply chain. This would make it much more difficult for criminal networks to sell their counterfeit drugs on the market.\u003C\u002Fp>\n\n\n\n\u003Cp>However, the advantages of blockchain for pharma does not stop there. Drug developers running clinical trials might be able to share clinical data and medical samples more securely.
