Here, we provide an overview of the development paths of medical associations from book clubs until advocacy networks – and draw up a potential way towards digitally savvy innovation hubs.\u003C\u002Fp>\n\n\n\n\u003C!--more-->\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>From medical book clubs to knowledge societies\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Societies, associations or groups comprising of doctors, pharmacists or other representatives of the medical profession have been present since the 18th century, mainly the revolution of science and the dawn of modern medicine. John Coakley Lettsom, who founded the Medical Society of London in 1773, which invited both physicians, surgeons and apothecaries in its ranks, \u003Ca href=\"https:\u002F\u002Fwww.ncbi.nlm.nih.gov\u002Fpmc\u002Farticles\u002FPMC5277686\u002Fpdf\u002Fedinbmedj74454-0001.pdf\">said that the association should be\u003C\u002Fa> “\u003Cem>A Society of medical practitioners of various ranks who would meet together to compare their observations and compare their thoughts mutually, taking note of new discoveries at home and abroad\u003C\u002Fem>”.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>The noble task of sharing knowledge, exchanging thoughts and the results of the latest research was the primary task of medical societies in the 18\u003Csup>th\u003C\u002Fsup>-19\u003Csup>th\u003C\u002Fsup> century as the exchange of information was way more limited than today\u003C\u002Fstrong>. That’s why in many cases, medical book clubs turned into organizations representing the medical profession. Beyond keeping their members informed through all means of communication – newsletters, websites, e-mails, conferences, medical associations undertook the causes of medical professionals by advocating for their rights and interests, assisting doctors in protecting the interests of their patients or helping settle their disputes.\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>On a more macro-level, if a physician belongs to a medical society, he or she is \u003Ca href=\"https:\u002F\u002Fwww.britannica.com\u002Ftopic\u002FAmerican-Medical-Association\">tapped into a broader professional network\u003C\u002Fa> whose mission is to inform members about the newest, best practices. Along these lines, the association has all the means as well as the pool to give guidance and direction to doctors and other medical professionals about the present as well as the future of their profession. Moreover, it could also provide tools to prepare for the coming changes.\u003C\u002Fp>\n\n\n\n\u003Cp>Also, a medical association with thousands of members is able to create large-scale collaborations and undertake large surveys. They have a structure to educate actors of the healthcare scene about the latest technological changes, how to integrate digital health achievements into practices or they might even finance specific projects in the area of digital health or health IT.\u003C\u002Fp>\n\n\n\n\u003Cp>Medical associations could also provide a long-term vision about the future of healthcare and help tackle the hurdles in front of reaching a desired state of play. Thus, these societies have a huge responsibility in preparing doctors and nurses for the coming years. The \u003Ca href=\"http:\u002F\u002Fwww.commonwealthdoctors.org\">Commonwealth Medical Association\u003C\u002Fa> set up the \u003Ca href=\"https:\u002F\u002Fcwcdh.org\u002F\">Commonwealth Center for Digital Health\u003C\u002Fa> and launched the institution in April 2018. With a central hub based in London, the CCDH has the goal to promote digital transformation in healthcare. In collaboration with regional centers in the Commonwealth nations, the organization supports the implementation and clinical application of digital health technologies, acts as an incubator for agile start-ups, and offers education in the area of digital health.\u003C\u002Fp>\n\n\n\n\u003Cp>The \u003Ca href=\"http:\u002F\u002Fwww.bundesaerztekammer.de\u002F\">German Medical Association\u003C\u002Fa> has uplifted former restrictions for doctors to provide telemedical services to patients in the summer of 2018. The move promotes the expansion of telemedicine and, in a broader sense, digital health. It turned out that although excitement was quite high, the adoption levels remained lower than expected and the concerns were cemented around four issues: the lack of scientific evidence, payment and reimbursement issues, privacy and security concerns and how to integrate tools into clinical practice.\u003C\u002Fp>\n\n\n\n\u003Cp>With the results of the survey in mind, \u003Cstrong>the AMA launched a network called \u003C\u002Fstrong>\u003Cstrong>\u003Ca href=\"https:\u002F\u002Fxcertia.org\u002F\">XCERTIA\u003C\u002Fa>\u003C\u002Fstrong>\u003Cstrong> to develop guidelines and standards for e-health and the \u003Ca href=\"https:\u002F\u002Finnovationmatch.ama-assn.org\u002F\">Physician Innovation Network\u003C\u002Fa>\u003C\u002Fstrong>\u003Cstrong>. The latter is a virtual place for physicians and entrepreneurs to engage in conversation about their innovations, to test prototypes and even pilot them. The Digital Health Implementation Playbook is the most recent tool of the AMA, a living document guiding physicians through the difficulties of the adoption of new technologies\u003C\u002Fstrong>. While the Playbook is the result of 80 interviews and working sessions with different healthcare professionals, consumers and others on how to best implement digital health solutions in practice, the AMA is still continuously looking for feedback to improve it. One of the most genius thinkers was French writer and father of science fiction, Jules Verne. He mentioned one hundred and eight sci-fi concepts and inventions in his vast oeuvre &#8211; \u003Ca href=\"https:\u002F\u002Fmult-kor.hu\u002Fa-20-szazad-szamos-talalmanyat-elore-megjosolta-jules-verne-20180208\">seventy was already realized\u003C\u002Fa>: the spaceship, the television, the machine gun, air conditioning or the atomic bomb.\u003C\u002Fp>\n\n\n\n\u003Cp>He fantasized about a colossal submarine powered by electricity crisscrossing the oceans, the Nautilus, decades before the modern submarine was built. \u003Cstrong>He drew up the concept of space travel in his tale \u003Cem>From The Earth To The Moon\u003C\u002Fem>. Not only that but he also gave calculations and described the entire process: a capsule called &#8220;projectile&#8221; is fired off into space. \u003C\u002Fstrong>\u003Cstrong>\u003Ca href=\"https:\u002F\u002Fbureautijdenruimte.nl\u002Fschoolofforesight\u002Fnl\u002F2012\u002F11\u002F21\u002Fvisionaries-of-the-future-jules-verne-2\u002F\">He guessed it would be the Americans who’d do it and foretold the place of launch right within 50 miles\u003C\u002Fa>\u003C\u002Fstrong>\u003Cstrong>: a facility in South Florida\u003C\u002Fstrong>. Also, he imagined the capsule to splash down in the ocean and to be recovered from the sea.\u003C\u002Fp>\n\n\n\n\u003Cp>Regarding communication technology, Verne predicted that news would be read out to people instead of written down in newspapers, and in his article \u003Ca href=\"https:\u002F\u002Fwww.goodreads.com\u002Fbook\u002Fshow\u002F78735.In_the_Year_2889\">\u003Cem>In the Year 2889\u003C\u002Fem>\u003C\u002Fa>, he described the &#8220;phonotelephote&#8221;—a \u003Ca href=\"https:\u002F\u002Fnews.nationalgeographic.com\u002Fnews\u002F2011\u002F02\u002Fpictures\u002F110208-jules-verne-google-doodle-183rd-birthday-anniversary\u002F\">forerunner to today&#8217;s video conferencing technologies\u003C\u002Fa>. The device allowed &#8220;the transmission of images using sensitive mirrors connected by wires,&#8221; Verne wrote. At the dawn of the radio age, \u003Ca href=\"https:\u002F\u002Fwww.smithsonianmag.com\u002Fhistory\u002Ftelemedicine-predicted-in-1925-124140942\u002F\">in 1925, the never-resting German technologist invented the concept of the “teledactyle”\u003C\u002Fa> that \u003Cstrong>would allow doctors not only to see their patients through a view screen but also touch them from miles away with spindly robot arms. As Gernsback explained it, the device would have made it possible to “feel at a distance”. He basically described the function and aim of telemedicine – going a “tiny bit” further than technology would allow it today\u003C\u002Fstrong>.\u003C\u002Fp>\n\n\n\n\u003Cp>He wrote that “\u003Cem>The doctor of the future, by means of this instrument, will be able to feel his patient, as it were, at a distance. The doctor manipulates his controls, which are then manipulated at the patient’s room in exactly the same manner. The doctor sees what is going on in the patient’s room by means of a television screen. Every move that the doctor makes with the controls is duplicated by radio at a distance. Whenever the patient’s teledactyle meets with resistance, the doctor’s distant controls meet with the same resistance. The distant controls are sensitive to sound and heat, all important to the future diagnosis\u003C\u002Fem>.”\u003C\u002Fp>\n\n\n\n\u003Cp>Thus, the “teledactyle” was actually much more than telemedicine today – sensing the patient not only through vision and hearing, but also touch. Humanity is unfortunately not as advanced as several decades ago futurists thought it would be, but who knows, maybe we’ll get there at some point. Just look at the concept of the \u003Ca href=\"https:\u002F\u002Fcutecircuit.com\u002Fthe-hug-shirt\u002F\">Hug Shirt\u003C\u002Fa>! It allows people to send hugs over distance – just like a text message. It can feel the strength, duration, and location of the touch, skin warmth, and the heart rate of the sender. The tragic ballad by the German poet, Johann Wolfgang von Goethe, \u003Ca href=\"https:\u002F\u002Fwww.poemhunter.com\u002Fpoem\u002Fthe-erl-king\u002F\">The Erl-King\u003C\u002Fa>, would not have been written if telemedicine had been around. According to the story, a dad gallops through dark forests in the night to bring his son to the doctor. The little boy is hallucinating due to fever, most likely, and imagines the Erl-King calling him. It’s a hauntingly beautiful poem – but wouldn’t it be much better if the boy could have stayed at home and received medication without the need to go anywhere?\u003C\u002Fp>\n\n\n\n\u003Cp>Alternative ways were sought to solve these burning issues from the dawn of technological advancement – and that’s where telemedicine jumps in. \u003Cstrong>Already an \u003C\u002Fstrong>\u003Cstrong>\u003Ca href=\"https:\u002F\u002Fwww.ncbi.nlm.nih.gov\u002Fbooks\u002FNBK207141\u002F\">1879 article in the \u003Cem>Lancet\u003C\u002Fem> talked about using the telephone\u003C\u002Fa>\u003C\u002Fstrong>\u003Cstrong> to reduce unnecessary office visits. Every invention was tried at how it fits the medical arena. For example, since the 1920s, the radio has been used to give medical advice to clinics on ships. Later on, in the 1950s, \u003Ca href=\"https:\u002F\u002Fblog.evisit.com\u002Fhistory-telemedicine-infographic\">medical personnel started doing experiments with closed-circuit television\u003C\u002Fa>\u003C\u002Fstrong>. Probably one of the earliest and most famous uses of hospital-based telemedicine was in the late 1950s and early 1960s when a closed-circuit television link was established between the Nebraska Psychiatric Institute and Norfolk State Hospital for psychiatric consultations.\u003C\u002Fp>\n\n\n\n\u003Cp>Nevertheless, many technologies had to be perfected to make telemedicine possible on a wide scale. Thus, it is absolutely not surprising that space research was the area where the method developed fully. In the 1970s, NASA first began performing physiologic monitoring over distance. That was \u003Cstrong>the first time a patient was successfully defibrillated by telephone – and that was just a beginning of the myriads of ways how phones could play a role in telemedicine.\u003C\u002Fstrong>\u003Cstrong> The advancement of telecommunication technologies brought new forms of patient-doctor remote communication all the time\u003C\u002Fstrong>. With the appearance of the World Wide Web, sending electronic messages became the new norm of exchanging information. Afterward, video chat programs such as Skype appeared. The practice of telemedicine spread around, and soon humanity woke up to the fact that eVisits are possible on our smartphones.\u003C\u002Fp>\n\n\n\n\u003Cp>You don’t need to go to a hospital or any other healthcare facility, you can be anywhere on the planet. That’s the main difference in how earlier visions pictured telemedicine and how it finally evolved. The “teledactyle” imagines the patient in a medical setting, so they imagined the point-of-care remaining mainly the hospital room – or perhaps the home of the bedridden patient. Thus, there is no mobility here: everyone from the patient until the doctor – even the nurse in the background – stays in their lane. We are not bound by hospital rooms – either physically, or mentally. Telemedicine is mostly about receiving medical care through phone, video or chat anywhere in the whole wide world. Moreover, with the prevalence of trackers, wearables and health sensors, the measurement of data, as well as the retrieval of archived data with the help of the EMRs, is possible – thus the traditional examination methods are on their way out.\u003C\u002Fp>\n\n\n\n\u003Cp>At the same time, telemedicine is soaring, and the phenomenon will most likely not slow down in the future. It is projected that it will \u003Ca href=\"https:\u002F\u002Fblog.evisit.com\u002Fhistory-telemedicine-infographic\">evolve into a $34 billion industry by 2020\u003C\u002Fa> and further integrates into healthcare becoming a crucial part of everyday medicine. In the future, the telemedical process is most likely to be a lot more efficient.
