But they attest to the relics of a workflow from the 20th century; while 21st-century technology can radically improve the system from the inside out. Quite literally.\u003C\u002Fp>\n\n\n\n\u003Cp>Indeed, in designing the future of hospitals, much will be offloaded to the outside of these institutions by leveraging digital health technologies. Patients could monitor their vitals wherever they are thanks to wearables and share the data to their physicians remotely. But that does not make hospitals obsolete altogether. Rather, they will become health centres for patients for preventing diseases, for acute care patients and for patients who need surgical procedures or large radiology machines.\u003C\u002Fp>\n\n\n\n\u003Cp>While these are long term goals, several adjustments can be undertaken in the short term to build up to the ideal design of future hospitals. These&nbsp;could help better incorporate digital health tools to fight the pandemic, which could help those institutions become more resilient against future health crises.\u003C\u002Fp>\n\n\n\n\u003Cp>First, let’s see how the point of care can be redesigned from the inside. Read the second part of this article, the future of the hospitals – \u003Cem>outside\u003C\u002Fem> the point of care \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-future-of-healthcare-design-outside-the-point-of-care\" target=\"_blank\" rel=\"noreferrer noopener\">here\u003C\u002Fa>.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Accommodate digital health technologies\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>The pandemic accelerated the trend to adopt digital health technologies, many of which \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fdigital-health-tech-here-to-stay-after-covid-19\u002F\" target=\"_blank\">are here to stay\u003C\u002Fa>. Healthcare facilities should be ready for such events.\u003C\u002Fp>\n\n\n\n\u003Cp>For example, robots also have \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002F8-exciting-medical-robot-facts\" target=\"_blank\">a myriad of uses in healthcare\u003C\u002Fa> from making in-house deliveries to disinfecting surfaces; with the added bonus of not risking cross-infection or succumbing to burnout. A doctor’s desk could turn into their computer with a rollable screen or \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Faugmented-reality-in-healthcare-will-be-revolutionary\u002F\" target=\"_blank\">augmented reality technology\u003C\u002Fa> to plan for a procedure while decluttering their workspace. The \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.anatomage.com\u002Ftable\u002F\" target=\"_blank\">Anatomage Table\u003C\u002Fa> already offers a virtual dissection surface on a desk-sized screen; and designers could factor in such options when visualising new hospitals.\u003C\u002Fp>\n\n\n\n\u003Cp>Another promising technology to accommodate is telemedicine. It saw \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fcovid-19-was-needed-for-telemedicine-to-finally-go-mainstream\u002F\" target=\"_blank\">a surge in adoption\u003C\u002Fa> to continue providing care to patients remotely during the pandemic. Already, \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.bloomberg.com\u002Fnews\u002Farticles\u002F2021-02-23\u002Fhow-the-pandemic-is-transforming-hospital-design\" target=\"_blank\">a McKinsey report estimates\u003C\u002Fa> that 25% of outpatient services could be conducted this way in the future. In the U.S., 911 emergency calls that do not require prompt medical assistance \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.bloomberg.com\u002Fnews\u002Farticles\u002F2021-02-23\u002Fhow-the-pandemic-is-transforming-hospital-design\" target=\"_blank\">can be relayed to\u003C\u002Fa> telemedical consultations. This practice benefits overburdened hospitals by drastically reducing unnecessary visits and making queues a thing of the past; while also enhancing the patient experience.\u003C\u002Fp>\n\n\n\n\u003Cp>To further support telemedicine consultations, hospitals &#8220;\u003Cem>require dedicated spaces for virtual consultations. These will include user-friendly sound and lighting control, with dual screens to simultaneously converse with the patient and access to their medical records\u003C\u002Fem>&#8221; – Dr. Diana Anderson, the &#8216;\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.dochitect.com\u002F\" target=\"_blank\">Dochitect\u003C\u002Fa>&#8216; said in an interview with The Medical Futurist.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Flexible design\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>In our conversation with Diana Anderson, MD, M.Arch, and Principal of \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.jacobs.com\u002Fdelivery\u002Farchitecture\" target=\"_blank\">Jacobs\u003C\u002Fa>, together with Cath Lewin RN, BN, senior health planner at Jacobs for our \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.patreon.com\u002Fthemedicalfuturist\u002Fposts\" target=\"_blank\">Patreon site\u003C\u002Fa>, they explained that &#8220;\u003Cem>one of the main innovations to perpetuate is designing flexible space that allows for conversion to ‘pandemic mode’. There are great opportunities for us to really rethink patient flows on the site, at the front door, and within treatment facilities\u003C\u002Fem>.&#8221; Dr Anderson added that “\u003Cem>there are great opportunities for us to really rethink patient flows on the site, at the front door, and within treatment facilities\u003C\u002Fem>.”\u003C\u002Fp>\n\n\n\n\u003Cp>Asking about the implementation of such flexible designs, Diana Anderson and Cath Lewin have several ideas. For instance, &#8220;\u003Cem>open areas like at-grade car parks and external entry foyers can double as external triage areas when accommodating increased patient capacity and implementing physical distancing strategies. Ventilation in patient rooms is isolated thanks to glass doors. These also enable medical staff to observe patients while reducing the risk of cross-contamination. Moreover, the building’s lobby itself \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.rushu.rush.edu\u002Fnews\u002F%E2%80%98-what-rush-was-built-%E2%80%99\" target=\"_blank\">can accommodate extra beds\u003C\u002Fa> as it is already equipped with electrical and medical gas outlets. Virtually any room of hospitals could incorporate such flexible spaces; whether it’s a conference room or offices to support the extra load of patients when needed.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Enhancing the workplace of caregivers\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>While the design of future hospitals will enhance the patient experience, it should also create an improved workplace for caregivers. Enabling hospitals to accommodate more patients in emergency situations can lift the psychological toll off the staff who might be forced to prioritise whom to allocate resources to, as it happened \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fchoosing-between-life-and-death-during-covid-19-the-a-i-trolley-problem\u002F\" target=\"_blank\">during the COVID-19 pandemic\u003C\u002Fa>.\u003C\u002Fp>\n\n\n\n\u003Cp>However, before the pandemic, healthcare workers were already afflicted by another epidemic: \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fartificial-intelligence-could-solve-alarm-fatigue-in-hospitals\u002F\" target=\"_blank\">alarm fatigue\u003C\u002Fa>. It refers to the point when caregivers become desensitised to alarm signs and can miss those that actually need medical attention. Caregivers can experience as many as \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.ncbi.nlm.nih.gov\u002Fpmc\u002Farticles\u002FPMC4206416\u002F\" target=\"_blank\">187 alarms per bed per day\u003C\u002Fa>; but \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fnurse.org\u002Farticles\u002Falarm-fatigue-statistics-patient-safety\u002F\" target=\"_blank\">up to 99% alarms\u003C\u002Fa> from patient beds can be false ones. But an artificial intelligence-based solution \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fartificial-intelligence-could-solve-alarm-fatigue-in-hospitals\u002F\" target=\"_blank\">can help reduce such notifications\u003C\u002Fa> received by the caregivers by up to 99.3%; enabling them to only hear those which require medical assistance. As the recipient of healthcare services, they are better placed to identify which areas need improvements.\u003C\u002Fp>\n\n\n\n\u003Cp>Take, for example, the Oral and Maxillofacial Surgery Department of Radboud University Medical Centre Nijmegen in the Netherlands. They \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fpeek-into-the-future-of-hospitals\u002F\" target=\"_blank\">redesigned the whole department’s rooms\u003C\u002Fa> based on patients’ suggestions. This involved simple yet important changes that enabled a more balanced patient-doctor relationship and friendly atmosphere. They prioritised round tables over square ones for more friendly conversations and also suggested brightly lit rooms with friendly colors. Knowing the medical reality, he inquired whether those existing computers and accompanying systems would allow the use of advanced technologies such as artificial intelligence (AI). Practically, he wanted to know how I think existing hardware should be upgraded to make it ready for AI.\u003C\u002Fp>\n\n\n\n\u003Cp>To answer him, I made a seemingly unrelated analogy coming from the video game industry: cloud gaming. This approach, where the cloud handles all the processing power, has gained traction; bypassing the need for powerful PCs and consoles altogether. I told him that I believe \u003Cstrong>the same fate awaits medical professionals and hospitals with limited IT infrastructure that still want to benefit from power-hungry technologies like AI\u003C\u002Fstrong>.\u003C\u002Fp>\n\n\n\n\u003Cp>And by now, \u003Ca href=\"https:\u002F\u002Fwww.healthcareitnews.com\u002Fnews\u002Fgoogle-cloud-teams-carecloud-bring-generative-ai-small-practices\" target=\"_blank\" rel=\"noreferrer noopener\">reality has caught up\u003C\u002Fa> with this prediction: CareCloud already uses Google Cloud to help power its own operational needs and enable generative AI and search functionality in its products.\u003C\u002Fp>\n\n\n\n\u003Cp>By using Google&#8217;s offerings such as &nbsp;Vertex AI and Generative AI App Builder, the company is developing tools to bring physicians at smaller practices the same AI-enabled capabilities that are increasingly available at large hospitals and health systems.&#8221;\u003C\u002Fp>\n\n\n\n\u003Cp>Physicians working in smaller practices could ask generative AI more complex clinical questions and gain deeper insights with evidence-based recommendations derived from analyzing datasets.\u003C\u002Fp>\n\n\n\n\u003Cp>And it all makes sense. Rather than upgrading physical hardware, cloud computing technology can upgrade healthcare institutions, and provide access to advanced technologies; while circumventing the need for physical health IT upgrades. Such an approach has not been explored at large, but it holds promise for the future of healthcare as we shall see in this article.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>From the moons of Mars to the cloud\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>For the majority of its existence, the video game industry relied on consumer hardware. The bigger the storage, the better the graphics cards and the faster the processors that gamers’ PCs and consoles could pack, the superior performance they would get, allowing them to play the latest games with more features. This bigger-better-faster approach extended past the gaming machines to the games themselves. The latest instalment in the series released in 2020, \u003Cem>DOOM Eternal\u003C\u002Fem>, boasts better visuals, and lets you roam the surface of Mars but has a \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Ftwinfinite.net\u002F2020\u002F03\u002Fdoom-eternal-install-size\u002F\" target=\"_blank\">file size of up to 50 GB\u003C\u002Fa>.\u003C\u002Fp>\n\n\n\n\u003Cp>However, in recent years, video games have increasingly bypassed storage media hardware altogether in favour of cloud storage. No need for physical copies, your games follow you wherever there’s an internet connection. Platforms like Epic Games Store and Steam serve as distributors for these digital versions of games, and digital sales from such platforms first \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fscreenrant.com\u002Fdigital-game-sales-consoles-outnumber-physical-first-time\u002F\" target=\"_blank\">outsold physical sales\u003C\u002Fa> in 2020.\u003C\u002Fp>\n\n\n\n\u003Cp>Now, even gaming systems are going digital with cloud technology. With services like \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fstadia.google.com\u002F\" target=\"_blank\">Google Stadia\u003C\u002Fa> and \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.amazon.com\u002Fluna\u002Flanding-page\" target=\"_blank\">Amazon Luna\u003C\u002Fa>, \u003Cstrong>gamers don’t need high-end PCs or new consoles to play the latest games, which would traditionally require such devices\u003C\u002Fstrong>. In fact, with a high-speed internet connection, they can play those demanding games like \u003Cem>DOOM Eternal\u003C\u002Fem>, on a supported, yet underpowered device they bought a couple of years ago. All this is made possible thanks to the power of the cloud, which can similarly give a boost to existing healthcare infrastructure.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Cloud magic\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Cloud gaming works through \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fazure.microsoft.com\u002Fen-us\u002Foverview\u002Fwhat-is-cloud-computing\u002F\" target=\"_blank\">cloud computing technology\u003C\u002Fa>. Rather than having the user’s computer handle all the processing to play a demanding game, it’s done remotely via servers which provide the computing power to the user and relay it via the net, or “the cloud.” It’s basically having powerful, distant PCs do the computing, while you benefit from the same software output on an underpowered system. You might even think of it as a beefed-up Netflix. Cloud gaming subscribers choose the game they want to play on a supported device anywhere with a robust connection and the servers will run the game while the player remotely controls it.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>It’s particularly handy for those who want to play a new game but can’t afford to buy a new console to play it on. They just use their cloud subscription on a device they already own and \u003Cem>voilà\u003C\u002Fem>, new software on older hardware. The continent is already \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fafrica-hotspot-digital-health\u002F\" target=\"_blank\">a hotspot for digital health\u003C\u002Fa>, but the infrastructure might limit the adoption of more advanced and helpful technologies like AI. These require newer systems to operate, which might not be so widely available. \u003Cstrong>So, instead of fitting medical institutions with costly, top-of-the-line computers to run those, how about outsourcing the processing power and providing it on demand?&nbsp;\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>This is what’s possible with cloud computing, and it circumvents the whole health IT upgrade process in healthcare institutions with limited IT infrastructure. The technology isn’t totally foreign to healthcare; systems already employ cloud technology from \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Faws.amazon.com\u002Fhealth\u002F\" target=\"_blank\">Amazon Web Services\u003C\u002Fa> and \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fazure.microsoft.com\u002Fen-us\u002Findustries\u002Fhealthcare\u002F\" target=\"_blank\">Microsoft Azure\u003C\u002Fa>. This helps develop chatbots and health platforms to more securely manage the increased data capacity that comes with the digitisation of healthcare. In fact, health systems \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.businessinsider.com\u002Fmicrosoft-bringing-cloud-ai-tech-to-clinicians-2019-7\" target=\"_blank\">spent almost $40 billion in 2022\u003C\u002Fa> on cloud computing. However, these haven&#8217;t been employed at a large scale for demanding technologies like AI.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>AI enters the game\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>In September 2020, a first step towards such an offering was made. Google Cloud partnered with the U.S. Department of Defense (DoD) to \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.mobihealthnews.com\u002Fnews\u002Fgoogle-cloud-provide-dod-ai-enabled-cancer-diagnostic-system\" target=\"_blank\">develop an AI-based digital pathology system\u003C\u002Fa>. It involved augmented reality microscopes to help physicians, assisted by an AI to improve the accuracy of cancer diagnoses.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>While this is just one example, potentials of the technology in healthcare \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002F7-things-you-can-expect-from-a-i-in-healthcare\u002F\" target=\"_blank\">are manifold\u003C\u002Fa>. These range from optimising logistics to digesting medical and research data at unprecedented rates to assist professionals in decision-making. All this will be possible without upgrading the local IT infrastructure.\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Before elevating healthcare to the cloud\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Just like gamers are now buying affordable processing power from the cloud to play high-performing games without spending much more on newer hardware, similar possibilities open up for healthcare institutions. But while hardware might not need upgrades, other factors will determine its successful implementation.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Similar to cloud gaming services, a high-speed internet connection will be a must, in particular for demanding AI processes\u003C\u002Fstrong>. The WHO article \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.frontiersin.org\u002Farticles\u002F10.3389\u002Ffpubh.2019.00341\u002Ffull#B10\" target=\"_blank\">further notes\u003C\u002Fa> that unstable and expensive internet connectivity and unstable power supply can’t sustain advanced technologies.
