However, with digital health technologies that democratise access to care, the point of care will not be limited to healthcare institutions. It will rather be split into two; with hospitals becoming health centres for disease prevention, acute care and surgical needs, while monitoring patients’ vitals will be done closer to patients themselves.\u003C\u002Fp>\n\n\n\n\u003Cp>We explore these aspects of the future of hospital design in this two-part series. In the \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-future-of-hospital-design-inside-the-point-of-care\" target=\"_blank\" rel=\"noreferrer noopener\">first article\u003C\u002Fa>, we looked at design adjustments to accommodate digital health technologies to enhance the workplace of caregivers while providing a better patient experience. In this second article, we will look at how hospitals will also need to adapt their designs from outside the institutions.\u003C\u002Fp>\n\n\n\n\u003Cp>For deeper insights into the subject, we turned to Diana Anderson, MD, M.Arch, and Principal of \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.jacobs.com\u002Fdelivery\u002Farchitecture\" target=\"_blank\">Jacobs\u003C\u002Fa>, together with Cath Lewin RN, BN, senior health planner at Jacobs. Interviewing them for our \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.patreon.com\u002Fthemedicalfuturist\u002Fposts\" target=\"_blank\">Patreon site\u003C\u002Fa>, they provided valuable insights to redesign healthcare from outside the point of care we also wanted to share here.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Ambulatory medicine transporting care outside of hospitals\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>One way medical services are already brought to the point of care is through \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Ffuture-emergency-medicine-innovations-making-patients-point-care\u002F\" target=\"_blank\">emergency medicine\u003C\u002Fa>; and in particular via ambulance services. Through these, first responders perform life-saving procedures where patients are. \u003C\u002Fp>\n\n\n\n\u003Cp>Already, the largest public hospital system of the U. S., NYC Health + Hospitals, enables the use of \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.bloomberg.com\u002Fnews\u002Farticles\u002F2021-02-23\u002Fhow-the-pandemic-is-transforming-hospital-design\" target=\"_blank\">telemedicine services\u003C\u002Fa> by emergency medicine staff to treat patients on the spot. However, they can be further assisted by fitting ambulances with portable diagnostic devices. Abbott’s \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.pointofcare.abbott\u002Fus\u002Fen\u002Fofferings\u002Fistat\u002Fistat-handheld\" target=\"_blank\">i-STAT blood analyser\u003C\u002Fa> can make hours-long waits for laboratory blood test results a thing of the past. It allows caregivers to evaluate the patient’s blood sample on the spot and wirelessly transmit the results to colleagues. Another handheld device is the FDA-approved \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Ffirst-medical-tricorder-viatom-checkme-pro-review\u002F\" target=\"_blank\">Viatom CheckMe Pro\u003C\u002Fa>. It measures a number of vital signs from ECG through blood oxygen level to body temperature. \u003C\u002Fp>\n\n\n\n\u003Cp>Equipping first responders with such tools will save valuable time. Thereby they can perform more tests and deliver appropriate care at the point-of-care. As Dr. Anderson told The Medical Futurist, healthcare of the future should occur across spatial scales; to include the micro-level of our homes and communities all the way up to the macro scale of the hospital campus within the urban context. And one space we can already conquer is emergency medicine, which already bridges hospitals and patients.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Patients as the point-of-care\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>As discussed in the previous section, portable diagnostic devices bring data to the point-of-care; but patients themselves can adopt personal health sensors and wearables to make more informed decisions about their health.\u003C\u002Fp>\n\n\n\n\u003Cp>A smartwatch embedded with an ECG monitor like the Apple Watch can monitor their cardiac activity and \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fcan-your-smartwatch-send-you-to-the-hospital\u002F\" target=\"_blank\">notify them\u003C\u002Fa> of suspicious signs. Others could use the FDA-cleared \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fbiointellisense.com\u002F\" target=\"_blank\">BioSticker\u003C\u002Fa> smartpatch to continuously monitor vital signs like respiratory rate, activity levels, sleep status, gait and more. Subsequently, they can send the data to their physician to evaluate their health remotely. They can serve as guides for patients adopting such tools. Physicians can instruct patients regarding how to analyse the metrics measured and when to seek medical attention.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Redesigning telemedicine for the receiving end\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>In the \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-future-of-hospital-design-inside-the-point-of-care\" target=\"_blank\" rel=\"noreferrer noopener\">first article\u003C\u002Fa> of this series, we discussed the increasing importance of telemedicine; and how designers must factor in the practice when planning future hospitals. “\u003Cem>As a result of COVID-19 and funding changes, virtual visits have now become more accepted by providers and consumers,\u003C\u002Fem>” said Dr. Anderson.\u003C\u002Fp>\n\n\n\n\u003Cp>Already, \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedcitynews.com\u002F2021\u002F03\u002Ftelehealth-is-ready-for-its-second-dose-introducing-virtual-care-2-0\u002F\" target=\"_blank\">95% of providers\u003C\u002Fa> are willing to use telemedicine. This indicates a trend that telemedicine is on track to become the new norm. To accommodate this need, Diana Anderson suggests “\u003Cem>dedicated spaces for virtual consultations. It should also be applied on the receiving end – or the patients’ side. The latter could perform minor adjustments on their own; such as using a room with good internet connection and proper lighting. While minor, they also make the experience better for physicians consulting those patients. In particular, chronic care patients could heed to the advice; as even cancer patients continued their treatment \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.timesrecordnews.com\u002Fstory\u002Fnews\u002Flocal\u002F2020\u002F04\u002F11\u002Ftelehealth-help-cancer-patients-continue-treatment\u002F5125094002\u002F\" target=\"_blank\">thanks to telemedicine\u003C\u002Fa> during the pandemic. \u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Democratising access to care\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Ultimately, such redesigning of hospitals from the inside out aims to achieve a core principle of digital health: to democratise access to care. With health metrics obtained from wearables, patients are more empowered in managing their own health and can engage in an equal-level partnership with their physicians.\u003C\u002Fp>\n\n\n\n\u003Cp>But while medical-grade tools become available at the comfort of one’s home, hospitals themselves won’t be obsolete. Rather, they could focus their resources on acute care treatment, surgical units, laboratories and imaging centres. Dr. Anderson and Cath Lewin share a similar view, seeing such high-tech facilities to be adequate for ‘very sick’ patients. “\u003Cem>Those who are not critically ill could be supported in more expansive home-based care models and community programs, she suggests. In this model, virtual care will play a key role in the monitoring of chronic conditions, prevention and wellness\u003C\u002Fem>.” \u003C\u002Fp>\n\n\n\n\u003Cp>They also added a utopistic concept. “P\u003Cem>erhaps our homes could contain a designated ‘health space’ or room\u003C\u002Fem>;\u003Cem> so that as we progress through life stages, we maintain our connectivity to care.\u003C\u002Fem>”\u003C\u002Fp>\n\n\n\n\u003Chr class=\"wp-block-separator\"\u002F>\n\n\n\n\u003Ch5 class=\"wp-block-heading\">As for the telemedicine service or app itself, a feature-rich and user-friendly interface could help. COVID-19 became the monster under the bed – and it’s likely to stay. No wonder these kids are already being dubbed as \u003Cstrong>Generation Q – as in quarantine\u003C\u002Fstrong>.\u003C\u002Fp>\n\n\n\n\u003Cp>COVID-19 has put a spotlight on existing problems that some call \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fedition.cnn.com\u002F2021\u002F01\u002F04\u002Fhealth\u002Fmental-health-during-covid-19-2021-stress-wellness\u002Findex.html\" target=\"_blank\">the biggest pandemic issue\u003C\u002Fa> we face today: mental health. \u003C\u002Fp>\n\n\n\n\u003Cp>The fears implanted by the virus are going to be embedded into this young generation. We can’t even begin to think about how it will turn out when they grow up. Will they be more fearful? Less independent, less brave, or right the opposite, will they become more resilient? \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.sciencedirect.com\u002Fscience\u002Farticle\u002Fpii\u002FS0278584620304875\" target=\"_blank\">Studies are in the making\u003C\u002Fa>, but what is certain is that this generation has way too much on their shoulder. \u003C\u002Fp>\n\n\n\n\u003Cp>A \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.washingtonpost.com\u002Fhealth\u002Fcoronavirus-is-harming-the-mental-health-of-tens-of-millions-of-people-in-us-new-poll-finds\u002F2020\u002F04\u002F02\u002F565e6744-74ee-11ea-85cb-8670579b863d_story.html\" target=\"_blank\">worldwide scale\u003C\u002Fa> of anxiety, loneliness and fear that had been there before added up, and investigation found rise in school safeguarding reports, \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.independent.co.uk\u002Fnews\u002Fuk\u002Fhome-news\u002Feating-disorder-rise-pandemic-lockdown-b1798387.html\" target=\"_blank\">eating disorders\u003C\u002Fa> and \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.theguardian.com\u002Fsociety\u002F2020\u002Foct\u002F21\u002Ffigures-lay-bare-toll-of-pandemic-on-uk-childrens-mental-health\" target=\"_blank\">sleeping pill prescriptions\u003C\u002Fa>. Parents struggle to get help, and, as a social worker in the UK \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.irishtimes.com\u002Flife-and-style\u002Fhealth-family\u002Fparenting\u002Flocked-down-and-vulnerable-it-s-the-kids-we-don-t-know-about-that-is-the-worry-1.4468231\" target=\"_blank\">put it\u003C\u002Fa> “\u003Cem>everybody feels anxious at the moment. You don’t want that to develop into anything more sinister for a young person\u003C\u002Fem>.” And without help, these kids have the chance to deteriorate. \u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Help is hard to come by\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Despite the fact that governments know of the impending issue of children being unable to cope with the aftermaths of the pandemic, with vaccinations and economic struggles on their minds, these issues are most likely not on the top of their agenda. Small-scale solutions are, however, popping up. Even though they don’t reach too many children (\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.usnews.com\u002Fnews\u002Fbest-states\u002Flouisiana\u002Farticles\u002F2021-02-13\u002Flouisiana-teacher-writes-childrens-book-about-covid-19\" target=\"_blank\">like this teacher\u003C\u002Fa> who wrote a children’s book about COVID-19 while in quarantine), they are steps into the right direction.\u003C\u002Fp>\n\n\n\n\u003Cp>“\u003Cem>By April last year, the pandemic had disrupted education for around 90 per cent of the world’s school-age children. An unprecedented 1.4 billion students were shut out of schools in 192 countries\u003C\u002Fem>” – wrote advocacy NGO Human Rights Watch \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.hrw.org\u002Fnews\u002F2021\u002F01\u002F25\u002Fcovid-19-school-closures-inaction-fuel-childrens-loss-education\" target=\"_blank\">in a report\u003C\u002Fa>. But that didn’t always turn out right.\u003C\u002Fp>\n\n\n\n\u003Cp>As UNICEF pointed out, “\u003Cem>while technology and digital solutions provide significant opportunities to keep children learning, entertained and connected, these same tools may also increase their exposure to a myriad of risks. That is why a new organisation called the European Children’s Hospitals Organisation (\u003Ca href=\"https:\u002F\u002Fwww.echohospitals.org\u002F\">ECHO\u003C\u002Fa>), representing leading paediatric hospitals across Europe, calls on children’s hospitals and public health systems to ensure that the rights of children are central in the new normal of COVID-19. \u003C\u002Fp>\n\n\n\n\u003Cp>As they mentioned in their study in \u003Ca href=\"https:\u002F\u002Fwww.thelancet.com\u002Fjournals\u002Flanchi\u002Farticle\u002FPIIS2352-4642(20)30300-X\u002Ffulltext\">The Lancet\u003C\u002Fa>, “\u003Cem>as anchors of paediatric care in our communities, children’s hospitals have a moral responsibility to ensure that the rights of children are protected and promoted in pandemic response planning. Their project, \u003Ca href=\"https:\u002F\u002Fwww.tdh.ch\u002Fsites\u002Fdefault\u002Ffiles\u002Fcovidunder19_thematic_summaries_english_20201209.pdf.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">#CovidUnder19: Life Under Coronavirus\u003C\u002Fa> is an initiative to meaningfully involve children in responses to the COVID-19 pandemic. 26,258 children from 137 countries responded to the survey – and the results are not surprising. \u003C\u002Fp>\n\n\n\n\u003Cblockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\u003Cp>\u003Cem>“I’m waiting when school will reopen so that I don’t need to work,”\u003C\u002Fem> \u003C\u002Fp>\u003Ccite>said a 13-year-old girl from India.\u003C\u002Fcite>\u003C\u002Fblockquote>\n\n\n\n\u003Cp>The pandemic had a major impact on children’s experiences and rights. Beside feeling pressured from school tasks, respondents also spoke about missing physical closeness and hugging their family and friends, missing out on key milestones such as birthdays and graduation.\u003C\u002Fp>\n\n\n\n\u003Cp>As the survey also \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.newswise.com\u002Fcoronavirus\u002Fsurvey-finds-pandemic-had-a-major-impact-on-children-s-experiences-and-rights\u002F?article_id=743054\" target=\"_blank\">served as a channel\u003C\u002Fa> for children around the world to portray their thoughts and feelings about the pandemic to the world, they took the chance and put forward a couple of suggestions to governments.
