So let’s see where we stand in this regard!\u003C\u002Fp>\n\n\n\n\u003Cp>This analysis is specifically about cognitive health, and we will not include mental health or emotional health.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>To put it very simply, cognitive health is how well you think, learn, and remember. This ability of our brains makes us capable of decision-making and maintaining focus. It encompasses the overall functioning and health of the mind, crucial for navigating everyday life and responding to its challenges.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>The most often occurring diseases\u002Fconditions damaging it are Alzheimer&#8217;s disease, dementia, stroke, Parkinson&#8217;s disease, depression, and multiple sclerosis. And of course, traumatic brain Injuries from the physical side. \u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Environmental factors can damage, reading and games can maintain cognitive health\u003C\u002Fh2>\n\n\n\n\u003Cp>According to the American \u003Ca href=\"https:\u002F\u002Fwww.nia.nih.gov\u002Fhealth\u002Fbrain-health\u002Fcognitive-health-and-older-adults#reduce%20risks\" target=\"_blank\" rel=\"noreferrer noopener\">National Institute of Aging\u003C\u002Fa>, the most important environmental (as opposed to genetic) factors that can damage our cognitive health are:&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cul class=\"wp-block-list\">\n\u003Cli>Some physical and mental health problems, such as high blood pressure or depression\u003C\u002Fli>\n\n\n\n\u003Cli>Brain injuries, such as those due to falls or accidents\u003C\u002Fli>\n\n\n\n\u003Cli>Some medicines, or improper use of medicines\u003C\u002Fli>\n\n\n\n\u003Cli>Lack of physical activity\u003C\u002Fli>\n\n\n\n\u003Cli>Poor diet\u003C\u002Fli>\n\n\n\n\u003Cli>Smoking\u003C\u002Fli>\n\n\n\n\u003Cli>Drinking too much alcohol\u003C\u002Fli>\n\n\n\n\u003Cli>Sleep problems\u003C\u002Fli>\n\n\n\n\u003Cli>Social isolation and loneliness\u003C\u002Fli>\n\u003C\u002Ful>\n\n\n\n\u003Cp>We have already written about \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fhow-to-improve-your-cognitive-health\u002F\" target=\"_blank\">how we can maintain our brain health\u003C\u002Fa>. Good practices include daily brain exercises (the most efficient of which \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fpsychcentral.com\u002Fhealth\u002Fbrain-exercises\" target=\"_blank\">is learning a foreign language\u003C\u002Fa>, but you can see other examples as well, like playing bridge, go, or sudoku). Also, \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.bustle.com\u002Fp\u002Fwhy-reading-is-the-best-workout-for-your-brain-57441\" target=\"_blank\">reading every day\u003C\u002Fa> is proven to be efficient. \u003C\u002Fp>\n\n\n\n\u003Cp>Games are great tools to kill two birds with one stone. Gameplay focuses and controls our attention, taps into our innate strengths, thrills us utterly, and compels us to greater resilience in the attainment of more powerful and useful skills. That’s why gamified apps (like \u003Ca href=\"https:\u002F\u002Fwww.lumosity.com\u002Fen\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">Lumosity\u003C\u002Fa>, \u003Ca href=\"https:\u002F\u002Felevateapp.com\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">Elevate\u003C\u002Fa>, or \u003Ca href=\"https:\u002F\u002Fwww.cognifit.com\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">CogniFit\u003C\u002Fa>) are widely used for improving and maintaining cognitive abilities.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Existing digital health technologies for cognitive health\u003C\u002Fh2>\n\n\n\n\u003Cp>Cognitive disorders are not just extremely scary from a personal point of view, these are also major global health concerns. \u003Ca href=\"https:\u002F\u002Fpubmed.ncbi.nlm.nih.gov\u002F36821951\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">According to this study\u003C\u002Fa>, digital healthcare technologies offer ways to manage and slow down the progression of conditions like dementia and mild cognitive impairment. However, choosing the right technology is difficult because there&#8217;s no comprehensive review that covers the various types of digital technology for cognitive impairment, including their effects and limitations.\u003C\u002Fp>\n\n\n\n\u003Cp>The goal of the study was to identify different types of digital health technologies used for dementia and mild cognitive impairment and evaluate how the results are measured and aligned with their intended purposes.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cfigure class=\"wp-block-image size-large\">\u003Cimg loading=\"lazy\" decoding=\"async\" width=\"768\" height=\"432\" src=\"https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2023\u002F11\u002Ftmf_article_391-768x432.png\" alt=\"\" class=\"wp-image-53653\" srcset=\"https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2023\u002F11\u002Ftmf_article_391-768x432.png 768w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2023\u002F11\u002Ftmf_article_391-370x208.png 370w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2023\u002F11\u002Ftmf_article_391-1536x864.png 1536w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2023\u002F11\u002Ftmf_article_391-2048x1152.png 2048w\" sizes=\"auto, (max-width: 768px) 100vw, 768px\" \u002F>\u003C\u002Ffigure>\n\n\n\n\u003Cp>A total of 135 articles were selected. For people with mild cognitive impairment, technologies focused on predicting and diagnosing dementia were explored. For those with dementia, technologies aimed at preventing health deterioration and inducing significant improvement were considered. Studies for caregivers focused on monitoring and daily living assistive technologies to reduce the burden of care. However, challenges like data collection, storage, safety, and the digital divide still exist, varying in intensity depending on the type of technology.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">The wonderful world of brain-computer interfaces\u003C\u002Fh2>\n\n\n\n\u003Cp>These devices actually have a marginal effect on cognitive health, but still, we can’t leave them out, as they can help individuals with severe cognitive or motor impairments to communicate and control their environment &#8211; so they will turn the world over for people with these conditions.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Some of these are already in more advanced phases and have proven their capabilities, like \u003Ca href=\"https:\u002F\u002Fsynchron.com\u002Fresearch\" target=\"_blank\" rel=\"noreferrer noopener\">Synchron’s implant\u003C\u002Fa>. Their interface was already \u003Ca href=\"https:\u002F\u002Fjamanetwork.com\u002Fjournals\u002Fjamaneurology\u002Farticle-abstract\u002F2799839\" target=\"_blank\" rel=\"noreferrer noopener\">tried with four patients\u003C\u002Fa> a few years ago and now they are moving on with completing the patient enrollment for this \u003Ca href=\"https:\u002F\u002Fwww.clinicaltrialsarena.com\u002Fnews\u002Fsynchron-bci-clinical-trial\u002F?cf-view\" target=\"_blank\" rel=\"noreferrer noopener\">feasibility study\u003C\u002Fa>. The trial has enrolled six patients with severe quadriparesis and has three active clinical trial sites in the US. \u003Ca href=\"https:\u002F\u002Fspectrum.ieee.org\u002Fneuralink-human-trials\" target=\"_blank\" rel=\"noreferrer noopener\">Other ventures\u003C\u002Fa> seem to be a bit \u003Ca href=\"https:\u002F\u002Fwww.theverge.com\u002F2023\u002F9\u002F20\u002F23882888\u002Felon-musk-brain-implant-startup-neuralink-monkeys-euthanized\" target=\"_blank\" rel=\"noreferrer noopener\">more worrisome\u003C\u002Fa>.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Early diagnostics may improve our odds\u003C\u002Fh2>\n\n\n\n\u003Cp>We see a growing amount of evidence that early detection of various conditions (like Alzheimer’s or Parkinson’s disease) can improve patients’ conditions. New technologies and methods, like dedicated AI algorithms, are great allies in that.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Although these are not strictly our subject, as these look for biomarkers, we have seen several promising announcements in the past years, like \u003Ca href=\"https:\u002F\u002Fwww.futurity.org\u002Fparkinsons-disease-smartwatches-2907442\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">smartwatches potentially detecting early Parkinson’s\u003C\u002Fa> or the \u003Ca href=\"https:\u002F\u002Fwww.frontiersin.org\u002Farticles\u002F10.3389\u002Ffnagi.2023.1123456\u002Ffull\" target=\"_blank\" rel=\"noreferrer noopener\">many studies\u003C\u002Fa> and reports about \u003Ca href=\"https:\u002F\u002Fwww.nei.nih.gov\u002Fabout\u002Fnews-and-events\u002Fnews\u002Froutine-eye-scans-may-give-clues-cognitive-decline-diabetes\" target=\"_blank\" rel=\"noreferrer noopener\">what eye scans can do\u003C\u002Fa> to \u003Ca href=\"https:\u002F\u002Fglobalnews.ca\u002Fnews\u002F9840571\u002Fai-alzheimers-disease-detection\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">predict such conditions\u003C\u002Fa>, often \u003Ca href=\"https:\u002F\u002Fwww.medicalnewstoday.com\u002Farticles\u002Fcan-an-eye-scan-predict-parkinsons-disease\" target=\"_blank\" rel=\"noreferrer noopener\">many years before\u003C\u002Fa> traditional diagnostics. And thus, we can’t just simply leave them out without a word.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">There is an app, some VR glasses, and a robot for that!\u003C\u002Fh2>\n\n\n\n\u003Cp>Many facets of cognitive help are supported by various apps and other devices, here are a few interesting examples.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Cognitive training apps &#8211; as we mentioned &#8211; are designed to enhance cognitive functions such as memory, problem-solving skills, and attention. They often include a range of games and tasks tailored to the user&#8217;s needs. It can be used to stimulate memory, attention, and problem-solving skills among others.\u003C\u002Fp>\n\n\n\n\u003Cp>And let’s not forget about our metallic friends! \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fsocial-robots-are-robotic-pets-any-worse-than-the-real-ones\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">Social robots\u003C\u002Fa> can also be useful in preventing decline: by providing companionship and cognitive stimulation for individuals, especially those in care homes, they can assist the elderly. They can engage in conversation, play games, and provide reminders for medication and appointments.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">AI for the rescue!\u003C\u002Fh2>\n\n\n\n\u003Cp>Artificial intelligence shows potential in many areas of saving\u002Fhelping\u002Frestoring our cognitive abilities.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.clinicsource.com\u002Fimpact-of-ai-on-speech-and-language-therapy\u002F\" target=\"_blank\">Speech and language therapy algorithms\u003C\u002Fa> are AI-driven tools that can help individuals who have lost certain speech or cognitive capabilities by providing personalised and adaptive language exercises.\u003C\u002Fp>\n\n\n\n\u003Cp>We can also use \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.ncbi.nlm.nih.gov\u002Fpmc\u002Farticles\u002FPMC9881033\u002F#:~:text=Conclusion%3A,in%20a%20population%2Dbased%20setting.\" target=\"_blank\">machine learning for early detection\u003C\u002Fa>: AI algorithms can analyse medical data to predict and detect cognitive decline early, allowing for early intervention strategies that can slow down the progression of diseases like Alzheimer&#8217;s.\u003C\u002Fp>\n\n\n\n\u003Cp>In the future, AI will also be able to help in designing personalised treatment plans based on the patient&#8217;s genetic makeup, lifestyle, and progression of the disease, potentially improving outcomes.\u003C\u002Fp>\n\n\n\n\u003Cp>These technologies are not without challenges, including ensuring privacy, managing data security, and providing equitable access. Furthermore, the efficacy of these technologies often depends on individual circumstances and may require integration with traditional therapeutic approaches. Being the most efficient and accurate method to edit a cell’s genome, CRISPR \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fwhat-could-crisprcas9-do-tomorrow\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">holds potentials\u003C\u002Fa> that range from treating conditions such as HIV to finding new drug targets.\u003C\u002Fp>\n\n\n\n\u003Cp>While such potentials \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.the-scientist.com\u002Fnews\u002Fnew-crispr-treatment-could-prevent-hiv-reinfection-after-viral-dna-excision-71284\" target=\"_blank\">are real\u003C\u002Fa> and are being \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.drugtargetreview.com\u002Farticle\u002F106588\u002Ffive-recent-crispr-drug-target-discoveries\u002F\" target=\"_blank\">actively investigated\u003C\u002Fa>, you might be curious about more practical examples of CRISPR applications. By taking the \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.clinicaltrials.gov\u002Fsearch?intr=CRISPR%2FCas9\" target=\"_blank\">US Clinical Trials registry\u003C\u002Fa> as an example, we consider listed clinical trials that involve CRISPR technology as of late November 2023. Such an undertaking can provide a better appreciation of the state of CRISPR potential, as well as practical use cases that we can expect in the coming years.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Clinical trials involving CRISPR&nbsp;\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>For this analysis, we looked at the CRISPR entries on \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fclinicaltrials.gov\u002F\" target=\"_blank\">ClinicalTrials.gov\u003C\u002Fa>, which is an online database of clinical research studies and shares information about their results. This database is maintained by the United States National Library of Medicine at the National Institutes of Health and holds registrations from over 444,000 trials from 221 countries. Completed CRISPR clinical trials\u003C\u002Fh3>\n\n\n\n\u003Cp>Considering that clinical trials can take \u003Ca href=\"https:\u002F\u002Fwww.cancerresearchuk.org\u002Fabout-cancer\u002Ffind-a-clinical-trial\u002Fhow-clinical-trials-are-planned-and-organised\u002Fhow-long-it-takes-for-a-new-drug-to-go-through-clinical-trials#:~:text=There%20is%20no%20typical%20length,a%20drug%20to%20be%20licensed.\" target=\"_blank\" rel=\"noreferrer noopener\">over 10 years to complete\u003C\u002Fa>, it is no surprise to see that from the database, the minority of the entries (only 6) are listed as complete. Most of them studied the safety and efficacy of potential new treatments involving CRISPR, whether it was modified cells or a new product targeting viral cells.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>For example, \u003Ca href=\"https:\u002F\u002Fwww.clinicaltrials.gov\u002Fstudy\u002FNCT02793856?intr=CRISPR%2FCas9&amp;aggFilters=status:com&amp;rank=3\" target=\"_blank\" rel=\"noreferrer noopener\">one completed Phase I clinical trial\u003C\u002Fa> involved CRISPR-edited T cells in patients with advanced non-small-cell lung cancer; with endpoints being the safety, feasibility and efficacy of these modified T cells. The results indicated that the clinical application of CRISPR gene-edited T cells is generally safe and feasible; while the therapeutic efficacy could be improved.\u003C\u002Fp>\n\n\n\n\u003Cp>Notably, half of the completed trials investigated interventions for advanced cancer types. The remaining involved either diabetes, viral eye infection or Kabuki syndrome (a rare genetic disorder).\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">2. Ongoing CRISPR clinical trials\u003C\u002Fh3>\n\n\n\n\u003Cp>The vast majority of the CRISPR clinical trials on the database, or 30 of them, are ongoing, whether they are actively recruiting or not. They are mostly investigating the safety of CRISPR-based therapies either for cancer treatment ranging from renal cell carcinoma to lymphomas or genetic disorders such as thalassemia and sickle cell disease.\u003C\u002Fp>\n\n\n\n\u003Cp>To illustrate one of the areas of focus, \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.clinicaltrials.gov\u002Fstudy\u002FNCT04208529?intr=CRISPR%2FCas9&amp;aggFilters=status:not%20rec%20act%20enr&amp;rank=25\" target=\"_blank\">one of the listed trials\u003C\u002Fa> is following up with patients who received CRISPR-modified hematopoietic stem infusion for either β-thalassemia or severe sickle cell disease. The aim is to determine the long-term safety and efficacy of the treatment. In particular, the researchers are interested in determining the occurrence of new or worsening conditions, as well as severe adverse events up to 15 years after the treatment.\u003C\u002Fp>\n\n\n\n\u003Cp>As for the remaining trials, one is studying the effects of CRISPR-modified pancreatic cells for Type 1 diabetic patients; and another trial is investigating a new CRISPR-based drug for HIV treatment.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">3. Terminated CRISPR clinical trials&nbsp;\u003C\u002Fh3>\n\n\n\n\u003Cp>15 of the entries are either labeled as terminated, suspended, withdrawn or have an unknown status. Nearly all of them investigated the treatment of cancers, but their outcomes will likely remain unknown considering the statuses of the trials. \u003C\u002Fp>\n\n\n\n\u003Cp>As there are some overlaps with conditions of focus in ongoing trials, the less positive outcomes of terminated trials do not mean that other therapies aren’t possible. It means the approach of a certain trial did not pan out as initially planned, which could be due to several causes from lack of funding to poor recruitment of participants. \u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">What to expect from CRISPR in healthcare\u003C\u002Fh2>\n\n\n\n\u003Cp>As the database indicates, CRISPR’s potential to devise new treatments is actively being investigated by researchers. In particular, we can expect a more significant impact of CRISPR in the potential treatment of cancers and genetic disorders, as the majority of trials currently have such a focus. Interest in such conditions is understandable considering that there are \u003Ca href=\"https:\u002F\u002Fwww.wcrf.org\u002Fcancer-trends\u002Fworldwide-cancer-data\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">over 18 million cancer cases worldwide\u003C\u002Fa> and \u003Ca href=\"https:\u002F\u002Fwww.sciencedirect.com\u002Fscience\u002Farticle\u002Fpii\u002FS1110863011000620\" target=\"_blank\" rel=\"noreferrer noopener\">2–5% of all live births\u003C\u002Fa> are affected by hereditary diseases.\u003C\u002Fp>\n\n\n\n\u003Cp>However, CRISPR-based treatments for less prevalent conditions are also gaining interest. For instance, the database lists \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.clinicaltrials.gov\u002Fstudy\u002FNCT03855631?intr=CRISPR%2FCas9&amp;aggFilters=status:com&amp;rank=4\" target=\"_blank\">a completed observational study\u003C\u002Fa> around gene therapy for Kabuki syndrome, a rare genetic condition that affects children at \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.ncbi.nlm.nih.gov\u002Fpmc\u002Farticles\u002FPMC9601850\u002F\" target=\"_blank\">an estimated frequency\u003C\u002Fa> of 1:32,000 to 1:86,000. Furthermore, upon completion of clinical trials, the treatment will need to get approval from relevant health authorities. This means more time before seeing marketable solutions for everyday patients.\u003C\u002Fp>\n\n\n\n\u003Cp>But we might be on the cusp of having CRISPR-based treatments become more widely available. The technology has been refined, with the potential to be more specific and applied for common diseases such as \u003Ca href=\"https:\u002F\u002Fwww.technologyreview.com\u002F2023\u002F01\u002F09\u002F1064857\u002Fcrispr-high-cholesterol-10-breakthrough-technologies-2023\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">high cholesterol\u003C\u002Fa>. In late November 2023, the world’s first CRISPR therapy \u003Ca href=\"https:\u002F\u002Fwww.nature.com\u002Farticles\u002Fd41587-023-00016-6\" target=\"_blank\" rel=\"noreferrer noopener\">was approved\u003C\u002Fa> by the UK’s regulator. Named Casgevy, the treatment is aimed at sickle cell disease and transfusion-dependent β-thalassemia.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>This is promising progress considering that CRISPR–Cas9 gene editing therapy was \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.science.org\u002Fdoi\u002F10.1126\u002Fscience.1225829\" target=\"_blank\">introduced 11 years ago\u003C\u002Fa>.
