However, that’s not only up to the physician but also the organization, because individual attempts might result in burnout symptoms. Adoption of A.I. could change the situation for the better in the future, as it would create space for doctors and nurses to spend more quality time with patients. The question is, are doctors ready for it?\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Medicating Albert, the plush armadillo\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Two years ago, my niece had to spend two weeks in\nhospital as she partially ruptured her Achilles-tendon when she landed unfortunately\nin the sand after a broad jump in her sports class at school. The crying and\nscared 12-year-old was brought to the emergency room by her parents, calmed\ndown and taken care of by a swarm of medical professionals buzzing around in\nher room almost every hour. They did not always come to see my niece – she was\nput in a hospital room shared with five other kids.\u003C\u002Fp>\n\n\n\n\u003Cp>One of them was 6-year-old Mark, who suffered a traffic accident when his parents were underway to bring him home from pre-school. One of his legs was in a plaster cast from his toes up to the thighs, and he was unspeakably sad that he wouldn&#8217;t be able to play football for a very long time. However, every time he looked at Albert, the plush armadillo, his very best friend in the hospital, he could cheer up a little. My niece told me that the nurses brought in medication for Mark as well as Albert every single day. Of course, the plush armadillo did not get “real” painkillers, but little candies, but he had his own medication box, and Mark was convinced that the nurses are taking the best of care of his armadillo, too.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Compassionate\ncare is paying attention to the needs of the others, listening to spoken or\nnoticing unspoken wishes, imagining the other person’s situation and expressing\nacts of empathy to lessen suffering, pain or distress. If you need to feed a\nplush armadillo with candies and invent a bedhead board for a toy animal to\nmake a patient feel better, than that’s compassionate care, but it could also\nbe the right tone of voice, a simple touch, a loving glance or just sitting on\nthe bed for a while to provide some company\u003C\u002Fstrong>. As \u003Ca href=\"https:\u002F\u002Fwww.kevinmd.com\u002Fblog\u002F2011\u002F10\u002Fcompassionate-care-crucial-component-care.html\">nurse\nand speaker, Joyce Hyam, penned it\u003C\u002Fa>, when you give compassion it will always\nbe remembered. When you receive it, you will always\nhold that memory in your heart. You can make a difference in someone’s life.\nCompassionate care is a crucial component of care. However, as they progress through medical school, stress, anxiety, competitiveness and lack of time often start to erode their levels of compassion. \u003C\u002Fp>\n\n\n\n\u003Cp>The same will continue on their professional journey: \u003Cstrong>the patient will “get lost” in the ocean of\nCT scans, biopsies, charts, diagnoses, and\nadministrative tasks. The doctor hides behind a pile of papers, badly designed\nIT interfaces, curses the monitor and alienates himself from the people in\nneed. The phenomenon is quite understandable: expressing compassion and\ncarrying the emotional burden together with patients is tough, energy-sucking\nand the environment gives no time for processing it\u003C\u002Fstrong>. It is “easier” to\nhandle 15 patients a day being shut down,\notherwise, the risk of burnout gets higher. Also, the doctor’s figure is\nusually cemented as someone with authority and respect with less inclination to\nshow signs of empathy and emotion in general. \u003C\u002Fp>\n\n\n\n\u003Cp>That’s entirely\nthe reverse in the case of the nurse. Since the era of Florence Nightingale,\nthe founder of modern nursing, \u003Ca href=\"https:\u002F\u002Fqualitysafety.bmj.com\u002Fcontent\u002F26\u002F12\u002F1023\">the profession was associated with an idea of doing\nGod’s work through attending to those in need\u003C\u002Fa> – which came into more and more contrast with the\nefficient, professional, “dispassionate healer”.\nNurses are regarded as the ones who will provide attention, deliver words of\nkindness and hold their hands when patients are in agony. \u003C\u002Fp>\n\n\n\n\u003Cp>The growing dichotomy between the need for the\n“surrogate parent” sitting at the bedside of patients and the\nperformance-seeking medical professional is at the core of present-day calls\nfor more compassionate care. No wonder –\nit’s expensive, but no one wants to pay\nfor the costs of compassionate care\u003C\u002Fstrong>. First, \u003Ca href=\"https:\u002F\u002Fqualitysafety.bmj.com\u002Fcontent\u002F26\u002F12\u002F1023\">staff may have to engage in emotionally charged\ninteractions\u003C\u002Fa>, which can\nleave them feeling psychologically exhausted and at risk of burnout. \u003C\u002Fp>\n\n\n\n\u003Cp>Second, for health professionals to be able to connect with patients and understand their unique situation while also addressing fundamental needs (e.g., hygiene, feeding) in a non-mechanistic manner, an appropriately resourced workplace is required. It’s not enough to be empathetic and compassionate on the individual level, the environment should provide a nurturing setting, too. Social norms, such as trust, concern, and empathy as well as leaders demonstrating the meaning of noticing, paying attention and compassion could provide that. A\n\u003Ca href=\"http:\u002F\u002Fjournals.lww.com\u002Facademicmedicine\u002FAbstract\u002F2011\u002F03000\u002FHumanism_at_Heart__Preserving_Empathy_in.24.aspx\">study\nof 209 medical students\u003C\u002Fa> at the Robert Wood Johnson Medical\nSchool found that the level of \u003Ca href=\"https:\u002F\u002Fwww.washingtonpost.com\u002Fnational\u002Fhealth\u002Fdoctors-often-struggle-to-show-compassion-while-dealing-with-patients\u002F2011\u002F05\u002F02\u002FAFiR8A5G_story.html?noredirect=on&amp;utm_term=.2a1d79ba8c9e\">empathy could be maintained\u003C\u002Fa> among third-year\nstudents who received specialized training. Also, \u003Cstrong>digital health technologies have some potential in aiding the efforts\nto teach compassion.\u003C\u002Fstrong>\u003Cstrong>\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>Researchers\nfrom \u003Ca href=\"https:\u002F\u002Fwww.sciencedaily.com\u002Freleases\u002F2017\u002F04\u002F170427091749.htm\">Medical\nCyberworlds Inc. and the University of Michigan conducted research with 421\nmedical students\u003C\u002Fa> about the use of virtual reality in\npatient-doctor communication. They used the Mpathic-VR\ntechnology that allowed users to talk with emotive, computer-based virtual\nhumans who can see, hear and react to them in real time. In spite of efforts for \u003C\u002Fstrong>\u003Cstrong>\u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Falgorithms-robots-mimic-empathy\">robots to mimic empathy\u003C\u002Fa>\u003C\u002Fstrong>\u003Cstrong>,\nthat will remain the territory of humans far into the future. What technology\ncan accomplish is the creation of space for compassionate care\u003C\u002Fstrong>. While the first wave of innovation in healthcare\nstarted to bring EHRs, digital health sensors, big data, genomic reports,\nwearables to care facilities – and the promise to take some burden off the\nshoulder of medical professionals, the process might be longer than expected.\nThe experience of the last years shows that care providers are even more\nburdened by administrative tasks, now also coupled with technological troubles\nand more extended working hours. \u003C\u002Fp>\n\n\n\n\u003Cp>In the long term, artificial\nnarrow intelligence has the\npotential to genuinely off-load the busy work of physicians. While compassionate care should be the basis of medicine,\nmany professionals, mostly doctors, are alienated from its expression – unlike\nnurses, for example\u003C\u002Fstrong>.\n\u003C\u002Fp>\n\n\n\n\u003Cp>As Yuval Noah Harari says in his latest book, \u003Ca href=\"https:\u002F\u002Fwww.goodreads.com\u002Fbook\u002Fshow\u002F38820046-21-lessons-for-the-21st-century\">21 Lessons For The 21\u003Csup>st\u003C\u002Fsup> Century\u003C\u002Fa>, that “\u003Cem>many\ndoctors focus almost exclusively on processing information: they absorb medical\ndata, analyze it, and produce a diagnosis. Nurses, in contrast, also need a good motor and emotional skills to give a painful injection, replace a bandage\nor restrain a violent patient. Hence, we will probably have an A.I. family\ndoctor on our smartphone decades before we have a reliable nurse robot.\u003C\u002Fem>”\u003C\u002Fp>\n\n\n\n\u003Cp>The passage sheds light on the fact that with the appearance of A.I., doctors will lose some tasks that are traditionally part of their jobs and should get better at others that were considered conventionally as the area of nursing before. Thus, \u003Cstrong>physicians have to get prepared to bring back their abilities to show empathy, compassion, paying attention and communicating with the patients. The shift already shows in the statistics. The US Bureau of Labor Statistics predicts that while jobs for doctors and surgeons will \u003Ca href=\"https:\u002F\u002Fwww.bls.gov\u002Fooh\u002Fhealthcare\u002Fphysicians-and-surgeons.htm\">rise\u003C\u002Fa> by 14 percent between 2014 and 2024, the top three direct-care positions – personal-care aide, home-health aide, and nursing assistant – are expected to grow by 26 percent.\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>For\nthat, as Livia Gershon writes \u003Ca href=\"https:\u002F\u002Faeon.co\u002Fessays\u002Fthe-key-to-jobs-in-the-future-is-not-college-but-compassion\">here\u003C\u002Fa>,\nwe seriously need to rethink our perspective towards jobs requiring social\nskills; as they are usually underpaid and undervalued. As care work and other\ntypes of labor containing mainly soft skills cannot contribute to the GDP\ngrowth as effectively as different types,\nthe global economic system cannot value it appropriately. \u003C\u002Fp>\n\n\n\n\u003Cp>We\nexpect that situation to change in the years to come. It featured the two best players in the history of the show, Ken Jennings, who had the longest unbeaten run of 74 winning appearances, and Brad Rutter, earner of the biggest prize of $3.25 million. Their opponent was a huge computer with over 750 servers and a cooling system stored at a location so as not to disturb the players. The room–sized machine was made by IBM and named after the company&#8217;s founder, Thomas J. Watson. It did not smile or show emotion, but it kept on giving good answers. At the end, \u003Ca href=\"http:\u002F\u002Fwww.ibm.com\u002Fsmarterplanet\u002Fus\u002Fen\u002Fibmwatson\u002F\" target=\"_blank\" rel=\"nofollow noopener noreferrer\">Watson\u003C\u002Fa> won the game with $77,147 leaving Rutter and Jennings with $21,600 and $24,000 respectively. It left the audience in shock and awe at the same time.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Cognitive computers have been developing rapidly\u003C\u002Fstrong> over the last few years following \u003Ca href=\"http:\u002F\u002Fwww.wired.com\u002F2014\u002F10\u002Ffuture-of-artificial-intelligence\" target=\"_blank\" rel=\"nofollow noopener noreferrer\">three technological breakthroughs\u003C\u002Fa>. One is cheap parallel computation due to a new kind of chip called a graphics processing unit (GPU). The second one is accessible big data due to massive databases, web cookies, wearable devices and decades of search results. In 1997, IBM&#8217;s supercomputer Deep Blue could beat Garry Kasparov, the reigning chess grand master that time. He said he could have performed better if he had access to the same databases as Deep Blue. So later, freestyle matches were organized in which supercomputers could play against human chess players assisted by AI (they were called human\u002FAI centaurs). Guess what! In 2014 in a&nbsp;Freestyle Battle, the AI chess players won 42 games, but centaurs won 53 games. The best potential pair is a human with technology.&nbsp;\u003Cstrong>This is the only balance that can lead to a positive future\u003C\u002Fstrong>&nbsp;with more and more disruptive innovations including ever-improving cognitive computing but an also ever-improving human intelligence and wisdom. It is not any different in healthcare. It took plenty of time until the stethoscope, the symbol of healthcare&nbsp;was accepted by the medical community back in the 19th century. The instrument was \u003Ca href=\"https:\u002F\u002Fen.wikipedia.org\u002Fwiki\u002FStethoscope#History\">invented by&nbsp;French physician René-Théophile-Hyacinthe Laënnec\u003C\u002Fa>, who published its description in 1819, but it took several decades until doctors actually started using it.\u003C\u002Fp>\n\n\n\n\u003Cp>I don&#8217;t expect any difference with A.I.&nbsp;After many of my talks, physician colleagues ask me whether artificial intelligence might replace them in their jobs and whether algorithms can eventually become better at making diagnoses. Both will happen but not the way they imagine it. \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fartificial-intelligence-will-redesign-healthcare\u002F\">Huge waves are coming to healthcare&nbsp;to transform the job of physicians\u003C\u002Fa> into something distinctly different than before. Although some of their tasks will be taken over by A.I., they will have more time for others, for example, deal with patients with real care and patience. Doctors will not have to&nbsp;struggle with being up-to-date in medical research, with administrative tasks, with consultation or making notes. As the amount of information they accumulate grows exponentially, the assistance of computing solutions in medical decisions is imminent. A.I. will open new dimensions for doctors on the personal level as well as for hospitals and other medical institutions on the structural level.\u003C\u002Fp>\n\n\n\n\u003Cp>On the institutional level, the most obvious use of A.I. will be data management.&nbsp;Collecting it, storing it, normalizing it, tracing its lineage – it is the first step in revolutionizing the existing healthcare systems. Recently, the AI research branch of the search giant, Google, launched its&nbsp;Google Deepmind Health&nbsp;project, which is used to mine the data of medical records in order to provide better and faster health services.\u003C\u002Fp>\n\n\n\n\u003Cp>It could also analyze entire healthcare systems. For example, 97 percent of healthcare invoices in the Netherlands are digital containing data regarding the treatment, the doctor, and the hospital. These invoices could be easily retrieved. A local company,&nbsp;\u003Ca href=\"https:\u002F\u002Fwww.zorgprismapubliek.nl\u002F\">Zorgprisma Publiek\u003C\u002Fa>&nbsp;analyzes the invoices and uses IBM Watson in the cloud to mine the data. A.I. assistants could also look up relevant medical information and keep doctors up-to-date in clinical research. IBM has the capacity to scan through millions of pages in seconds. Van Oijen, Ph.D. is an Associate Professor at the Academic Medical Center at the University of Amsterdam about Watson. As a clinical epidemiologist, he worked with IBM Watson for Oncology in preparation for a research proposal studying the role of Digital Decision Support tools. With several clinical colleagues of the department of Medical Oncology, they studied Watson for Oncology’s approach in approximately 400 surrogate patients with breast, lung or colorectal cancer. He believes that Watson for Oncology could result in a reduction of costs and efforts.\u003C\u002Fp>\n\n\n\n\u003Cp>They both agreed that AI cannot be a substitute for communication.&nbsp;\u003Cem>Right now, the technology is in its infancy, but improving all the time\u003C\u002Fem>, said Hennessey. If we introduced A.I. everywhere from Bolivia to Harvard, healthcare would&nbsp;not change a bit.
