We just wish they would each get their solution(s) to market more quickly!&#8221; &#8211; says the OpenAPS community FAQ, summarising the sentiment perfectly. \u003C\u002Fp>\n\n\n\n\u003Cp>It’s not enough to have patients fill out surveys as part of “patient centricity”. \u003Cstrong>What pharma companies need to adopt is patient design. \u003C\u002Fstrong>It goes beyond the passive act of asking about their opinion and actively involving them in crucial steps that will impact their health.\u003C\u002Fp>\n\n\n\n\u003Cp>In 2017, the FDA held its \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.fda.gov\u002Fnews-events\u002Fpress-announcements\u002Fstatement-fda-commissioner-scott-gottlieb-md-new-steps-fda-advance-patient-engagement-agencys\" target=\"_blank\">first Patient Engagement Advisory Committee meeting\u003C\u002Fa>. It set the pace to involve patients in their regulatory activities. The European Federation of Pharmaceutical Industries and Associations (\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.efpia.eu\u002Fabout-us\u002Fwho-we-are\u002F\" target=\"_blank\">EFPIA\u003C\u002Fa>) also took steps in this direction. It \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.efpia.eu\u002Fmedia\u002F24310\u002F3c_efpia-code-of-practice-on-relationships-pharmapluspt-orgs.pdf\" target=\"_blank\">developed a framework\u003C\u002Fa> to integrate patient organizations in their decision-making. Other members of the pharmaceutical industry could adopt similar strategies to better attend to their patients’ needs. In addition, such approaches will help deliver appropriate solutions faster.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>2. Integrating AI in drug development slashes time and cost\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>From finding suitable candidates for animal trials, from unexpected side effects in clinical trials to multiple trial-and-error sequences, drug development is understandably a lengthy and costly process. In fact, estimates put the numbers at about \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.ncbi.nlm.nih.gov\u002Fpmc\u002Farticles\u002FPMC1299137\u002F#:~:text=At%20present%2C%20bringing%20a%20single,progress%20from%20bench%20to%20market.\" target=\"_blank\">12 years\u003C\u002Fa> and an investment of \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fpubmed.ncbi.nlm.nih.gov\u002F32125404\u002F\" target=\"_blank\">between $1 billion and $2.8 billion\u003C\u002Fa> for an experimental drug to advance from a lab to the market. Although last year&#8217;s median new drug prices \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.reuters.com\u002Fbusiness\u002Fhealthcare-pharmaceuticals\u002Fprices-new-us-drugs-rose-35-2023-more-than-previous-year-2024-02-23\u002F\" target=\"_blank\">were &#8220;only&#8221; $300,000\u003C\u002Fa>, the FDA approved five new drugs that had their development costs in the $1.8-3.2 billion range. \u003C\u002Fp>\n\n\n\n\u003Cp>However, developments in the field of artificial intelligence can help diminish the cost and time traditionally associated with drug development. To exemplify this possibility, Insilico Medicine, an AI pharma startup, identified a potential new drug \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.technologyreview.com\u002F2019\u002F09\u002F03\u002F133175\u002Fan-ai-system-identified-a-potential-new-drug-in-just-46-days\u002F\" target=\"_blank\">in only 46 days\u003C\u002Fa>. Its algorithm achieved this by analysing vast amounts of data, which would otherwise take humans years to go through. The drug didn’t result in a commercial one as this achievement was to prove the potential of AI in drug development. In September 2023, the company started working with U.K.-based AI specialists BenevolentAI and Exscientia, aiming to significantly reduce drug discovery timelines –&nbsp;\u003Ca href=\"https:\u002F\u002Fwww.wsj.com\u002Ftech\u002Fbiotech\u002Fgermanys-merck-bets-on-ai-drug-design-partnerships-rules-out-acquisitions-interview-943c5b7a\">The Wall Street Journal reported\u003C\u002Fa>&nbsp;in June 2024.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Clinical trials alone are a vast field for AI, we&nbsp;\u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fai-assistance-in-clinical-trials-the-practical-opportunities\" target=\"_blank\" rel=\"noreferrer noopener\">recently analysed this segment\u003C\u002Fa>&nbsp;and listed examples of how various algorithms can be used in pre-trial assistance, how they can support trials, and what they can do after the trials. Tools like&nbsp;\u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-promise-of-crispr-gpt-specialized-chatgpts-could-transform-medicine\" target=\"_blank\" rel=\"noreferrer noopener\">CRISPR-GPT\u003C\u002Fa>&nbsp;can help us automate trial designs. \u003C\u002Fp>\n\n\n\n\u003Cp>In-silico platforms can predict real-world results with high accuracy. Novadiscovery’s jinkō&nbsp;\u003Ca href=\"https:\u002F\u002Fwww.novadiscovery.com\u002Fnovadiscovery-announces-success-of-first-of-its-kind-clinical-trial-simulation-to-accurately-predict-findings-of-phase-iii-clinical-study\u002F#:~:text=The%20jink%C5%8D%2Dpredicted%20findings%20were,a%20median%20TTP%20of%2025.9%20%5B\" target=\"_blank\" rel=\"noreferrer noopener\">predicted the results of an AstraZeneca trial\u003C\u002Fa>&nbsp;with about 97% accuracy before the company published the results. And it took three weeks to design, one hour to execute – and had a cost of a couple thousand dollars.\u003C\u002Fp>\n\n\n\n\u003Cp>To illustrate the speed of development, we only have to look around for some \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fphys.org\u002Fnews\u002F2021-12-scientists-unveil-drug-discovery-tool.html\" target=\"_blank\">existing good examples\u003C\u002Fa>. \u003C\u002Fp>\n\n\n\n\u003Cp>Scientists from the University of Southern California developed a computational method (V-SYNTHES) that allows them to computationally test billions of compounds against a therapeutic target. As described in their Nature paper from 2021, they tested 11 billion theoretical compounds against a cannabinoid receptor (CB2) that marijuana&#8217;s active ingredient THC targets. \u003C\u002Fp>\n\n\n\n\u003Cp>V-SYNTHES is now used by research labs and pharmaceutical companies in drug discovery. It&#8217;s particularly valuable for screening large chemical libraries against specific targets, helping identify novel drug candidates that may have been overlooked using traditional methods.\u003C\u002Fp>\n\n\n\n\u003Cp>And this is just the beginning! Look at \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fnews.mit.edu\u002F2022\u002Fai-model-finds-potentially-life-saving-drug-molecules-thousand-times-faster-0712\" target=\"_blank\">this announcement\u003C\u002Fa>: &#8220;In a paper that will be presented at the International Conference on Machine Learning (ICML), MIT researchers developed a geometric deep-learning model called EquiBind that is 1,200 times faster than one of the fastest existing computational molecular docking models, QuickVina2-W, in successfully binding drug-like molecules to proteins.&#8221;\u003C\u002Fp>\n\n\n\n\u003Cp>No genius drug researcher can compete with something being 1,200 times (!) faster than not them, but the previous supercomputer model.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>3. With 3D printing, pharma companies can make personalized medicine a reality&nbsp;\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Despite being a relatively new technology adopted in healthcare, 3D-printed drugs show great promise. The \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fqz.com\u002F471030\u002Fthe-fda-has-approved-the-first-drug-made-by-a-3d-printer\u002F\" target=\"_blank\">first FDA approval of a 3D-printed pill\u003C\u002Fa> was issued only in 2015, but the technology is already paving the way for personalized medicine. This was, in fact, the conclusion of a 2019 research. The researchers successfully 3D-printed 6 different drugs into a single, multilayered polypill. With this technique, pills of specific doses tailored to individual patients are made possible.\u003C\u002Fp>\n\n\n\n\u003Cp>Medical 3D-printing company \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.fabrx.co.uk\u002F\" target=\"_blank\">FabRx\u003C\u002Fa> is aggressively working towards this goal. In April 2020, they \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.fabrx.co.uk\u002F2020\u002F04\u002F06\u002Ffabrxs-pharmaceutical-3d-printer-for-personalised-medicines-m3dimaker-is-now-available\u002F\" target=\"_blank\">released M3DIMAKER\u003C\u002Fa> &#8211; the first pharmaceutical 3D printer to manufacture personalized medicines. In 2024, Gustave Roussy, Europe&#8217;s leading oncology hospital, successfully implemented the M3DIMAKER 2 3D drug printing device \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.sciencedirect.com\u002Fscience\u002Farticle\u002Fabs\u002Fpii\u002FS0378517324005404\" target=\"_blank\">for a pioneering clinical trial\u003C\u002Fa>. Focussed on improving adherence and efficacy in breast cancer treatment, the project combines tamoxifen with antidepressants to mitigate treatment side effects. This meant to 3D-print 40,000 units of polypills. This method potentially leads to dosage errors or even improper drug use. With 3D printing, this issue is a thing of the past. The printer allows pharmacists to make pills with precise doses for individual patients. “I truly believe that we are one step closer to personalized medicine thanks to the M3DIMAKER,” \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.fabrx.co.uk\u002F2020\u002F04\u002F06\u002Ffabrxs-pharmaceutical-3d-printer-for-personalised-medicines-m3dimaker-is-now-available\u002F\" target=\"_blank\">said Dr. Alvaro Goyanes\u003C\u002Fa>, Director of Development at FabRx.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>4. Boosting adherence of chronic care patients with digital pills\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Yearly, lack of adherence to one’s medication amounts to \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.pillsy.com\u002Farticles\u002Fmedication-adherence-stats#:~:text=Today%2C%20medication%20non%2Dadherence%20leads,billion%20in%20avoidable%20healthcare%20costs.\" target=\"_blank\">125,000 deaths and around $300 billion\u003C\u002Fa> in healthcare costs; all of these are in fact preventable. To remedy this pressing issue, digital pills can boost adherence while preventing deaths.\u003C\u002Fp>\n\n\n\n\u003Cp>The FDA first approved such a digital pill \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.mobihealthnews.com\u002Fcontent\u002Fotsuka-proteus-finally-win-fda-approval-sensor-equipped-abilify\" target=\"_blank\">in 2017\u003C\u002Fa> produced by Otsuka Pharmaceutical and Proteus for chronic mental disorders. This pill contains an ingestible sensor, which is a wearable patch sensor and an app track. This allows physicians to monitor a patient’s compliance with the treatment.\u003C\u002Fp>\n\n\n\n\u003Cp>Although Proteus \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.fiercebiotech.com\u002Fmedtech\u002Fdigital-pill-developer-proteus-files-for-bankruptcy\" target=\"_blank\">filed for bankruptcy\u003C\u002Fa> in June 2020, the technology it employed isn’t at fault. The digital pill field was born, and there are a number of interesting players. \u003C\u002Fp>\n\n\n\n\u003Cp>Digital health company \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fetectrx.com\u002F\" target=\"_blank\">etectRx \u003C\u002Fa>aims to advance patient medication adherence and improve ingestion tracking technology. These functionalities provide valuable insights into efficiency, enabling pharmacists to continuously optimize their use. Automated dispensing systems are becoming increasingly prevalent in pharmacies worldwide. While the exact penetration rates vary by region, industry reports suggest significant growth in recent years, with Europe being a particularly active market for such technologies. Dispensing robots are already employed in \u003Ca href=\"https:\u002F\u002Fthejournalofmhealth.com\u002Fhow-robots-are-changing-community-pharmacy\u002F\">70% of community pharmacies in Denmark\u003C\u002Fa>, while in mainland Europe, \u003Ca href=\"https:\u002F\u002Fwww.p3pharmacy.co.uk\u002F10-things-about-automated-dispensing\">30-40% of pharmacies\u003C\u002Fa> use automated dispensers.\u003C\u002Fp>\n\n\n\n\u003Cp>All in all, robotics in pharma can automate repetitive tasks such as sorting, packaging, and labeling medications, minimizing the risk of human error and freeing up personnel for more complex tasks.\u003C\u002Fp>\n\n\n\n\u003Cp>A growing concern with automation is that of not requiring human assistants, leading to rising unemployment. However, the opposite might be true. A pharmacist in Scotland employed more people after including automated technologies in the pharmacy; the reason being the need for more people to manage the resulting increased turnover with automation.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Moreover, with mundane, repetitive tasks delegated to robots, human input will be important in responsibilities requiring creativity and problem-solving skills. Remember the bug in Neo’s stomach in the Matrix? He couldn’t have taken it out without help – but after some time, no lasting marks remained. \u003C\u002Fp>\n\n\n\n\u003Cp>In the second stage of augmentation, technology interacts with the human body in a way that it already has a lasting, but not irreversible impact. Although their removal might be problematic, implants, patches, and tattoos could be cut out and discarded.
