These are solutions everyone’s talking about or the ones they \u003Cem>should\u003C\u002Fem>. We are beginning with skin checking algorithms, which is definitely a course currently rising. Let’s have a deeper look.\u003C\u002Fh5>\n\n\n\n\u003Chr class=\"wp-block-separator\"\u002F>\n\n\n\n\u003Cp>Everyone should have a skin check done from time to time. Skin cancer is one of the most common cancer types worldwide: \u003Ca href=\"https:\u002F\u002Fwww.skincancer.org\u002Fskin-cancer-information\u002Fskin-cancer-facts\u002F#:~:text=1%20in%205%20Americans%20will,doubles%20your%20risk%20for%20melanoma.\" target=\"_blank\" rel=\"noreferrer noopener\">one in five\u003C\u002Fa> people in the U.S. is expected to receive a skin cancer diagnosis during their lifetime. Early detection and treatment are invaluable: almost all skin cancers (both melanoma and nonmelanoma) \u003Ca href=\"https:\u002F\u002Fwww.cancer.org\u002Fcontent\u002Fdam\u002Fcancer-org\u002Fresearch\u002Fcancer-facts-and-statistics\u002Fannual-cancer-facts-and-figures\u002F2021\u002Fcancer-facts-and-figures-2021.pdf\">can be cured\u003C\u002Fa> if found and treated early. Prevention and detection are the key.\u003C\u002Fp>\n\n\n\n\u003Cp>However, these two concepts failed miserably in 2020. The hardships of COVID included a general absence from all diagnostic appointments. One fearful fact dermatologists pointed out in a survey was that \u003Ca href=\"https:\u002F\u002Fwww.melanomauk.org.uk\u002Fnews\u002Fnew-partnership-with-skin-vision-gives-free-licences-to-my-melanoma-app-users\" target=\"_blank\" rel=\"noreferrer noopener\">about 21% of melanomas\u003C\u002Fa> might have gone undetected in 2020.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Therefore, the \u003Ca href=\"https:\u002F\u002Fwww.cancer.org\u002Fcontent\u002Fdam\u002Fcancer-org\u002Fresearch\u002Fcancer-facts-and-statistics\u002Fannual-cancer-facts-and-figures\u002F2021\u002Fcancer-facts-and-figures-2021.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">American Cancer Society estimated\u003C\u002Fa> that the number of new melanoma cases diagnosed in 2021 would increase by 5.8 per cent. Telemedicine could be helpful if there were enough medical professionals to work online&nbsp; – but ultimately, technology could provide much more than online visits. The \u003Ca href=\"https:\u002F\u002Fwww.theweek.in\u002Fnews\u002Fhealth\u002F2021\u002F02\u002F19\u002Falgorithm-can-detect-skin-cancer-as-accurately-dermatologists.html\" target=\"_blank\" rel=\"noreferrer noopener\">results\u003C\u002Fa> are plausible, and it seems the ever-present saying about A.I. in healthcare has its place here, too: “\u003Cstrong>\u003Cem>A.I. will not replace dermatologists. But dermatologists using A.I. will replace those who don’t.”\u003C\u002Fem>\u003C\u002Fstrong>&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>A \u003Ca href=\"https:\u002F\u002Fwww.nature.com\u002Farticles\u002Fs41591-020-0942-0\" target=\"_blank\" rel=\"noreferrer noopener\">study in Nature\u003C\u002Fa> in 2020 confirmed that on cleaned data for selected lesions, A.I. is as good as or even superior to human experts in image-based diagnosis. Which is a good thing, considering that there’s a constant \u003Ca href=\"https:\u002F\u002Fwww.jaad.org\u002Farticle\u002FS0190-9622(17)30017-8\u002Ffulltext#sec3.2\" target=\"_blank\" rel=\"noreferrer noopener\">shortage of dermatologists\u003C\u002Fa>, especially in rural areas. Therefore, it comes as no surprise that Google decided to throw their hat in the ring: the \u003Ca href=\"https:\u002F\u002Fwww.fiercehealthcare.com\u002Ftech\u002Fgoogle-previews-ai-dermatology-tool-to-help-consumers-identify-skin-conditions\" target=\"_blank\" rel=\"noreferrer noopener\">tech giant launched\u003C\u002Fa> an A.I.-based app to diagnose skin conditions. It turns out the search titan took notice of the ten billion searches annually that were aimed at skin\u002Fhair\u002Fnail issues and acted upon. The algorithm also shows information about the particular condition reviewed by dermatologists and advises on the next steps, like offering a doctor’s appointment. And the best thing? Pioneering the technology on the market, \u003Ca href=\"https:\u002F\u002Fwww.skinvision.com\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">SkinVision\u003C\u002Fa> started working on the technology in 2012. We \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Faspiring-dermatology-app-under-the-microscope-the-skinvision-review\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">reviewed the tech\u003C\u002Fa> in 2019 and were left with generally positive experiences. Ever since the very first certified skin cancer application has continued to grow, it has also \u003Ca href=\"https:\u002F\u002Fbridgingandcommercial.co.uk\u002Farticle-desc-17157\" target=\"_blank\" rel=\"noreferrer noopener\">been offered\u003C\u002Fa> to employees of companies among well-being benefits.\u003C\u002Fp>\n\n\n\n\u003Cp>Not even two weeks after Google’s launch of Derm Assist, another company \u003Ca href=\"https:\u002F\u002Fwww.mobihealthnews.com\u002Fnews\u002Fapac\u002Fadvanced-human-imagings-smartphone-based-derma-ai-gets-ce-mark\" target=\"_blank\" rel=\"noreferrer noopener\">announced that\u003C\u002Fa> it received the European CE mark for its smartphone-based dermatology A.I. Earlier this year, Australian-based medical investment firm \u003Ca href=\"https:\u002F\u002Fwww.advancedhumanimaging.com\u002Ftechnology\u002Fdermascan\" target=\"_blank\" rel=\"noreferrer noopener\">Advanced Human Imaging\u003C\u002Fa> (AHI) bought a \u003Ca href=\"https:\u002F\u002Fwww.crunchbase.com\u002Forganization\u002Ftriage-com\" target=\"_blank\" rel=\"noreferrer noopener\">Canadian startup\u003C\u002Fa>, Triage, to enter the market. DermaScan is capable of screening 588 skin conditions in 133 categories with a smartphone. So it seems like an excellent investment – especially given that the CE mark validates the A.I.-powered tool throughout the EU.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>How much regulation is enough?\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Some researchers, however, \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.bmj.com\u002Fcontent\u002Fbmj\u002F368\u002Fbmj.m127.full.pdf\" target=\"_blank\">question\u003C\u002Fa> the accuracy and reliability of these apps. Scientists from the University of Birmingham \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.mobihealthnews.com\u002Fnews\u002Femea\u002Fstudy-skin-cancer-detection-apps-cannot-be-relied-accurate-diagnosis\" target=\"_blank\">have found\u003C\u002Fa> that some apps had “\u003Cem>poor and variable performance\u003C\u002Fem>”, even though they had the necessary regulatory approval. The authors of the study were concerned about both kinds of&nbsp;outcomes. False positives are causing unnecessary anxiety, while false negatives may result in undiagnosed melanoma.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Hungarian-based skin checker application \u003Ca href=\"https:\u002F\u002Femdee.ai\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">emdee.ai\u003C\u002Fa> is trying to overcome this gap. The company claims to provide a ‘pocket-sized dermatologist’ by giving a prediction &amp; prevention platform for both medical professionals and patients. I have talked with their CEO recently about how the app works and how they built the deep neural network for better A.I. performance together with Semmelweis Medical University. He \u003Ca href=\"https:\u002F\u002Fwww.patreon.com\u002Fposts\u002F50849968\" target=\"_blank\" rel=\"noreferrer noopener\">explained\u003C\u002Fa> how their system is used as a supplement to a dermatologist’s professional opinion.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Emdee.ai helps to improve the diagnostic accuracy, but the diagnosis in fact comes from the dermatologist. “\u003Cem>The app has a version that the dermatologists and GPs can use to get a kind of second diagnosis.\u003C\u002Fem>” And there’s also a web-based system for medical professionals supporting diagnostics: \u003Cem>“the emdee Physician Console can be used to improve their diagnostic accuracy and store the dermoscopic and regular photos of patients.”\u003C\u002Fem>\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Ca href=\"https:\u002F\u002Fnomela.com\u002Fthe-medical-test\" target=\"_blank\" rel=\"noreferrer noopener\">Nomela\u003C\u002Fa>, another skin checker, offers professionals a CE marked rule-out screening test of pigmented skin lesions suspected of melanoma. The application \u003Ca href=\"https:\u002F\u002Fwww.med-technews.com\u002Fnews\u002Fmelanoma-screening-test-developed-to-reduce-dermatology-refe\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">provides\u003C\u002Fa> primary care professionals with the opportunity to reduce unnecessary dermatology referrals to secondary care. The screen test is quick and easy, and gives immediate, live feedback to the trained medical professional carrying out the screening with an iPad.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Ca href=\"https:\u002F\u002Fmiiskin.com\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">Miiskin\u003C\u002Fa> offers access to its platform for patients and providers alike. Patients can do regular checking, while physicians have access to A.I.-powered diagnostic support. The first skin checker app approved on the \u003Ca href=\"https:\u002F\u002Fwww.nhs.uk\u002Fapps-library\u002Fmiiskin\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">NHS App Library\u003C\u002Fa>, Miiskin clearly states that it does not provide a diagnosis. Instead, “\u003Cem>it’s simply your own tool to document changes to your skin when performing your regular skin check.\u003C\u002Fem>”\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Issues\u003C\u002Fstrong> with methods\u003C\u002Fh2>\n\n\n\n\u003Cp>Although professionals in principle agree on the need for such apps, they often question the methods. For example, smartphone-based photo-taking might lack the clarity and the meticulous details that are very significant in skin diagnostics. Results from skin checking apps can be \u003Ca href=\"https:\u002F\u002Fwww.mobihealthnews.com\u002Fnews\u002Femea\u002Fstudy-skin-cancer-detection-apps-cannot-be-relied-accurate-diagnosis\" target=\"_blank\" rel=\"noreferrer noopener\">misleading\u003C\u002Fa>, bringing the patients false hopes or anxiety. Their price questions the concept that these apps would help save people’s lives regardless of income inequalities or location.\u003C\u002Fp>\n\n\n\n\u003Cdiv style=\"height:100px\" aria-hidden=\"true\" class=\"wp-block-spacer\">\u003C\u002Fdiv>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">Ultimately, these are m\u003Cstrong>y 3As for skin checker algorithms for the future\u003C\u002Fstrong>:\u003C\u002Fh3>\n\n\n\n\u003Col class=\"wp-block-list\">\u003Cli>\u003Cstrong>Accessibility: \u003C\u002Fstrong>if the app works under at-home conditions with photos taken by a patient\u003C\u002Fli>\u003Cli>\u003Cstrong>Analyse: \u003C\u002Fstrong>can the app reliably analyse the images for the patient and not give a false reassurance that they should never have to see a doctor\u003C\u002Fli>\u003Cli>\u003Cstrong>Availability:\u003C\u002Fstrong> whether the service will remain available to the masses without the analysis of the follow-up being very expensive for the patient.\u003C\u002Fli>\u003C\u002Fol>\n\n\n\n\u003Chr class=\"wp-block-separator\"\u002F>\n\n\n\n\u003Ch5 class=\"wp-block-heading\">\u003Cstrong>Using advanced technologies to reduce the number of skin cancer issues could be crucial in pushing back the disease. But they attest to the relics of a workflow from the 20th century; while 21st-century technology can radically improve the system from the inside out. Quite literally.\u003C\u002Fp>\n\n\n\n\u003Cp>Indeed, in designing the future of hospitals, much will be offloaded to the outside of these institutions by leveraging digital health technologies. Patients could monitor their vitals wherever they are thanks to wearables and share the data to their physicians remotely. But that does not make hospitals obsolete altogether. Rather, they will become health centres for patients for preventing diseases, for acute care patients and for patients who need surgical procedures or large radiology machines.\u003C\u002Fp>\n\n\n\n\u003Cp>While these are long term goals, several adjustments can be undertaken in the short term to build up to the ideal design of future hospitals. These&nbsp;could help better incorporate digital health tools to fight the pandemic, which could help those institutions become more resilient against future health crises.\u003C\u002Fp>\n\n\n\n\u003Cp>First, let’s see how the point of care can be redesigned from the inside. Read the second part of this article, the future of the hospitals – \u003Cem>outside\u003C\u002Fem> the point of care \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-future-of-healthcare-design-outside-the-point-of-care\" target=\"_blank\" rel=\"noreferrer noopener\">here\u003C\u002Fa>.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Accommodate digital health technologies\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>The pandemic accelerated the trend to adopt digital health technologies, many of which \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fdigital-health-tech-here-to-stay-after-covid-19\u002F\" target=\"_blank\">are here to stay\u003C\u002Fa>. Healthcare facilities should be ready for such events.\u003C\u002Fp>\n\n\n\n\u003Cp>For example, robots also have \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002F8-exciting-medical-robot-facts\" target=\"_blank\">a myriad of uses in healthcare\u003C\u002Fa> from making in-house deliveries to disinfecting surfaces; with the added bonus of not risking cross-infection or succumbing to burnout. A doctor’s desk could turn into their computer with a rollable screen or \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Faugmented-reality-in-healthcare-will-be-revolutionary\u002F\" target=\"_blank\">augmented reality technology\u003C\u002Fa> to plan for a procedure while decluttering their workspace. The \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.anatomage.com\u002Ftable\u002F\" target=\"_blank\">Anatomage Table\u003C\u002Fa> already offers a virtual dissection surface on a desk-sized screen; and designers could factor in such options when visualising new hospitals.\u003C\u002Fp>\n\n\n\n\u003Cp>Another promising technology to accommodate is telemedicine. It saw \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fcovid-19-was-needed-for-telemedicine-to-finally-go-mainstream\u002F\" target=\"_blank\">a surge in adoption\u003C\u002Fa> to continue providing care to patients remotely during the pandemic. Already, \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.bloomberg.com\u002Fnews\u002Farticles\u002F2021-02-23\u002Fhow-the-pandemic-is-transforming-hospital-design\" target=\"_blank\">a McKinsey report estimates\u003C\u002Fa> that 25% of outpatient services could be conducted this way in the future. In the U.S., 911 emergency calls that do not require prompt medical assistance \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.bloomberg.com\u002Fnews\u002Farticles\u002F2021-02-23\u002Fhow-the-pandemic-is-transforming-hospital-design\" target=\"_blank\">can be relayed to\u003C\u002Fa> telemedical consultations. This practice benefits overburdened hospitals by drastically reducing unnecessary visits and making queues a thing of the past; while also enhancing the patient experience.\u003C\u002Fp>\n\n\n\n\u003Cp>To further support telemedicine consultations, hospitals &#8220;\u003Cem>require dedicated spaces for virtual consultations. These will include user-friendly sound and lighting control, with dual screens to simultaneously converse with the patient and access to their medical records\u003C\u002Fem>&#8221; – Dr. Diana Anderson, the &#8216;\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.dochitect.com\u002F\" target=\"_blank\">Dochitect\u003C\u002Fa>&#8216; said in an interview with The Medical Futurist.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Flexible design\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>In our conversation with Diana Anderson, MD, M.Arch, and Principal of \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.jacobs.com\u002Fdelivery\u002Farchitecture\" target=\"_blank\">Jacobs\u003C\u002Fa>, together with Cath Lewin RN, BN, senior health planner at Jacobs for our \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.patreon.com\u002Fthemedicalfuturist\u002Fposts\" target=\"_blank\">Patreon site\u003C\u002Fa>, they explained that &#8220;\u003Cem>one of the main innovations to perpetuate is designing flexible space that allows for conversion to ‘pandemic mode’. There are great opportunities for us to really rethink patient flows on the site, at the front door, and within treatment facilities\u003C\u002Fem>.&#8221; Dr Anderson added that “\u003Cem>there are great opportunities for us to really rethink patient flows on the site, at the front door, and within treatment facilities\u003C\u002Fem>.”\u003C\u002Fp>\n\n\n\n\u003Cp>Asking about the implementation of such flexible designs, Diana Anderson and Cath Lewin have several ideas. For instance, &#8220;\u003Cem>open areas like at-grade car parks and external entry foyers can double as external triage areas when accommodating increased patient capacity and implementing physical distancing strategies. Ventilation in patient rooms is isolated thanks to glass doors. These also enable medical staff to observe patients while reducing the risk of cross-contamination. Moreover, the building’s lobby itself \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.rushu.rush.edu\u002Fnews\u002F%E2%80%98-what-rush-was-built-%E2%80%99\" target=\"_blank\">can accommodate extra beds\u003C\u002Fa> as it is already equipped with electrical and medical gas outlets. Virtually any room of hospitals could incorporate such flexible spaces; whether it’s a conference room or offices to support the extra load of patients when needed.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Enhancing the workplace of caregivers\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>While the design of future hospitals will enhance the patient experience, it should also create an improved workplace for caregivers. Enabling hospitals to accommodate more patients in emergency situations can lift the psychological toll off the staff who might be forced to prioritise whom to allocate resources to, as it happened \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fchoosing-between-life-and-death-during-covid-19-the-a-i-trolley-problem\u002F\" target=\"_blank\">during the COVID-19 pandemic\u003C\u002Fa>.\u003C\u002Fp>\n\n\n\n\u003Cp>However, before the pandemic, healthcare workers were already afflicted by another epidemic: \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fartificial-intelligence-could-solve-alarm-fatigue-in-hospitals\u002F\" target=\"_blank\">alarm fatigue\u003C\u002Fa>. It refers to the point when caregivers become desensitised to alarm signs and can miss those that actually need medical attention. Caregivers can experience as many as \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.ncbi.nlm.nih.gov\u002Fpmc\u002Farticles\u002FPMC4206416\u002F\" target=\"_blank\">187 alarms per bed per day\u003C\u002Fa>; but \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fnurse.org\u002Farticles\u002Falarm-fatigue-statistics-patient-safety\u002F\" target=\"_blank\">up to 99% alarms\u003C\u002Fa> from patient beds can be false ones. But an artificial intelligence-based solution \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fartificial-intelligence-could-solve-alarm-fatigue-in-hospitals\u002F\" target=\"_blank\">can help reduce such notifications\u003C\u002Fa> received by the caregivers by up to 99.3%; enabling them to only hear those which require medical assistance. As the recipient of healthcare services, they are better placed to identify which areas need improvements.\u003C\u002Fp>\n\n\n\n\u003Cp>Take, for example, the Oral and Maxillofacial Surgery Department of Radboud University Medical Centre Nijmegen in the Netherlands. They prioritised round tables over square ones for more friendly conversations and also suggested brightly lit rooms with friendly colors.
