While technology, especially EHRs, is often considered as an essential factor contributing to physician burnout, we expect artificial intelligence to significantly reduce the administrative burden and improve medical professionals’ work experience in the future.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>The rough numbers of physician burnout\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>The emotional and mental well-being of medical\nprofessionals around the world has reached such historically low levels, that\nthe incoming President of the World Medical Association, Dr. Leonid Eidelman, \u003Ca href=\"https:\u002F\u002Fwww.wma.net\u002Fnews-post\u002Fwma-leader-warns-of-global-physician-burnout\u002F\">started his inauguration speech in October 2018 with\naddressing the “pandemic of physician burnout”\u003C\u002Fa>. At that moment, \u003Cstrong>nearly\nhalf of the world’s 10 million physicians had symptoms of burnout, including\nemotional exhaustion, interpersonal disengagement, and a low sense of personal\naccomplishment, he explained. It continues to negatively influence the quality\nof care and shorten the lifetime a\nphysician is able to practice medicine\u003C\u002Fstrong>, Dr. Eidelman added.\u003C\u002Fp>\n\n\n\n\u003Cp>The situation has never been so disappointing as today – in the age of technological promise – and it is getting worse day by day. While then acting \u003Ca href=\"https:\u002F\u002Fwww.medpagetoday.com\u002Fpublichealthpolicy\u002Fgeneralprofessionalissues\u002F57280\">Surgeon General, Vivek Murthy already sounded the alarm bells in the US three years ago\u003C\u002Fa>, \u003Ca href=\"https:\u002F\u002Fwww.beckershospitalreview.com\u002Fehrs\u002F6-stats-on-ehr-related-physician-burnout-and-7-tips-to-combat-it.html\">statistics show\u003C\u002Fa> that the phenomenon is worsening: about half (51 percent) of physicians reported frequent feelings of burnout in 2017, up from 40 percent in 2013. Beyond the colossal sum that physician burnout could cost the U.S. healthcare industry &#8211; $17 billion each year -, it implicates lowering levels of patient care with potential dangers for patients’ lives and the \u003Ca href=\"https:\u002F\u002Fwww.medscape.com\u002Fviewarticle\u002F896257\">highest suicide rate among professions\u003C\u002Fa>. \u003C\u002Fp>\n\n\n\n\u003Cp>Not only the U.S. has such depressing numbers, although the range of statistics around the phenomenon could \u003Cg class=\"gr_ gr_388 gr-alert gr_spell gr_inline_cards gr_run_anim ContextualSpelling ins-del\" id=\"388\" data-gr-id=\"388\">vary\u003C\u002Fg> highly (from 7 to 70 percent!) depending on organization context and samples. According to a \u003Ca href=\"https:\u002F\u002Fwww.ncbi.nlm.nih.gov\u002Fpubmed\u002F30580199\">study among French physicians\u003C\u002Fa>, the prevalence estimate for burnout was 49 percent&nbsp;and 5 percent for severe burnout.  A \u003Ca href=\"https:\u002F\u002Fwww.ncbi.nlm.nih.gov\u002Fpubmed\u002F18622012\">survey among family doctors in 12 European countries\u003C\u002Fa> about the same subject showed that nearly\u003Cbr> 65&nbsp;percent of participants had signs of high burnout. But how did we get there? The\ndoctor or the nurse feel that no matter how hard they work, it has no result\nwhatsoever, and after several failed attempts it isn’t worth trying anymore,\nand life will end anyway – what’s the point? To the question of what\ncontributes most to physician burnout, \u003Cstrong>most\nof the respondents answered the followings: the burden of bureaucratic tasks\n(56 per cent), long hours at work (39 per cent), the lack of respect coming\ntowards them (26 per cent) and the increasing computerization of the practice,\nincluding the issue around EHRs (24 per cent)\u003C\u002Fstrong>. \u003C\u002Fp>\n\n\n\n\u003Cp>Except for the issue of respect, the correlation between the other three elements seems to be straightforward. The doctor is overwhelmed,\nextremely tired, doesn’t see the point of working anyway, so just wants to\nfinish as soon as possible. Would you entrust your mom’s health with a\nphysician with burnout? What will the chances be for medical errors and\nunexpected complications?\u003C\u002Fp>\n\n\n\n\u003Cp>In the US, \u003Ca href=\"https:\u002F\u002Fwww.usnews.com\u002Fnews\u002Farticles\u002F2016-05-03\u002Fmedical-errors-are-third-leading-cause-of-death-in-the-us\">medical errors are the third leading cause of death\u003C\u002Fa> with more than 250,000 deaths per year. Although\nthere are no longitudinal studies about the correlation of burnout and medical\nerrors, for example, Dr. David Meyers, chief medical officer of the Agency for\nHealthcare Research and Quality, \u003Ca href=\"https:\u002F\u002Fhealth.usnews.com\u002Fhealth-care\u002Fpatient-advice\u002Farticles\u002F2018-04-17\u002Fis-physician-burnout-affecting-your-care\" target=\"_blank\" rel=\"noreferrer noopener\">noted\u003C\u002Fa>, “when doctors and nurses feel burnt out, they are\nmore likely to make a medical error.” With the increasing numbers of burnout,\nhigh levels of patient care are endangered. \u003C\u002Fp>\n\n\n\n\u003Cp>Or, what’s worse, the actual act of care. \u003Cstrong>The phenomenon strains a global healthcare\nsystem already battling the lack of medical professionals. \u003C\u002Fstrong>\u003Cstrong>\u003Ca href=\"http:\u002F\u002Fwww.massmed.org\u002Fburnoutpaper\u002F#.XFDJIlxKg2z\">The U.S. Department of Health and Human Services\npredicts\u003C\u002Fa>\u003C\u002Fstrong>\u003Cstrong> that by 2025, there will be a nationwide shortage of\nnearly 90,000 physicians, many driven away from medicine or out of practice\nbecause of the effects of burnout\u003C\u002Fstrong>. While, according to \u003Ca href=\"http:\u002F\u002Ftmfinstitute.org\u002F\">The Medical Futurist Institute\u003C\u002Fa>, the needs-based shortage of healthcare workers\nglobally is already about 17.4 million. That’s almost the entire population of Chile\nand twice as many as living in Austria or Israel. As \u003Ca href=\"https:\u002F\u002Fhuman-resources-health.biomedcentral.com\u002Farticles\u002F10.1186\u002Fs12960-016-0108-9\">avoiding physician burnout is a much more successful method of dealing with the problem than “firefighting”\u003C\u002Fa>, prevention should get in the forefront of battling prolonged exhaustion. \u003Cstrong>Change in the work environment, constant schedule shifts break the monotony of the job, the introduction of the sabbatical year could recharge medical professionals, and various professional training courses might give back the belief in the value of healing and taking care of others.\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>Looking at the individual level, when the above\nmentioned AMA survey asked doctors how they personally deal with their burnout,\nhalf of them brought up exercise and\nsports, and 46 per cent said they talk their troubles through with family and\u002For\nclose friends. Beyond these methods, applying various stress-reducing techniques, such as meditation and the attempt to\nreach mindfulness might also be successful. What’s\nmore, innovative boardgames could come into the picture. Focus Games\nintroduced the so-called \u003Ca href=\"https:\u002F\u002Fshop.focusgames.com\u002Fproducts\u002Fworking-stress-game\">Working Stress\nGame\u003C\u002Fa> for physicians to talk about the effects of stress and how to\nunderstand stressors better in a playful way. \u003C\u002Fp>\n\n\n\n\u003Cp>Here, \u003Cstrong>technology as a helper might come into the picture. Activity and fitness trackers could \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fhow-to-start-running\">help exercise better\u003C\u002Fa> and more efficiently, sleep sensors and apps could take care of the good night sleep, and plenty of mental health apps aim for guiding users to mindfulness\u003C\u002Fstrong>. IBM\nlaunched an algorithm called \u003Ca href=\"http:\u002F\u002Fresearcher.watson.ibm.com\u002Fresearcher\u002Fview_group.php?id=4384\">Medical Sieve\u003C\u002Fa>. It is an ambitious long-term exploratory project to\nbuild the next-generation “cognitive\nassistant” with analytical, reasoning capabilities and a wide range of clinical\nknowledge. \u003C\u002Fp>\n\n\n\n\u003Cp>San Francisco-based \u003Ca href=\"https:\u002F\u002Fwww.augmedix.com\u002F\">Augmedix\u003C\u002Fa> aims to \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Ftop-9-augmented-reality-companies-healthcare\u002F\">harness the power of the Google Glass to make\nhealthcare more patient-centric\u003C\u002Fa> and cut down on administration. It provides a technology-enabled\ndocumentation service for doctors and health systems, so physicians do not have\nto check their computers during patient visits, while medical notes are still\ngenerated in real time.\u003C\u002Fp>\n\n\n\n\u003Cp>Moreover, \u003Ca href=\"https:\u002F\u002Fwww.wired.com\u002Fstory\u002Fdoes-your-doctor-need-a-voice-assistant\u002F\">voice assistants have the potential to free up the time of doctors\u003C\u002Fa> spent on administration. Although we still have 3 years from the predicted decade, it is unlikely that a change of such magnitude is going to happen in this timeframe.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Silicon Valley investor Vinod Khosla \u003Ca rel=\"noreferrer noopener\" href=\"http:\u002F\u002Fwww.wired.co.uk\u002Farticle\u002Fdoctors-replaced-with-machines\" target=\"_blank\">said\u003C\u002Fa> that \u003Cem>“machines will substitute 80 percent of doctors in the future in a healthcare scene driven by entrepreneurs, not medical professionals\u003C\u002Fem>.” Prof Geoffrey Hinton, the godfather of neural networks, once \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Ftowardsdatascience.com\u002Fwhy-ai-will-not-replace-radiologists-c7736f2c7d80\" target=\"_blank\">said that\u003C\u002Fa> it’s \u003Cem>“quite obvious that we should stop training radiologists”\u003C\u002Fem> as image perception algorithms are very soon going to be demonstrably better than humans. And smart algorithms are also there to supplement professionals’ tasks in many fields, like diagnostics, image analysis and decision support,&nbsp;hospital resource or clinical workflow optimisation, forecasting, drug discovery and precision medicine.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Healthcare will need humans in the future\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Don’t get me wrong, AI will be visibly present in healthcare in the next 10-15 years. We already see good examples, like the sepsis watch algorithm that cut sepsis mortality rates \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.wsj.com\u002Farticles\u002Fhow-hospitals-are-using-ai-to-save-lives-11649610000\" target=\"_blank\">by nearly 30%\u003C\u002Fa>, or \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.eurekalert.org\u002Fnews-releases\u002F1003963\" target=\"_blank\">skin cancer detection with near-perfect accuracy\u003C\u002Fa> lending a hand to dermatologists, or the algorithm that predicts \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.uni.lu\u002Flcsb-en\u002Fnews\u002Fpredicting-arrhythmia-30-minutes-before-it-happens\u002F\" target=\"_blank\">atrial fibrillation 30 minutes before onset\u003C\u002Fa> allowing time for preemptive intervention.\u003C\u002Fp>\n\n\n\n\u003Cp>AI will transform what it means to be a doctor; some tasks will disappear, while others will be added to the work routine. An algorithm cannot replace that.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>2) Physicians have a non-linear working method\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>There was \u003Ca rel=\"noreferrer noopener\" href=\"http:\u002F\u002Fwww.housemd-guide.com\u002Fseason1\u002F108poison.php\" target=\"_blank\">an episode\u003C\u002Fa> in House M.D. where the team couldn’t figure out how a young boy could have been poisoned. They considered many options: drugs, food poisoning, pesticide poisoning. For every possible diagnosis, they suggested a different treatment option. Each made the patient worse – until they figured out, by accident, that the boy picked up a phosmet, a type of insecticide from the jeans he bought from a street vendor who kept the trousers in a truck. The boy didn’t wash the pants before wearing them, that’s how his skin could absorb the poison.\u003C\u002Fp>\n\n\n\n\u003Cp>No algorithm could have made such a diagnosis. Data, measurements, and quantitative analytics are crucial parts of a doctor’s work. It will be even more critical in the future. Setting up a diagnosis and treating a patient are not linear processes. Diseases have the same features. Thus, no case is the same; every one of them requires the attention of human physicians. Until the emergence of complex, digital solutions, doctors translated data from simple medical devices into medical decisions. In the future, the task will be the same – they will only use more complicated technologies.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>3) Complex digital technologies require competent professionals\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>More and more sophisticated digital health solutions will require qualified medical professionals’ competence, no matter whether it’s about robotics or AI.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cmark style=\"background-color:rgba(0, 0, 0, 0)\" class=\"has-inline-color has-black-color\">The human brain is so complex and able to oversee such a vast scale of knowledge and data that it is merely not worth developing an AI that takes over this job – the human brain does it so well. It is more worthwhile to program those repetitive, data-based tasks, and leave the complex analysis\u002Fdecision to the person. While they will provide the data, interpretation will always remain a human territory.\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>4) There will always be tasks algorithms and robots can never complete\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Physicians, nurses, and other medical staff members have plenty of cumbersome, monotonous, and repetitive tasks to complete every day. A \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.ncbi.nlm.nih.gov\u002Fpubmed\u002F25626223\" target=\"_blank\">study says\u003C\u002Fa> that in the United States, the average doctor spends 8.7 hours per week on administration. Psychiatrists spent the highest proportion of their working hours on paperwork (20.3%), followed by internists (17.3%) and family\u002Fgeneral practitioners (17.3%). These types of tasks and procedures can be automated – and they should be.\u003C\u002Fp>\n\n\n\n\u003Cp>However, there are responsibilities and duties which technologies cannot perform. While AI can sift through millions of pages of documents in seconds, it will never be able to do the Heimlich maneuver. There will always be tasks where humans will be faster, more reliable – or cheaper than technology.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>5) It has never been tech vs human\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>The consistent and constant enemy image building should stop once and for all.&nbsp;It has never been technology versus humans since technological innovations always serve the purpose of helping people. Collaboration between humans and technology is the \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fknowledge.wharton.upenn.edu\u002Farticle\u002Freimagining-work-age-ai\u002F\" target=\"_blank\">ultimate response\u003C\u002Fa>. \u003C\u002Fp>\n\n\n\n\u003Cp>\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Farxiv.org\u002Fabs\u002F1606.05718\" target=\"_blank\">A study\u003C\u002Fa> for identifying metastatic breast cancer through deep learning shows something similar. When the deep learning system’s predictions were combined with the human pathologist’s diagnoses, the image classification and the tumor localization score increased significantly. Moreover, the human error rate decreased by 85 percent. The findings show that artificial intelligence and humans are the most potent when they cooperate.\u003C\u002Fp>\n\n\n\n\u003Cp>Chess is a great example. It was believed to meet its demise following Garry Kasparov&#8217;s defeat by a supercomputer. However, the opposite has happened. Today, over 600 million people engage in daily chess, and AI has become an indispensable tool for players, enthusiasts, and coaches alike.\u003C\u002Fp>\n\n\n\n\u003Cp>AI has demonstrably enhanced chess by creating a more competitive, popular, and ultimately, more effective environment.
