The centuries-old approach to the medical profession cannot deal with the waves of the 21st century. Physicians should experience a shift from the lone wolf to the curious team player, from the rule follower to the creative and from the demigod to the guide in the jungle of digital health. That’s the main argument of the latest \u003Ca href=\"https:\u002F\u002Fwww.jmir.org\u002F2019\u002F3\u002Fe12490\u002F\">publication\u003C\u002Fa> of The Medical Futurist Institute entitled \u003Cem>The Rise of the Empowered Physician in the Digital Health Era \u003C\u002Fem>published in the Journal of Medical Internet Research. \u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Atlases\nholding up an increasingly heavier, ancient world \u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>The level of physician burnout is at an all-time high. The President of the World Medical Association, Dr. Leonid Eidelman, \u003Ca href=\"https:\u002F\u002Fwww.wma.net\u002Fnews-post\u002Fwma-leader-warns-of-global-physician-burnout\u002F\">started his inauguration speech in October 2018 by addressing the “pandemic of physician burnout”\u003C\u002Fa>. At that moment, nearly half of the world’s 10 million physicians had symptoms of burnout, including emotional exhaustion, interpersonal disengagement, and a low sense of personal accomplishment, he explained.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>The growing number of chronic\ndiseases, global doctor shortages, increasing costs of medical treatment, and\nrising life expectancy all together result in an immense challenge for medical\nprofessionals. Moreover, even innovations that are supposed to help physicians\ncope with the burden tend to cost in more working hours: electronic medical\nrecords, the increasing computerization of the practice coupled with \u003Ca href=\"https:\u002F\u002Fwww.healthcarefinancenews.com\u002Fnews\u002Fphysician-pay-increases-gender-gap-widens-and-administrative-burden-increases-medscape-shows#.XLIzkbCEy9M.twitter\">the\nincrease of bureaucratic tasks\u003C\u002Fa> all add to the burnout epidemic. \u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>So\nfar it seems technology offered more benefits to patients than medical professionals.\nPatients have more information at their fingertips, have access to many more\ntechnologies, smartphone apps, and gadgets aiding their healing process. They\ncan follow their care closely, ask questions, turn to online patient\ncommunities for support, go after treatments in foreign countries, etc. It\nseems that they don’t need the too confident demigod-like physicians of the\npast anymore. They need partners who can navigate them through the jungle of\ndigital health.\u003C\u002Fp>\n\n\n\n\u003Cp>The challenges of the 21st century pull the rug from under the feet of medical professionals trying to hold up the ancient image of the doctor as Atlas holding an entire world on his back. Doctors, nurses and other caregivers are stressed and puzzled looking for their new place and function. Neither medical education nor regulations or guidelines prepare them for how to act in this new era. What should they do? Medical professionals\nare either reluctant to adopt digital health or they are clinging to the power\nthey used to have. However, they cannot bury their heads in the sand for too\nlong. A \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fdigital-health\">profound\nparadigm shift in healthcare called digital health\u003C\u002Fa>\nis underway – it doesn’t matter whether they want it or not. As this would be an immense and slow undertaking, as practical advice, we recommend starting the support for re-shaping professional competence in medical school by rewriting medical curricula. As e-patients have become experts in their illnesses, their chosen health care providers should be able to help them. For that to happen, healthcare providers should be able to form an appropriate relationship with patients. As insights about medical issues and the use of technology can now come from both sides, and they are moving toward shared decision-making, communicating extensively, and managing health and disease through teamwork, physicians should arm themselves with sufficient communication skills and the readiness to the involvement of patients in prevention and treatment.\u003C\u002Fp>\n\n\n\n\u003Cp>As\nthrough the use of portable diagnostics, telemedicine, and other\nstate-of-the-art technologies patients become the point of care, healthcare providers\nwill need to be trained in such a way that they can diagnose, treat, educate,\nand monitor patients who are far away. Rules and behavior patterns for\nsynchronous and asynchronous communication should be studied and pinned down.\nBeyond communication skills, digital literacy, as well as the skills of\nmanaging, protecting, and orienting within datasets, will also become\nirreplaceable. An e-physician will need to handle the information at hand in a\ncritical and selective manner, and educate patients on occasion about data\nmanagement issues.\u003C\u002Fp>\n\n\n\n\u003Cp>However, we are aware that these are the skills necessary in the future and as a starting point, familiarising ourselves with communication tools, social media, as well as digital health technologies, could also help ease the hardships of the transition into digital health. The high walls of the ivory tower of medicine started to crumble: vast sources of knowledge became available for anyone online, and social media channels have connected patients of many backgrounds with similar symptoms. The power of online communities, as well as empowered patients, appeared, pushing for a more equal-level doctor-patient relationship. As patients are more informed and more inclined to ask questions, they need a different type of rapport with their doctors as before.\u003C\u002Fp>\n\n\n\n\u003Cp>As digital health technologies are developing at an incredible speed, a general trend emerged to produce more and more miniaturized, digitized and connected medical devices for personal use. Telemedicine, portable diagnostics, medical drones or 3D printing push patients to become the point-of-care: to be able to measure their vital signs at home or wherever they are – not only in medical facilities. Coupled with the appearance of direct-to-consumer genetic testing and the progress of genomics as well as artificial intelligence, digital health technologies add to the move of healthcare from reactive to preventive medicine. For that, the medical community needs to set up new principles for education, and it’s also necessary to revise the Hippocratic Oath. We will elaborate on the former in the coming sections, while we enlisted our ideas for the latter \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fwhy-an-upgraded-hippocratic-oath-is-needed-in-the-digital-era\">here\u003C\u002Fa>. The Medical Futurist is aware of how hard it is to change attitudes and old habits. Thus \u003Cstrong>we would like to provide a toolkit to make the shift as smooth as possible. It should be so for medical students, professors, and physicians, too. Knowing how to search for, find and assess information; assessing the quality of medical websites; knowing how to work with empowered patients should all be basic skills for physicians of the near future.\u003C\u002Fp>\n\n\n\n\u003Cp>Besides being omnipresent, the digital terrain has another persistent characteristic: it changes constantly. Medical students should understand and embrace the idea that the patient is at the center of our universe. No matter whether it’s about developing a new drug, designing a new treatment path or shaping the blueprint of a brand new hospital building.\u003C\u002Fp>\n\n\n\n\u003Cp>Patients should be included on the advisory board of big pharma companies, of hospitals, they should get a say in organizing medical events, and a lot more. They’ll be able to better empathize with their sufferings and learn their way of thinking, how they are trying to solve their own health troubles with or without technology. They should also familiarize with the latest health and fitness gadgets very early and see how trackers, wearables, and other digital health solutions change their own lives. It is the principle of matching user’s needs with what is technologically feasible and a viable business model. If a doctor has an idea how to improve a treatment method, a drug or patient management with the help of technology, they could make improvements by themselves. Students should be familiarized with the need for both taking initiatives and acting responsibly. And they could already include new technologies as well.\u003C\u002Fp>\n\n\n\n\u003Cp>For example, \u003Ca href=\"http:\u002F\u002Fwww.anatomage.com\u002Ftable\u002F\">Anatomage Table\u003C\u002Fa> is the world’s first virtual dissection surface perfect for teaching anatomy effortlessly and spectacularly. The Hololens mixed reality device is also an excellent tool for studying the human body. There are already some universities which plan to introduce such new technologies: \u003Ca href=\"https:\u002F\u002Fwww.futurum.xyz\u002Fhow-microsoft-hololens-is-changing-medicine-and-surgery\u002F\">Case Western opens its new health education campus in collaboration with the Cleveland Clinic in 2019\u003C\u002Fa>, where students won’t learn anatomy from cadavers, they’ll learn it from virtual reality through Hololens. Moreover, the medical professional’s place in the healthcare system, as well as the whole social structure of medicine, will considerably alter soon, and without preparation, newcomers will be swept away by the waves of change. Dr. Meskó launched „\u003Cstrong>Lessons in Digital Health\u003C\u002Fstrong>” at Semmelweis Medical School in the 2017 autumn semester to experience whether and how the principles we believe in, the content we consider essential and the chosen course format could reach medical students; and whether and how they would benefit from them.\u003C\u002Fp>\n\n\n\n\u003Cp>It turned out to be a success: not only did the university allow the launch of the course again, but it even nodded consent for an advanced course based on Lessons in Digital Health.
