You see dozens every day, and they all sound grandiose and important &#8211; obviously.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>But how can you separate the wheat from the chaff? How to decide if the given report is about something important, about a technology or method that might become really significant? This is what we try to do with the team of The Medical Futurist by providing context, but our goal is that you could do it as well. \u003C\u002Fp>\n\n\n\n\u003Cp>We collected a six-question checklist to help with this. The method is simple: if the answer is yes to any 2 of these questions, the announcement is in line with the digital health trends and directions, so it is worth keeping an eye on.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">1. Why was it made? Is this addressing real clinical or patient needs?\u003C\u002Fh3>\n\n\n\n\u003Cp>The first question you need to answer is why a given technology was developed. One of the most typical shortcomings of new “digital health” technologies is the lack of focus on practical benefits.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>This affects startups and major players alike. Do you remember when NASA announced ‘\u003Ca href=\"https:\u002F\u002Fwww.sciencealert.com\u002Fnasa-surgeon-beamed-to-international-space-station-in-holoportation-world-first?fbclid=IwAR0cfT0PvMoZhn3sXKFCs0hVEtfqd-KQAKn-U-jtEUMp4iDfXQJHdemPPf4\" target=\"_blank\" rel=\"noreferrer noopener\">holoporting’ the first doctor\u003C\u002Fa> to the International Space Station? The answer is brilliant, but man, what was the question? How is that a better solution than having a video call between the astronaut and the flight surgeon? Keep an eye on new technologies that are addressing an existing pain point of healthcare professionals or patients.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Before we go along, here is one thing many of you will ask. We said only two of the six answers have to be yes to qualify as meaningful digital health tech, which implies that some solutions don’t have to address clinical\u002Fpatient needs and still be noteworthy. How does it make sense?&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Actually, it is possible for any scenario for which there is no demand \u003Cem>yet\u003C\u002Fem>, but will be later &#8211; and the underlying technology needs to be developed today. There is no demand for A.I. analysis of data provided by an at-home full-genome sequencing kit now, but I am sure there will be at some point in the future.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">2. Are there studies and clinical trials to back those claims?\u003C\u002Fh3>\n\n\n\n\u003Cp>It is always worth taking a look at how much evidence there is about any given method. Can they provide peer-reviewed studies and clinical trials to back their claims? You typically don’t have to dig deep to answer this question: anyone who has scientific verification will proudly publish it.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Looking for digital health technologies like \u003Ca href=\"https:\u002F\u002Fwww.sciencedirect.com\u002Fscience\u002Farticle\u002Fpii\u002FS2589537019300264?via%3Dihub\" target=\"_blank\" rel=\"noreferrer noopener\">the study on the Kardia 6-lead device\u003C\u002Fa> or \u003Ca href=\"https:\u002F\u002Fmedinform.jmir.org\u002F2020\u002F7\u002Fe15182\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">the data on sepsis-watch A.I\u003C\u002Fa>. will quickly lead you to studies and trials, be careful if you find none.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">3. Do both patients and clinicians gain information?\u003C\u002Fh3>\n\n\n\n\u003Cp>A principle definition of any digital health technology is that it provides relevant health information to the patient and the clinician as well. Anything outside this realm is not proper digital health. You don’t have to look for anything special, simple examples illustrate this principle.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Your smartwatch, checking your heart rate throughout the day, every day provides you with data you never had before, data that \u003Ca href=\"https:\u002F\u002Fwww.medicalfuturist.com\u002Flifestyle-medicine-could-be-the-key-for-digital-health-adoption\u002F\">might become relevant to your general practitioner\u003C\u002Fa>. Your doctor, who had checked your heart rate maybe once or twice a year before can now quickly look at a continuous stream of data, collected for months or years.  \u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">4. Is this helping to form a better relationship between patients and healthcare professionals?\u003C\u002Fh3>\n\n\n\n\u003Cp>Digital health is a road leading us toward a more balanced partnership between the patient and the clinician, where both parties add value to health management. Thus you should always take a look at how the technology in question addresses this relationship: is this helping the partnership or will just alienate the parties?&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Good examples are \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.medicalfuturist.com\u002Fvoice-to-text-technologies-shape-the-future\u002F\" target=\"_blank\">voice-to-text \u003C\u002Fa>applications, which allow healthcare professionals to focus on the patient instead of spending time with the administration of the appointment. \u003C\u002Fp>\n\n\n\n\u003Cp>Any technology that takes over non-essential tasks from doctors and nurses allowing them to practice empathy, and patient-focused care is helping the relationship. Similarly, remote monitoring models can vastly improve patients’ comfort and reduce anxiety, offering a simple and convenient way to make sure no medical intervention is needed and creating a platform to discuss questions without in-person appointments.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">5. Can we use our existing devices to access the new features?\u003C\u002Fh3>\n\n\n\n\u003Cp>New technologies like to flow in direction of the least possible resistance. We could endlessly list incredible technologies and initiatives that have great potential, but to become mainstream, they would require significant investment either from the healthcare system (the reason we don’t have \u003Ca href=\"https:\u002F\u002Fwww.medicalfuturist.com\u002Fwhere-are-3d-printed-casts\">3D printed casts\u003C\u002Fa> in every hospital), the physician (the reason why we don’t see a \u003Ca href=\"https:\u002F\u002Fwww.medicalfuturist.com\u002Fnot-in-vein-the-aimvein-pro2-0-vein-finder-review\u002F\">vein scanner\u003C\u002Fa> at every GP’s surgery) or the patient (the reason why tens of millions can’t benefit from using smart canes to support their movement). \u003C\u002Fp>\n\n\n\n\u003Cp>Offering additional (and relevant) health functionality for devices we already own and use scores great in terms of being low-resistance. Does that make sense on a global (or at least continental) scale?&nbsp;\u003C\u002Fh3>\n\n\n\n\u003Cp>Local solutions are working less and less, as digital health progress typically requires us to connect data and technology that could not have been connected earlier.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>A prime example is how Foundation Medicine’s \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.foundationmedicine.com\u002Fservice\u002Fclinical-research-and-trial-matching\" target=\"_blank\">clinical trial matching system\u003C\u002Fa> works: it matches patients to precision oncology trials based on genomic alterations, tumour type, and age. This method clearly benefits from broad access to data and patients.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Similarly, deep learning and machine learning algorithms require vast amounts of data, preferably from a heterogeneous sample to develop the best possible general models for diagnoses or case assessment. This can’t be done on a local scale.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Thinking globally also benefits patients with rare conditions: doctors and patients are able to gain access to data from all over the globe. A uniquely interesting example is the \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fgeneticalliance.org\u002Fbiobank\" target=\"_blank\">BioBank\u003C\u002Fa> of the \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fen.wikipedia.org\u002Fwiki\u002FGenetic_Alliance\" target=\"_blank\">Genetic Alliance\u003C\u002Fa>, a nonprofit organization founded by non-medical professionals, that is collecting genetic samples of patients living with rare genetic diseases on all six continents, providing a never before seen abundance of genetic samples for research.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">It’s much easier to say what is not revolutionary\u003C\u002Fh3>\n\n\n\n\u003Cp>With the 6-question checklist above you can easily screen out anything that is definitely not relevant or not digital health at all. The tricky part is that a good amount of the remaining initiatives will also fall short somewhere along the line. The pandemic highlighted how avoiding contact with a large number of random patients can be a wise idea. Besides, home-sampling of blood can provide a very efficient and convenient method for clinical trials, encouraging patient participation.\u003C\u002Fp>\n\n\n\n\u003Cp>Let’s see the technology first. The initiatives we judge as trustworthy at the moment work along the same principles: they collect capillary blood from the upper arm with the help of a vacuum. We have also worked on reducing all possible sources of bruising. However, this remains a blood collection process and on occasion, for some patients that are specifically sensitive to bruising, that can still happen.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Can the vacuum damage red blood cells?\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>Red blood cells are usually damaged by the shear forces when flow is forced through small openings, not the vacuum itself. We have worked on optimizing these parameters to reduce to the lowest possible level the amount of hemolysis.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">One upside of DIY blood draw: saving time and inconvenience\u003C\u002Fh2>\n\n\n\n\u003Cp>One potential upside is pretty obvious: you don’t need to get ready, don’t leave the house, don’t have to wrestle your way through the morning traffic without having had a bite to eat or even a coffee. You don’t get bored (and possibly catch a virus) in a crowded waiting room, and there is no commute after your blood draw.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Depending on the area where you live and the services you have access to, taking a blood test can take hours of your time, if you include travel and the waiting times &#8211; although the actual process takes less than five minutes. It is safe to assume that you are very lucky if the whole process takes less than an hour of your day, and are more likely to spend at least 2 hours or more with it. \u003C\u002Fp>\n\n\n\n\u003Cp>Consumer convenience has not traditionally been a factor taken into consideration &#8211; to safely draw blood you needed trained nurses or phlebotomists, and their capacities can be best used if they stay put and patients visit. Logical.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>So much so, that when I tried to find data about the average total time &#8211; including travel and waiting &#8211; it takes for patients to have a blood test, I couldn’t find any. Looks like nobody even cared about it. While it wouldn’t cut down the time of the sampling process, it could spare you the travel and waiting times.  \u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Argument 2: Cutting the middlemen, protecting the data\u003C\u002Fh2>\n\n\n\n\u003Cp>\u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-rise-of-at-home-lab-tests\" target=\"_blank\" rel=\"noreferrer noopener\">Direct-to-consumer (DTC) testing is booming\u003C\u002Fa> since the pandemic. What started as a necessity, quickly turned into real market demand. The reasons are manifold.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>The number of health-conscious consumers is on the rise: they want a more active role and more control in managing their health, thus they are happy to initiate and pay for specific tests that have been formerly only possible through a doctor&#8217;s order.\u003C\u002Fp>\n\n\n\n\u003Cp>Also, we have insurance companies in the equation, that might\u002Fmay not cover those specific tests, and whose coverage might (or may not) make the given test financially sound to the laboratory. Selling tests directly to the patients creates a simpler model &#8211; although not one without barriers.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>And there is one more important issue: confidential health data, that patients do not necessarily wish to share with others. DTC tests have one thing in common: the results arrive directly to the customer, who can decide if and with whom they are willing to share them.\u003C\u002Fp>\n\n\n\n\u003Cp>While anyone could get their blood work done by ordering one of these kits, many medical tests will require a larger amount of blood. Also, most patients will still require assistance in interpreting the results.
