This topic has been analysed over and over in the past two years, I also wrote about it more than once. Here, at the beginning,\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fwhen-and-how-will-covid-19-end\u002F\" target=\"_blank\"> we outlined possible scenarios\u003C\u002Fa> on how the pandemic will develop. It is in line with\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.cnbc.com\u002F2022\u002F02\u002F24\u002Fmoderna-says-covid-is-entering-an-endemic-phase-but-annual-vaccines-will-be-needed.html\" target=\"_blank\"> recent reports\u003C\u002Fa>, and the discussion about the \u003Cstrong>pandemic entering the endemic phase\u003C\u002Fstrong>. In other words: turning into a virus we can co-exist with in ‘normal’ terms. Without global lockdowns, overloaded hospitals, travel and social restrictions. One that can be kept at bay with or without seasonal vaccines.\u003C\u002Fp>\n\n\n\n\u003Cp>Something that is not defining our lives anymore.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">\u003Cstrong>So how do we know when we reach endemic phase?\u003C\u002Fstrong>\u003C\u002Fh3>\n\n\n\n\u003Cp>Naturally, COVID to become endemic is not something that happens overnight. There is not a day we can point out, and declare: here it is. We have to look for a number of clues and local\u002Fglobal trends, not only in medical data but also in policies, general behaviour of the population and the sentiment. Also, the endemic phase is not happening at the same time everywhere.\u003C\u002Fp>\n\n\n\n\u003Cp>So, let’s take a look! See where we stand now, and what the clues are that your country is getting there!\u003C\u002Fp>\n\n\n\n\u003Cp>Policy changes reflect on how healthcare systems are able to cope with the situation. When a country is easing the restrictions, it means policy makers are confident that the implemented changes will not result in overloaded hospitals.\u003C\u002Fp>\n\n\n\n\u003Ch5 class=\"wp-block-heading\">\u003Cstrong>Easing mask requirements\u003C\u002Fstrong>\u003C\u002Fh5>\n\n\n\n\u003Cp>This is in! \u003Ca href=\"https:\u002F\u002Finews.co.uk\u002Fnews\u002Fface-masks-when-not-compulsory-covid-rules-mandatory-coverings-uk-explained-1419266\" target=\"_blank\" rel=\"noreferrer noopener\">More\u003C\u002Fa> and \u003Ca href=\"https:\u002F\u002Fwww.nytimes.com\u002Fexplain\u002F2022\u002F03\u002F01\u002Fus\u002Fmask-mandates-us\" target=\"_blank\" rel=\"noreferrer noopener\">more\u003C\u002Fa> countries are eliminating mask mandates, some totally, others in certain situations. When you are not required to wear a mask in the shop, in your office, the kids can go to school without it – you are getting there.\u003C\u002Fp>\n\n\n\n\u003Cp>Normally this happens gradually. The riskiest contact situations – like using public transport, visiting healthcare facilities and so on – will be the last ones where easing the mandatory mask use is to be expected.\u003C\u002Fp>\n\n\n\n\u003Ch5 class=\"wp-block-heading\">\u003Cstrong>No travel and social restrictions\u003C\u002Fstrong>\u003C\u002Fh5>\n\n\n\n\u003Cp>Limiting movement between regions, countries and continents is a somewhat cruel but quite logical step when the virus is roaring around us. Lowering the chances of new variants getting around is fully logical as asymptomatic patients invisibly spread the virus, generating outbursts that are close to impossible to track back. Despite its logic, these restrictions are hurting our lives. If ICU units are able to manage the cases, and above all: if healthcare systems get back to normal.\u003C\u002Fp>\n\n\n\n\u003Cp>By normal we of course mean how it used to be before pandemic. This might not be great, but if you can talk to your general practitioner about your prescriptions, and don’t have to wait four days to finally get in touch, that surely is an improvement compared to the COVID-era, and a clue that we are getting to the endemic phase.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>However, COVID, or rather the capacity shortage created by the pandemic created a \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.weforum.org\u002Fagenda\u002F2022\u002F02\u002Fcancer-oncology-covid-radio-davos\u002F\" target=\"_blank\">second wave of healthcare problems\u003C\u002Fa>. The problem of everyone who could not receive a diagnosis or a necessary treatment for anything non-COVID. Cancer patients, people living with chronic diseases, or with not life threatening but uncomfortable and\u002For painful health problems. In most countries we have yet to see these accumulated backlogs disappear.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">\u003Cstrong>Rich countries’ endemic is poor countries’ pandemic\u003C\u002Fstrong>\u003C\u002Fh3>\n\n\n\n\u003Cp>As this\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.theatlantic.com\u002Fhealth\u002Farchive\u002F2022\u002F03\u002Fpandemic-global-south-disease-health-crisis\u002F624179\u002F\" target=\"_blank\"> very thorough analysis\u003C\u002Fa> points out, “By epidemic we actually mean a pandemic that no longer kills people in rich countries&#8221;. A depressing thought, nevertheless an important one. We have seen it happening with a number of different diseases from malaria to tuberculosis, from AIDS to now COVID. And not just by handing over vaccine shipments, but more importantly, handing over knowledge and technology, allowing vaccines to be manufactured locally and cost effectively.\u003C\u002Fp>\n\n\n\n\u003Cp>Unless generic drugs and vaccines are available for the less developed countries, the pandemic will continue to take its toll. Besides this being deeply inhumane, it is also not wise. The virus spreading uncontrollably among billions of people will almost certainly result in new variants emerging, annihilating the results we achieved with so much work and personal sacrifice. In other words: unless it is under control everywhere, it is not over for good.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">\u003Cstrong>5 important takeaways\u003C\u002Fstrong>\u003C\u002Fh3>\n\n\n\n\u003Cp>As the situation is getting close to normal, and we start to feel more comfortable in our new-old lives, there are some takeaways we should keep in mind:\u003C\u002Fp>\n\n\n\n\u003Cp>1.&nbsp; Let’s not forget about personal hygiene. Washing our hands often and everywhere is a habit we should not drop.\u003C\u002Fp>\n\n\n\n\u003Cp>2.&nbsp; Always look for vaccination recommendations when travelling. This is true for everyone, but especially important for the elderly and everyone living with a chronic disease.\u003C\u002Fp>\n\n\n\n\u003Cp>3.&nbsp; Just because our\u002Fyour country has the situation under control, this is not the case in other parts of the world. Unless it is under control everywhere, it is not over.\u003C\u002Fp>\n\n\n\n\u003Cp>4.&nbsp; We have an unprecedented opportunity to lay the foundations of a new kind of healthcare. Digital health-adoption – forced on us by COVID-related capacity shortage – is on an all-time high, opening avenues for development never seen before.\u003C\u002Fp>\n\n\n\n\u003Cp>5.&nbsp; Wearing masks if you don’t feel safe is perfectly fine. I will certainly do so in certain situations. We should allow ourselves plenty of time to get our confidence back in everyday situations.\u003C\u002Fp>\n\n\n\n\u003Cp>So, where do we stand now, you ask? Everything we discussed about the near-end of the pandemic is only true if we don&#8217;t see a new, dangerous virus variant emerging. If you needed to go to your physician over the past weeks, first you had to call in and describe your symptoms. The Spanish region of 7.5M inhabitants has always been one of the frontrunners of digital health technologies. Health information was handled electronically already in the 1990s! They integrated personal glucose meters and \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.linkedin.com\u002Fpulse\u002Fcatalonia-advancing-open-standards-personal-health-devices-str%C3%BCbin\u002F\" target=\"_blank\">data from mobile apps and devices\u003C\u002Fa> into their Digital Health Platform years ago. And although Spain was one of the countries which were hit hardest by the COVID-19, Catalonia was \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fpublichealth.jmir.org\u002F2020\u002F2\u002Fe19106\u002F\" target=\"_blank\">quick to turn to digital health technologies\u003C\u002Fa> facing the pandemic.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">\u003Cstrong>“You can’t stop the change, any more than you can stop the suns from setting”\u003C\u002Fstrong>\u003C\u002Fh3>\n\n\n\n\u003Cp>It is important to note that facing COVID-19, the reason why Catalonia could act fast was not only because they were ready technologically. They were ready due to their medical education. Because the entire digital health reform is \u003Cstrong>not a matter of technology\u003C\u002Fstrong>. \u003Cstrong>It is a \u003C\u002Fstrong>\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fdigital-health\" target=\"_blank\">\u003Cstrong>cultural transformation\u003C\u002Fstrong>\u003C\u002Fa>\u003Cstrong>.\u003C\u002Fstrong> Contrary to popular belief, disruptive technologies cannot bring \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fdigital-health-manifesto\u002F\" target=\"_blank\">change on their own\u003C\u002Fa>. And you don’t need to be a futurist to tell that there will be a surge in these numbers, not necessarily because there will be more sick people but because more and more people will gain access to healthcare whatsoever. There is no way to avoid the rise of telemedicine. And indeed, there has been a gigantic surge in the use of telemedicine, in particular virtual visits in the US. Over the past 3 months, \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.healthpopuli.com\u002F2020\u002F05\u002F22\u002Ftrust-my-doctor-fear-the-office-and-losing-health-insurance-in-the-covid-19-pandemic\u002F\" target=\"_blank\">telehealth usage has tripled\u003C\u002Fa>, mainly because people were simply afraid to go to the doctor. For example \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.teladoc.com\u002F\" target=\"_blank\">Teladoc\u003C\u002Fa>, a company that offers medical visits online and over the phone experienced a \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.healthleadersmedia.com\u002Ffinance\u002Fteladoc-health-visits-jump-92-q1-revenue-tops-180m\" target=\"_blank\">surge of 92% \u003C\u002Fa>(!) in remote health visits. As a result, the \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.cnbc.com\u002F2020\u002F05\u002F23\u002Fdigital-health-stocks-are-surging-amid-coronavirus-pandemic.html\" target=\"_blank\">value of remote care companies\u003C\u002Fa> began to rise.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Another part of the issue in the US is Medicare: \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.aarp.org\u002Fcontent\u002Fdam\u002Faarp\u002Fppi\u002F2019\u002F08\u002Ftelehealth-medicare-what-is-covered.doi.10.26419-2Fppi.00080.001.pdf\" target=\"_blank\">CMS started reimbursing telemedicine services\u003C\u002Fa> just before COVID hit, in January 2020. According \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.jdpower.com\u002Fbusiness\u002Fpress-releases\u002F2019-us-telehealth-satisfaction-study\" target=\"_blank\">to a study\u003C\u002Fa>, only one in 10 patients used telehealth options in the US in 2019. In 2020? Some providers reported a \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fqz.com\u002F1821549\u002Ftelemedicine-faces-unprecedented-demand-in-the-wake-of-coronavirus\u002F\" target=\"_blank\">650% growth in one  state\u003C\u002Fa>! And overall satisfaction rate with telehealth was at 89%, as of a \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.healthpopuli.com\u002F2020\u002F05\u002F22\u002Ftrust-my-doctor-fear-the-office-and-losing-health-insurance-in-the-covid-19-pandemic\u002F\" target=\"_blank\">US survey in May\u003C\u002Fa>. It is clear that even with COVID gone, telemedicine will stay. \u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">\u003Cstrong>Post-COVID Life: A Life Yet Uncertain\u003C\u002Fstrong>\u003C\u002Fh3>\n\n\n\n\u003Cp>It is not yet known how we will tackle the global pandemic. What is certain is that masks, social distancing and a general fear will most likely remain with us for quite a while – \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002F28443?post_id=28443&amp;_wpnonce=54ff8aa3b6&amp;status=future&amp;type=post&amp;preview=1\" target=\"_blank\" rel=\"noreferrer noopener\">and even some technologies, too.\u003C\u002Fa> We will also be less likely willing to go to the hospital or even to go see our own GP, probably for years to come. When (and definitely not ‘if’) telemedicine systems are set up, more and more people will try it. And as we have seen the numbers above, they will stick to it. They will not wait for hours to see a doctor. They’d rather stay at home and do chores while waiting for the doctor online! And doctors will have more time on their hands – and possibly experiencing less burnout. Looks like a win-win situation, doesn&#8217;t it?\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">\u003Cstrong>Use the Force, Luke\u003C\u002Fstrong>\u003C\u002Fh3>\n\n\n\n\u003Cp>I wish moving on to digital health solutions would be as easy as this. What is definite is that we’ll see more and more solutions becoming the norm, and that will make more and more patients use these. We have the ability to make use of these solutions as we must all become empowered and knowledgeable patients, thereby bring the future upon ourselves.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>By becoming the so-called “\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fnnlm.gov\u002Fclass\u002Fsuperpatients-patients-who-extend-science-when-all-other-options-are-gone\u002F9254\" target=\"_blank\">Superpatients\u003C\u002Fa>”, we take the first step into the age of medical digitalisation. We ourselves need to find what solutions are available in our regions\u002Finsurers or GP and take on from there. Sounds difficult?
