Eye-care is full of awesome technologies. Even if Google and Novartis&nbsp;\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.bbc.com\u002Fnews\u002Ftechnology-46262520\" target=\"_blank\">halted their glucose-sensing contact lens project\u003C\u002Fa>, there’s no need to worry – researchers \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.medgadget.com\u002F2020\u002F04\u002Fsmart-contact-lenses-measure-glucose-deliver-drugs.html\" target=\"_blank\">have worked out a method\u003C\u002Fa> for simultaneous glucose measurement and controlled drug delivery in a contact lens. This medical specialty is already bringing science fiction technologies to patients right now.\u003C\u002Fp>\n\n\n\n\u003Cp>We love inexpensive, smartphone-connected&nbsp;\u003Ca rel=\"noreferrer noopener\" href=\"http:\u002F\u002Fsciencenordic.com\u002Fsmartphone-app-can-reveal-eye-disease\" target=\"_blank\">sensors and apps\u003C\u002Fa>&nbsp;that use the phone’s camera. These can also help in diagnosing eye conditions in under-developed regions. Devices like \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.eyeque.com\u002Fpvt\u002F\" target=\"_blank\">Personal Vision Tracker\u003C\u002Fa> for measuring an individual’s refractive status, or \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fan-ophthalmologist-in-your-kitchen-the-eyeque-insight-review\u002F\" target=\"_blank\">EyeQue Insight\u003C\u002Fa>&nbsp;to determine visual acuity fit exactly into this category. And so does D-EYE. The device has a brand-new take on the traditional ophthalmoscope. Let’s see their creative solution!\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Eye candy for smartphones\u003C\u002Fh2>\n\n\n\n\u003Cp>D-EYE S.r.l. is an Italian developer of retinal screening systems for smartphones. They won numerous awards for their ideas and implementation of the project. So we were quite excited to test it.\u003C\u002Fp>\n\n\n\n\u003Cfigure class=\"wp-block-image size-large\">\u003Cimg decoding=\"async\" src=\"https:\u002F\u002Fwww.fondazioneveronesi.it\u002Fuploads\u002Fthumbs\u002F2017\u002F05\u002F10\u002Fd-eye-used-in-exam-of-a-child_thumb_720_480.jpg\" alt=\"\"\u002F>\u003Cfigcaption>Source: fondazioneveronesi.it\u003C\u002Ffigcaption>\u003C\u002Ffigure>\n\n\n\n\u003Cp>In recent studies, this fundoscopic smartphone lens lived up to the standards of researchers. It was found useful in the \u003Ca href=\"https:\u002F\u002Fpubmed.ncbi.nlm.nih.gov\u002F28306635\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">ER to quickly evaluate hypertensive retinopathy\u003C\u002Fa> or other pathological fundus abnormalities.\u003C\u002Fp>\n\n\n\n\u003Cp>D-EYE also proved to be just as practical in \u003Ca href=\"https:\u002F\u002Fpubmed.ncbi.nlm.nih.gov\u002F27513903\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">measuring vertical cup-to-disc ratio\u003C\u002Fa> as slit-lamp biomicroscopy. Moreover, it showed great promise in \u003Ca href=\"https:\u002F\u002Fwww.researchgate.net\u002Fpublication\u002F318286002_Comparison_Study_of_Funduscopic_Exam_of_Pediatric_Patients_Using_the_D-EYE_Method_and_Conventional_Indirect_Ophthalmoscopic_Methods\" target=\"_blank\" rel=\"noreferrer noopener\">pediatric settings\u003C\u002Fa> and \u003Ca href=\"https:\u002F\u002Flink.springer.com\u002Farticle\u002F10.1007\u002Fs10916-019-1477-0\" target=\"_blank\" rel=\"noreferrer noopener\">medical students and patients found it to be more comfortable\u003C\u002Fa> and educational than the usual fundoscopic exam.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">The test\u003C\u002Fh2>\n\n\n\n\u003Cp>We tested this device with Zoltán Cserháti, MD, our ophthalmologist expert to truly assess its usefulness in different fields, situations, or used by different people. Both the box and the device are modern, elegant and minimalistic, which gives D-EYE a professional look. Opening the box we find a bumper case, the D-EYE lens, a quick set-up guide and a user guide.\u003C\u002Fp>\n\n\n\n\u003Cp>The bumper is fragile, made from a low-quality material and the installation process seems to put too much stress on some weaker spots. At the moment, the bumper only fits certain iPhone models, so make sure to check compatibility before the purchase. And since the device itself is just an extension of the phone and not an electronic gadget, it truly seems durable and well-made. No battery, no buttons, no hardware issues. It’s fairly easy to use even without much technological literacy.\u003C\u002Fp>\n\n\n\n\u003Cp>After downloading the app and creating an account, we’re ready to go. The app is exactly what you would expect from a new medical device. It has a nice layout, it’s user-friendly and easy to navigate.\u003C\u002Fp>\n\n\n\n\u003Cp>Starting the app, we have the option to start a new examination or access previously recorded files and patient info. Before the examination begins, we can either choose a previous patient or add a new one to the list. Multi-shot just shoots pictures at set intervals. Although it’ll create a bigger file, we still preferred the video option. Autofocus didn’t work well, but manually setting the camera performs just fine.\u003C\u002Fp>\n\n\n\n\u003Cp>The file’s resolution is naturally lower than what you can achieve with the traditional ophthalmoscope. It depends on the phone’s camera. In most cases, ophthalmologists would still stick with a slit lamp or a traditional ophthalmoscope.\u003C\u002Fp>\n\n\n\n\u003Cp>Traditionally, fundus exams are uncomfortable for the patients: the light of the device is strong, the doctor is just a few centimetres away from their face while they’re required to stay still for the duration of the procedure.\u003C\u002Fp>\n\n\n\n\u003Cp>The D-EYE smartly resolved these issues. The light they use is not that blinding, but it’s not as customisation either. There’s no need to be so close to anyone, we only need to move the phone near the eye. And this means it’s easier to follow or correct patients’ movements.\u003C\u002Fp>\n\n\n\n\u003Cp>It worked perfectly on normal, not dilated pupils in the dark, but not in a lighter room. It’s a quick and comfortable solution for a retina test. And it works even better on dilated pupils! Using dilation drops and a dark room –the standard examination setup – offers a wider field of view.\u003C\u002Fp>\n\n\n\n\u003Cp>Another aspect we loved is how we can show the results as soon as the recording stops. Educating patients and showing them therapeutic results just became super simple. Do we recommend it at all? After discussing our experience with Dr. Cserháti, we decided to say we loved the product! \u003C\u002Fp>\n\n\n\n\u003Cp>However, as an ophthalmologist working at a public hospital, Dr. Cserháti wouldn&#8217;t replace traditional tools with D-EYE just yet. Instead, he would use it in specific situations where traditional options fail: be it bedridden patients and children, a quick consultation in another ward or teaching patients more about their conditions. Dr. Cserháti can see how new digital health technologies, like D-EYE, could replace the instruments currently used in the near future.\u003C\u002Fp>\n\n\n\n\u003Cblockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\u003Cp>&#8220;In the future, tools like this may replace those currently in use&#8230;&#8221; \u003C\u002Fp>\u003Ccite>Dr. Zoltan Cserhati\u003C\u002Fcite>\u003C\u002Fblockquote>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">A useful tool for challenging situations\u003C\u002Fh2>\n\n\n\n\u003Cp>It’s a winner when it comes to examining fidgety children and disabled patients. Furthermore, in the time of a pandemic, not being right up another person’s face is a huge advantage as well. And not just this time, really.\u003C\u002Fp>\n\n\n\n\u003Cp>The device also comes in handy when teaching students and educating patients. Let’s just say, it is great for pupils! Pun intended. It’s cheap, compared to any device currently used to take videos, yet its files are good enough quality for papers, conferences and teaching. \u003C\u002Fp>\n\n\n\n\u003Cp>It’s a great option for GPs, ER doctors, paediatricians and neurologists, too. Oh, and let’s not forget vets – they’ll love this device. D-EYE is sci-fi and fun to use. Sure, we cannot turn a blind eye on the not-so-strong bumper, lower resolution images and the limited compatibility. But at the end of the day, we were pleased with how it performed.\u003C\u002Fp>\n\n\n\n\u003Cp>Eyes are our most important sensory organ. 80-85 percent of our perception, learning, cognition, and activities are mediated through vision. That means on average at least one in seven people have trouble seeing perfectly. That’s an insanely huge number – and The Medical Futurist team is not an exception here either. The majority has glasses or problems with their eyes for a long time.\u003C\u002Fp>\n\n\n\n\u003Cp>I only realized as an adult that I have problems with my vision, I’m short-sighted – so I have to live with a “telescope” on my head, but it proved to be great, especially while driving. I go to the ophthalmologist yearly to check whether my sight worsened. Rozina, our marketing guru, said she really loved to go to the ophthalmologist when she was a kid because she always had to read a board full of smileys and the entire exam seemed like a game for her. She was diagnosed with astigmatism and had her first glasses when she was three or four years old, but fortunately could put them down by the age of seven. Currently, she only wears the frames as her eyes are dry and glasses help in mitigating headaches.\u003C\u002Fp>\n\n\n\n\u003Cp>Nora, our editor-in-chief, has been wearing eyeglasses since she was 10 years old due to nearsightedness. At first, she only needed minus 0.5 diopters and the doctor said she didn’t have to wear it all day, but she loved the colorful frames so much that no one could have convinced her to put them down. Not like her classmates: when the fourth-graders had to go to the doctor for an eye check, many were really afraid of getting glasses. As the eyesight exam was a recurring event in school each year, some already knew what to do. Several start-ups offer point-of-care eye tests which enable to examine eye health at home and thus, people only need to go to the doctor when they have a symptom to cure. The EyeQue Insight portable device is one of these solutions, and The Medical Futurist team had the chance to try it. Briefly, we loved it. If you are interested in more details, read on.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>The EyeQue and its technology\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>The company was founded in 2015 on a quest to develop optical smartphone attachments and mobile applications that let people test their own vision anywhere – without the need to go to an ophthalmologist and sit in front of room-sized ophthalmic refractometers. The Newark-based \u003Ca href=\"https:\u002F\u002Fwww.eyeque.com\u002Fblog\u002Fwhy-eyeque-the-technology-behind-the-products\u002F\">EyeQue has an MIT patented technology\u003C\u002Fa> based on instruments and charts used in vision testing for years. They offer two products enabling low-cost and accessible eye tests for everyone. \u003Ca href=\"https:\u002F\u002Fwww.eyeque.com\u002Fpvt\">The Personal Vision Tracker\u003C\u002Fa> measures an individual’s refractive status, including near-or-farsightedness and astigmatism, while the \u003Ca href=\"https:\u002F\u002Fwww.eyeque.com\u002Finsight\">EyeQue Insight\u003C\u002Fa> determines visual acuity or the sharpness of vision.\u003C\u002Fp>\n\n\n\n\u003Cp>As explained on their website, the Insight employs a binocular viewing device attached to a smartphone and presents a chart of randomly rotated “E” patterns or letters of different sizes. The viewer is asked to swipe the smartphone screen in the direction of the opening of the “E”. Just like the traditional vision testing, it measures each eye separately and then together, revealing the visual acuity on the smartphone screen upon completion. The instrument itself is not heavy, and if you have glasses like me, you will be able to use the gadget both with on and off your frames.\u003C\u002Fp>\n\n\n\n\u003Cp>You can purchase the EyeQue for less than 100 dollars (within 30 days you get your money back if you are not satisfied), but it’s useful to know that the company is only shipping to the US, Canada, UK, Spain, Germany, France, Italy, Ireland, Australia, and New Zealand at the moment. After you receive the product, you are told to create a personal profile on the company website and download the app. The instrument requires an active internet connection and subscription to the EyeQue service. Both the software and the hardware are user-friendly enough, thus they have built-in tutorials and practice tests to set up the gadget and the app appropriately. It was also straightforward how to use the on-board pupillary distance estimator to set the optics of the screener.\u003C\u002Fp>\n\n\n\n\u003Cp>Afterward, you can choose kid or adult mode. For kids, the company created the test as a cartoon with a little blue figure telling them jokes and rewarding the child for taking the test. In my case, it was accurate. As it is useful to check your eyesight every now and then, the device’s best quality will appear after a while: it tracks vision changes, which is very useful to see whether your vision improves or deteriorates over time. In the latter case, you will have a clearer picture of the situation and discuss any further step with your ophthalmologist.\u003C\u002Fp>\n\n\n\n\u003Cp>Besides, the instrument tests contrast sensitivity and color deficiency, basically color blindness. The former could determine if you have problems seeing variations of light and dark indicating conditions such as glaucoma or cataract. Fortunately, I have no major issues with contrast sensitivity.\u003C\u002Fp>\n\n\n\n\u003Cp>The testing process has two pitfalls which the company should definitely improve. It is not easy to strap the phone to the device, and some users even experienced that it attaches too strongly when it’s on. For me, the details were understandable and clear, but another physician, Dr. Alina Doe also confirmed my uncertainty about it. She believes the instrument tests refractive error well, but she noted it’s important to always remember that refractive error is just one indicator of eye health; these vision tests aren’t meant to replace visits to an optometrist or ophthalmologist. The retina and pressures need to be checked as well as other tests need to be done yearly.\u003C\u002Fp>\n\n\n\n\u003Cp>Her Hungarian colleague, Dr. Zoltán Cserháti, said that the company’s Glossary of Terms in the report about the results comes in handy for anyone who wants to understand the jargon. Moreover, the charts and details could prove to be important for optometrists\u002Fophthalmologists, not just for the user itself. He also likes the fact that setting up the right pupillary distance and being able to switch between adult\u002Fchild modes make the gadget more personal.
