We are at the dawn of \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fdigital-health\" target=\"_blank\" rel=\"noreferrer noopener\">digital health\u003C\u002Fa>. \u003C\u002Fp>\n\n\n\n\u003Cp>A cultural transformation of how disruptive technologies that provide digital and objective data accessible to both caregivers and patients leads to an equal level of \u003Cem>doctor-patient relationship with shared decision-making and the democratisation of care.\u003C\u002Fem>\u003C\u002Fp>\n\n\n\n\u003Cp>The essential shifts and trends are already prevalent in the definition.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">(1) \u003Cstrong>Access to medical data and better care\u003C\u002Fstrong>\u003C\u002Fh3>\n\n\n\n\u003Cp>Unlike before, digital technologies, social media, wearables, and trackers provide medical data and knowledge to many more stakeholders in healthcare beyond doctors. That means the walls of the isolated ivory tower of medicine are crumbling, enabling better access to medical data and through that, better access to care.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">(2) \u003Cstrong>A shift in the patient-doctor relationship\u003C\u002Fstrong>\u003C\u002Fh3>\n\n\n\n\u003Cp>At the same time, the status of medical professionals, first of all, doctors, as the sole repositories of medical knowledge ceases to exist as we know it. As patients have access to online medical information sources, patient communities on social media, wearables and health sensors providing medical data, they could present more questions and more knowledge related to their own health. The asymmetrical doctor-patient relationship placing responsibility and hope solely in the hands of the physician will shift towards a more equitable partnership. The patient might assume a more “mature” position, while the doctor might become a guide in the jungle of digital health instead of being a declaratory reference point.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">(3) \u003Cstrong>Physicians are teaming up with technology\u003C\u002Fstrong>\u003C\u002Fh3>\n\n\n\n\u003Cp>Doctors have been aided by technology for centuries, however, in most cases, this only meant auxiliary tools and their impact was rather negligible. However, with the emergence of the industrial revolution in the 18\u003Csup>th\u003C\u002Fsup>-19\u003Csup>th\u003C\u002Fsup> century, more and more medical instruments appeared and their gravity in healing increased. Currently, and in the future more than ever, \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-7-must-haves-for-the-doctor-of-the-21st-century\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">physicians should team up with technology\u003C\u002Fa> to provide their best performance and the most efficient care to their patients. The need for human contact and empathy will be more critical than ever in our technologised environment. Also, neither artificial intelligence nor virtual reality or health trackers will be able to solve complex cases, so in general, creativity and problem solving will become skills to nurture and look for in the future generation of medical professionals.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>How will medical specialties change due to the technological revolution?\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>As the landscape of medicine is as diverse as the metabolic composition of the human body, technological change affects the various medical specialties differently. While it is impossible to predict accurately to what extent the individual clinical areas will be influenced by technology, we have certain “strong guesses” about the ones most and least impacted. Fields using easily repeatable and computerisable tasks, such as administrative duties or the prescription of medicines, could expect shifts.\u003C\u002Fp>\n\n\n\n\u003Cp>Areas where natural language processing and computer vision, the fastest growing fields of artificial narrow intelligence (ANI) could be applied, are better to prepare for the coming transformation; and the domain where the novelties of precision medicine could be introduced, will all be prone to change.\u003C\u002Fp>\n\n\n\n\u003Cfigure class=\"wp-block-image\">\u003Cimg loading=\"lazy\" decoding=\"async\" width=\"788\" height=\"870\" src=\"https:\u002F\u002F1712507217.rsc.cdn77.org\u002Fwp-content\u002Fuploads\u002F2018\u002F11\u002F000.png\" alt=\"medical specialty\" class=\"wp-image-22329\" srcset=\"https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2018\u002F11\u002F000.png 788w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2018\u002F11\u002F000-768x848.png 768w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2018\u002F11\u002F000-464x512.png 464w\" sizes=\"auto, (max-width: 788px) 100vw, 788px\" \u002F>\u003C\u002Ffigure>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Diagnostics, radiology or primary care top the list\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>The illustration shows that medical specialities that rely heavily on medical imaging and laboratories, will be significantly influenced by the digital health revolution including radiology, pathology – and other non-diagnostic fields, such as nephrology, rheumatology, haematology and \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Ffuture-of-dermatology\" target=\"_blank\" rel=\"noreferrer noopener\">dermatology\u003C\u002Fa>.\u003C\u002Fp>\n\n\n\n\u003Cp>In the case of \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-future-of-radiology-and-ai\" target=\"_blank\">radiology\u003C\u002Fa>, numerous signs point in the direction that the use of artificial intelligence will prove to be revolutionary. The close cooperation of A.I. and radiology in clinical practice is nigh. The FDA approved the first cloud-based deep learning algorithm for cardiac imaging developed by \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Farterys.com\u002F\" target=\"_blank\">Arterys\u003C\u002Fa> already in 2017. According to some estimations, within 2-3 years we’ll have many machine learning algorithms in active clinical pilot testing and approved use. That’s why many radiologists got the idea that A.I. might replace radiologists soon. Yet, I believe instead that A.I. will augment their jobs and free them from plenty of monotonous and repetitive tasks.\u003C\u002Fp>\n\n\n\n\u003Cp>Regarding \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fdigital-technology-make-an-impact-on-primary-care\" target=\"_blank\" rel=\"noreferrer noopener\">primary care\u003C\u002Fa>, technology can help “manage” overcrowded waiting rooms. Wearable sensors and devices that stream data to a GP’s smartphone, notifying them whenever vital signs of patients are acting up will provide doctors with all the necessary data for delivering care. Thus, chronic patients will not have to travel to their physician only for regular check-ups. Moreover, chatbots and smartphone-based digital assistants could diagnose minor conditions and offer simple treatment advice remotely in the future, which would spare people in rural communities or hard-to-reach places the trip to the doctor. Chatbots could even become the first line of primary care – meaning that patients might only get a doctor’s appointment if the A.I.-based chatbot could not offer sufficient medical help.\u003C\u002Fp>\n\n\n\n\u003Cp>In the field of \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fprecision-medicine-best-hope-fight-cancer\" target=\"_blank\">oncology\u003C\u002Fa>, A.I., nanotechnology, and genetics all could bring the change we all wish to see. Oncologists already customise therapies based on patients&#8217; genetic backgrounds and their tumours’ molecular makeup. Cheaper genome sequencing and measuring blood biomarkers are speeding up this process. \u003C\u002Fp>\n\n\n\n\u003Cp>Smart algorithms are designed to offer personalised treatments for any cancer type or patient faster than any traditional healthcare service. All specialities where human factors such as empathy and caring attention are the core of the practice, high levels of creativity and problem solving, and quick decision-making abilities will override any technological solution in the majority of cases.\u003C\u002Fp>\n\n\n\n\u003Cp>In the case of psychiatry and psychology, therapists are irreplaceable – and it will remain so for at least as long as \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Falgorithms-robots-mimic-empathy\" target=\"_blank\">robots don’t learn how to mimic empathy\u003C\u002Fa>. This doesn&#8217;t mean however that digital health will not knock on the door. \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-new-buzz-these-are-the-top-examples-of-digital-therapeutics\u002F\" target=\"_blank\">Digital therapeutics (DTx) solutions\u003C\u002Fa> are \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fdtxalliance.org\u002Funderstanding-dtx\u002Fproduct-library\u002F\" target=\"_blank\">getting ripe\u003C\u002Fa>, and there are already a number of FDA-approved, prescription-only solutions increasing the efficiency of therapies fighting addictions or depression. \u003C\u002Fp>\n\n\n\n\u003Cp>Of course, conversations about the psyche are usually complicated and stuffed with insightful non-verbal communication, which machines are unable to catch or explain. It is highly unlikely that algorithms or robots could react to patients’ sentences in the right way, which occurrence truly needs the ability to pay attention and empathise with the other in a profoundly human manner. \u003C\u002Fp>\n\n\n\n\u003Cp>Based on the same argumentation, \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-future-of-nurses-superheros-aided-by-technology\" target=\"_blank\">nursing\u003C\u002Fa> cannot be based on technological grounds, but it will be assisted in many ways for physical, repetitive and non-essential tasks. The \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fwould-you-let-a-robot-take-your-blood-sample\u002F\" target=\"_blank\">blood-drawing robots\u003C\u002Fa> have arrived, and others are on their way to help nurses \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.nature.com\u002Farticles\u002Fd41586-022-00072-z\" target=\"_blank\">lift\u003C\u002Fa> and \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.newscientist.com\u002Farticle\u002F2315052-robotic-nurse-can-dress-a-mannequin-in-a-hospital-gown\u002F\" target=\"_blank\">dress \u003C\u002Fa>patients. A.I. solutions will aid them to filter out irrelevant noise and \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.virtusense.ai\u002Fblog\u002Fan-ai-solution-to-nursing-alarm-fatigue-and-decision-fatigue\" target=\"_blank\">prevent alert fatigue\u003C\u002Fa>.  \u003C\u002Fp>\n\n\n\n\u003Cp>Regarding \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Ffuture-emergency-medicine-innovations-making-patients-point-care\" target=\"_blank\">emergency medicine\u003C\u002Fa>, portable diagnostic devices or medical drones can significantly reduce the time until a patient receives a proper diagnosis or specific treatment, but the ambulance crew will always remain indispensable on the spot. They will however receive immense support from a number of A.I. (like \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedcitynews.com\u002F2022\u002F05\u002Frapidai-gets-fda-clearance-for-ai-designed-to-detect-pulmonary-embolism\u002F\" target=\"_blank\">this one detecting pulmonary embolism\u003C\u002Fa>) and machine learning algorithms (like this one \u003Ca href=\"https:\u002F\u002Fwww.mobihealthnews.com\u002Fnews\u002Fstudy-ml-algorithm-helps-detect-traumatic-intracranial-hemorrhage-using-prehospital-data\" target=\"_blank\" rel=\"noreferrer noopener\">detecting intracranial haemorrhage\u003C\u002Fa> using information collected before patients reach the hospital). \u003C\u002Fp>\n\n\n\n\u003Cp>The ambulance crew will always make the decisions about the patient’s state, provide the initial diagnosis and an amount of care which guarantees the individual will arrive in the hospital. However, we could discern some general skills and principles based on the requirements of the digital age and technologically boosted healthcare. If someone acquires this knowledge, the waves of innovations could bring new tools and gadgets for them, but the grounds will be as firm as a rock in the face of any transformation.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">\u003Cstrong>1) Patient design \u003C\u002Fstrong>\u003C\u002Fh3>\n\n\n\n\u003Cp>For decades, the medical community has left the single most important player out of every major decision regarding healthcare: the patient. Medical students and doctors should understand and embrace the idea once and for all that the patient is at the centre of our universe. This is the basis of both modern and digital healthcare. No matter whether it’s about developing a new drug, designing a new treatment path or shaping the blueprint of a brand new hospital building.\u003C\u002Fp>\n\n\n\n\u003Cp>Patients should be included on the advisory board of big pharma companies, of hospitals, they should get a say in organising medical events, and a lot more. Social media can be incredibly useful not only for solving medical mysteries but also in the case of rare diseases. The National Institute of Health, America’s medical agency, \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.economist.com\u002Fnews\u002Fscience-and-technology\u002F21651765-many-hands-make-diagnosis-work-crowd-will-see-you-now\" target=\"_blank\">recognises 7,000 rare illnesses\u003C\u002Fa> – defining as rare the ones that affect fewer than 200,000 people each. A general practitioner cannot possibly remember all of these. That’s where for example CrowdMed comes in. Patients submit their cases to the site, and medical students, retired doctors, nurses or even laypeople offer their potential diagnoses. The ones whose medical assessment was accurate for the case receive some reward: a few dollars and a rise in the system’s ranking.\u003C\u002Fp>\n\n\n\n\u003Cp>Beyond CrowdMed, many doctors use Twitter for asking around about cases. Using hashtags like #quickcase, many posts unidentifiable pictures about patients specifying their problems and\u002For possible solutions and ask for help to move forward. Five-to-ten years from now, \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-future-of-radiology-and-ai\u002F\">radiology departments in the U.S. will actively harness artificial intelligence\u003C\u002Fa> to treat patients. Five-to-ten years from now, pharmacies in \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Ffuture-3d-printing-drugs-pharmacies-closer-think\u002F\">hospitals will 3D print personalized drugs\u003C\u002Fa> in specific dosages.\u003C\u002Fp>\n\n\n\n\u003Cp>Yet, when do medical students first meet artificial intelligence, surgical robots or 3D printing? The medical curriculum today does not prepare students for the world they will face when they start practicing medicine. Why?\u003C\u002Fp>\n\n\n\n\u003Cul class=\"wp-block-list\">\u003Cli>\u003Cstrong>No training in administration\u003C\u002Fstrong>. A big part of a physician’s job is spent with administration which students do not learn in school.\u003C\u002Fli>\u003Cli>\u003Cstrong>Not teaching essential skills\u003C\u002Fstrong>. The world of technology is changing so fast that instead of teaching them how to use certain technologies, we should give them skills so they can make their own assumptions in the future.\u003C\u002Fli>\u003Cli>\u003Cstrong>Patients are out of sight\u003C\u002Fstrong>. Curriculums today do not focus on the needs of patients.\u003C\u002Fli>\u003Cli>\u003Cstrong>Outdated\u003C\u002Fstrong>. In general, the curriculum today is like it was 50 years ago, not much has changed. All the while patients are becoming empowered; the practice of medicine is filled with more and more technologies.\u003C\u002Fli>\u003Cli>\u003Cstrong>Not encouraging life-long learning\u003C\u002Fstrong>. Disruptive technologies start to replace jobs and nobody, even in medicine, is safe without constantly learning and improving. But how would med schools teach this attitude if they are far behind the latest innovations and are not eager to catch up?\u003C\u002Fli>\u003C\u002Ful>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>What should change and how?\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Instead of only burying students in libraries with gazillions of books for memorizing data, they should get a chance to acquire proper digital literacy skills and a general overview of digital health too. These latter two will enable them to get the most out of any healthcare system and to become exceptional physicians since I believe it is not enough today to study medicine and grow into a good doctor based on&nbsp;formal studies and experience gained during residency years.\u003C\u002Fp>\n\n\n\n\u003Cp>Technology is so much ahead of average practitioners that it&#8217;s sometimes painful to see. And problems start when empowered patients bring their wearables’ data to the GP. Oh wait, they have already started it, just look at headlines like \u003Ca href=\"http:\u002F\u002Fwww.huffingtonpost.co.uk\u002Fentry\u002Fpatricia-lauder-fitbit-saved-her-life-by-alerting-doctors-to-blood-clots_uk_58e622f4e4b0917d347760d3\">73-year-old’s Fitbit saved her life by alerting doctors to blood clots in her lungs.\u003C\u002Fa>&nbsp; What if the doctor couldn&#8217;t have dealt with the wearable data? Thus, technology should be an organic part of the medical practice from day one – and it should be reflected by the university curriculums. They’ll be able to better empathize with their sufferings and learn their way of thinking, how they are trying to solve their own health troubles with or without technology. They should also familiarize with the latest technologies very early and see how trackers, wearables, and other digital health gadgets change their own lives. Only if they truly now the latest innovations could they later help patients.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">\u003Cstrong>2) Design thinking\u003C\u002Fstrong>\u003C\u002Fh3>\n\n\n\n\u003Cp>Although the approach originates from design, it already spilled over to business life – and I hope soon will be commonplace in healthcare. It is the principle of matching user’s needs with what is technologically feasible and a viable business model. If a doctor has an idea how to improve a treatment method, a drug or patient management with the help of technology,&nbsp;they could make improvements by themselves. Moreover, DYI workshops, such as Makerspaces could even help make things possible.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">\u003Cstrong>3) Understanding digital health technologies and the cultural changes they induce\u003C\u002Fstrong>\u003C\u002Fh3>\n\n\n\n\u003Cp>It is of utmost importance to familiarize students with technologies that will shape their medical practice in the coming years. Artificial intelligence, VR\u002FAR, 3D printing, robotics or cheap genome sequencing will all change how we know medicine today. Empowered patients challenge doctors and expect medical professionals to treat them as equal partners when designing their treatment plans and carry out their therapy. Physicians could learn from them a lot!\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">\u003Cstrong>5) Dealing with ethical issues due to new technologies\u003C\u002Fstrong>\u003C\u002Fh3>\n\n\n\n\u003Cp>Who will have the responsibility when a machine learning algorithm misses a nodule on a chest X-ray? How far should genome editing methods go in improving babies? Where are the boundaries of private patient data? Plenty of serious ethical questions and concerns arise due to the latest innovations, and it is important for the young generation to think about and discuss all these dilemmas in details.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">\u003Cstrong>6) Advanced medical communication\u003C\u002Fstrong> \u003Cstrong>coupled with digital literacy\u003C\u002Fstrong>\u003C\u002Fh3>\n\n\n\n\u003Cp>\u003Cem>The profession of practicing medicine is based on communication.\u003C\u002Fem> And as social media and other digital technologies play a major role in today’s communication, digital literacy must be included in the medical curriculum. The value of social media has been demonstrated several times in medicine and healthcare, therefore it is time to prepare medical students for the conditions they will have to face when they graduate. The Hololens mixed reality device is also a great tool for studying the human body. There are already some universities who plan to introduce such new technologies: \u003Ca href=\"https:\u002F\u002Fwww.futurum.xyz\u002Fhow-microsoft-hololens-is-changing-medicine-and-surgery\u002F\">Case Western opens its new health education campus in collaboration with the Cleveland Clinic in 2019\u003C\u002Fa>, where students won’t learn anatomy from cadavers, they’ll learn it from virtual reality through Hololens. I hope that more and more med schools will follow their example.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">\u003Cstrong>8) Gamification based learning materials\u003C\u002Fstrong>\u003C\u002Fh3>\n\n\n\n\u003Cp>I’m not saying that memorizing information and learning from age-old pearls of wisdom is not a feasible way to get to important medical knowledge. But would you not use gamification methods to make the learning process easier and more effective? Are we not supposed to also have some fun while studying? There are many gamification-based learning materials or \u003Ca href=\"https:\u002F\u002Fwww.picmonic.com\u002F\">knowledge testing sites\u003C\u002Fa> already out there to break away from traditional studying and motivate more parts of our brains.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">\u003Cstrong>9) Professor-student equal-level partnership\u003C\u002Fstrong>\u003C\u002Fh3>\n\n\n\n\u003Cp>As the patient-doctor relationship turns from a hierarchical, asymmetric power relationship into an equal-level partnership, the same could happen with the professor-student rapport. This was the first time the most prestigious Hungarian university allowed the launch of such a special course and as there were too many applicants, I pre-selected eager international students based on introductory essays about digital health. I used the format I learned as a student at Harvard Extension School, as I was interested whether it could be implemented elsewhere. It turned out it could. During the 10 weeks, \u003Cstrong>we discussed telemedicine, communities, information management, health sensors, wearables, genetic testing, privacy issues, ethical concerns, new technologies\u003C\u002Fstrong>, and many more. Students had to discuss in groups how\u003Cstrong> they would design the best health sensor\u003C\u002Fstrong> or how they would build up a telemedicine application to get the most optimal outcome. At first, they had to \u003Cstrong>write a science fiction short story about a medical professional who lives in a paradigm shift\u003C\u002Fstrong>; perhaps during the 18th century when the first vaccinations were used or in the late 19th century when Wilhelm Conrad Röntgen discovered the X-ray. For their second essay, \u003Cstrong>they had to come up with a news article from the future\u003C\u002Fstrong>. Another piece of science fiction discussing all the ethical concerns around digital health technologies through a dystopia or utopia. They planned a card game for the group to illustrate the nature of probability and planning in epidemiology. One part of the class had to come up with factors facilitating the spread of the pest, the other part with elements hindering the escalation of the epidemic. Afterwards, two pack of cards and chance helped decide whether the pest could be contained in an imagined country. Exciting!\u003C\u002Fp>\n\n\n\n\u003Cp>As it seems it was not only me who enjoyed the format and the content of the course. In the evaluation, one student wrote: \u003Cem>Thoroughly enjoyed the uniqueness and openness of the course. Enjoyed working on new projects that were far removed from the usual boring things and hearing other people’s ideas. Most of all I learned even more about the future of medicine and the direction in which it’s going, I actually learned more than I thought I would! \u003C\u002Fem>Other students thought that more group projects, discussions and debates have made&nbsp;the course even more thought-provoking. But I was really glad that many gave me the feedback that now they have an opinion about issues in digital health – and this was the aim of the course. Every teacher lives for feedback like this. \u003Cem>I really enjoyed attending the classes. I can say with confidence that throughout my 5 years of medical schooling, this course has been by far my favorite (and it&#8217;s an elective!!!) and the one that I have been the most active and interested in. (off the record: I even skipped my practices on Tuesdays to attend the class because they are so inspiring to me and I didn&#8217;t want to miss any!)
