NHS doctor Rashid Riaz, from Hereford, borrowed the device from a flight attendant to check the patient&#8217;s oxygen levels.\u003C\u002Fp>\n\n\n\n\u003Cp>&#8220;The Apple Watch helped me find out the patient had low oxygen saturation,&#8221; the medic explained. Later, he also called on all airlines to consider having emergency physician kits as standard, which would ideally include tools to take basic measurements, diabetic and blood pressure meters, and an oxygen saturation monitor.\u003C\u002Fp>\n\n\n\n\u003Cp>We all know that aircraft have some medical supplies on board, but it is kind of surprising that something as simple as a pulse oximeter is not part of it. But how did we get here in the first place? And how to proceed to better utilise portable technologies of the 21st century? This is the topic we discuss here. \u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">The curious case of flight medicine\u003C\u002Fh2>\n\n\n\n\u003Cp>As flight transitioned from an experimental hobby by a small group of daredevils to a huge, regulated industry, the field of aviation medicine evolved in parallel. It started with pioneers like \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.ncbi.nlm.nih.gov\u002Fpmc\u002Farticles\u002FPMC1312212\u002F\" target=\"_blank\">Dr. Benjamin Rush, the only physician who signed the U.S. Constitution\u003C\u002Fa> and an avid supporter of ballooning, who wrote that “the time may come when this mode of sailing will be employed…with as much safety as sailing upon the water, and with much more advantage.” He proved to be right.\u003C\u002Fp>\n\n\n\n\u003Cp>But we could also mention French physiologist Claude Bernard, the ‘Father of Aviation Medicine’, who researched the effects of altitude on the human body and developed the first hypobaric chamber. Later on, the Germans were the first to develop a minimal set of \u003Ca rel=\"noreferrer noopener\" href=\"http:\u002F\u002Fgoflightmed.wpengine.com\u002Faeromedical-standards-and-fitness-for-flight\u002F\" target=\"_blank\">aeromedical standards\u003C\u002Fa> in 1910, which were soon imitated by the Italians, the British, and the French. The United States followed this trend, publishing instructions for aviation physical examinations in 1912 and then actual aeromedical standards in 1916.\u003C\u002Fp>\n\n\n\n\u003Cp>It was just one step from here that the first American flight surgeon was appointed, and aeronautics spilled over from the territory of the army to the commercial area. \u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Prepared for mid-air emergencies \u003C\u002Fh2>\n\n\n\n\u003Cp>Currently, the world’s \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.oag.com\u002Fblog\u002Fexploring-the-worlds-10-longest-non-stop-airline-routes\" target=\"_blank\">longest non-stop commercial flight\u003C\u002Fa> takes an average of 18 hours and 40 minutes, and it takes passengers from New York JFK to Singapore. Any medical emergency can happen during such a long flight – or during any flight for that matter – for which the crew has to prepare. A 2013 study found that there is one medical emergency for every 604 flights, though the actual number is likely higher, as not all incidents are reported. Worldwide, about 144-160 flights a day have an urgent medical matter.\u003C\u002Fp>\n\n\n\n\u003Cp>The same research found that the most common in-flight medical emergencies are fainting (syncope) (37%), respiratory difficulties (12%), nausea and vomiting (9.5%), followed by cardiac symptoms (7.7%), seizures (5.8%), and abdominal pain (4%). But \u003Ca href=\"https:\u002F\u002Fnews.aamc.org\u002Fpatient-care\u002Farticle\u002Fis-there-doctor-onboard\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">doctors also see their share of complaints like allergies, anxiety, ear pain, and headaches\u003C\u002Fa>. Cardiac arrest accounts for just 0.3% of in-flight emergencies.\u003C\u002Fp>\n\n\n\n\u003Cp>That’s why the flight crew has to be prepared: according to the Federal Aviation Administration (FAA), all flights should carry oxygen, equipment for closely monitoring a patient, analgesics, a bronchodilator inhaler, oral antihistamine, an automated emergency defibrillator (AED) to re-start a heart in cardiac arrest, and a first-aid kit with bandages, splints, and ace wraps, \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.faa.gov\u002FdocumentLibrary\u002Fmedia\u002FAdvisory_Circular\u002FAC121-33B.pdf\" target=\"_blank\">among others\u003C\u002Fa>. \u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Portable diagnostics offers brilliant tools for in-flight emergencies\u003C\u002Fh2>\n\n\n\n\u003Cp>Over the years, the content of the must-have emergency medical kit grew, but there is plenty of room for improvement. \u003C\u002Fp>\n\n\n\n\u003Cp>Some airlines have already recognized the gap between the regulatory must-haves and the &#8220;technically possible&#8221; regarding the content of their medical bags. For example, \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.foxbusiness.com\u002Ffox-news-health\u002Fdelta-launches-new-health-technologies-flight-medical-emergencies-safe-possible\" target=\"_blank\">Delta announced a few months ago\u003C\u002Fa> that they are rolling out a new policy that includes additional \u002F better devices compared to the FAA list. Like a cardiologist-grade stethoscope, as, according to their chief health officer, the required device is little more than a toy. \u003C\u002Fp>\n\n\n\n\u003Cp>But we could do more. Having a good stethoscope is great. It turned out that several years before, he had had a stent placed for a coronary artery obstruction. So, Dr. Topol took out his AliveCor ECG for iPhone, opened up the gentleman’s shirt and placed it on his anterior chest. Within seconds, it turned out that the guy was having an acute heart attack. He immediately asked the captain to land the plane. Without the AliveCor ECG, Dr. Topol couldn’t have made the diagnosis, and the man couldn’t have been saved. We could include devices like the smart, ECG-equipped stethoscope, the \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.youtube.com\u002Fshorts\u002FWn5nG7EmTJw\" target=\"_blank\">credit card-sized ECG from Kardia\u003C\u002Fa>, an \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.youtube.com\u002Fwatch?v=_MvImEQhUzM\" target=\"_blank\">ultra-portable, AI-enhanced ultrasound \u003C\u002Fa>device, or the \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Ffirst-medical-tricorder-viatom-checkme-pro-review\" target=\"_blank\">Viatom Checkme, the first diagnostic tool that resembles Star Trek’s medical tricorder\u003C\u002Fa>. The advantages of these gadgets are that they spit out the results in seconds, and the data is easy to share with medical assistance on the ground.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Is there a doctor on board?\u003C\u002Fh2>\n\n\n\n\u003Cp>We all know this question, if we were lucky enough to not hear it in person, it is familiar from movies and TV shows. Not all passengers are lucky enough to have an emergency specialist, or cardiologist with portable devices in his pocket on board, but usually there is some kind of doctor on hand. They aren’t on their own with the emergency kit when it comes to an emergency, as flight attendants are trained in CPR and the use of the AED.\u003C\u002Fp>\n\n\n\n\u003Cp>More importantly, most airlines have remote assistance from \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Feu.usatoday.com\u002Fstory\u002Ftravel\u002Fcolumnist\u002F2023\u002F08\u002F02\u002Fair-ground-medical-hotline-cruising-altitude\u002F70506707007\u002F\" target=\"_blank\">services like MedAire and STAT-MD\u003C\u002Fa>. The doctors of these companies guide the on-flight team in evaluating and stabilising patients. Still, a mid-air medical emergency needs more support – as being caught in such a situation as a doctor could feel like being trapped with a patient on Mars. For a start, \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fdigital-health-trends-what-to-expect-in-2019\" target=\"_blank\">the seat could act like a biometric sensor, just as in recent developments in the car industry\u003C\u002Fa>. Imagine the seat measuring your vital signs, sending the data to the cloud and sending alerts to the flight crew in case a medical event is about to emerge. This way, the predictive element could also be brought to mid-air, which has so far been impossible to imagine in connection to a medical hardship during a flight.\u003C\u002Fp>\n\n\n\n\u003Cp>On another note, sitting in one place for hours could increase a person’s deep venous thrombosis risk, thus, a smartphone application that would encourage the user to get up and walk after a pre-programmed period could prove to be a useful flight supporter, recommended Dr. Adam Osomanski, a prehospital and retrieval medicine specialist in talking to The Medical Futurist. He also mentioned that it might be useful to have certain digital fatigue and stress level management tools in place for the crew, which also enhances health and safety on board.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Comfort technologies for passengers\u003C\u002Fh2>\n\n\n\n\u003Cp>On the other hand, airlines could offer a lot more comfort technologies for their passengers in the future, enhancing their well-being and thus preventing medical emergencies. The videos, made in partnership with \u003Ca rel=\"noreferrer noopener\" href=\"http:\u002F\u002Fthemindfulnessinstitute.com\u002F\" target=\"_blank\">The Mindfulness Institute\u003C\u002Fa>, apply meditative techniques to the uniquely nerve-wracking experience of air travel by incorporating the feelings of flying into the mindfulness exercises. Others experiment with virtual and immersive reality. Tech startup \u003Ca href=\"https:\u002F\u002Fwww.futuretravelexperience.com\u002F2019\u002F01\u002Fairlines-leveraging-science-technology-passenger-wellbeing\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">SkyLights has added ‘fear-of-flying’ relaxation videos from air wellness company Mataki Group\u003C\u002Fa> to its Allosky VR IFE catalog.\u003C\u002Fp>\n\n\n\n\u003Cp>A light and healthy diet, noise control, and ambient lighting could also help passengers. \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.futuretravelexperience.com\u002F2019\u002F01\u002Fairlines-leveraging-science-technology-passenger-wellbeing\u002F\" target=\"_blank\">Panasonic Avionics serves as a great example of an innovator in this field\u003C\u002Fa>. The tech company launched a solution set, aptly named Wellness, which tailors the passenger’s lighting experience to different phases of the flight. Moreover, specialized sleep and wake algorithms ensure passengers have the best possible rest inflight and awake refreshed and at their body’s own pace.\u003C\u002Fp>\n\n\n\n\u003Cp>Figuring out how to react to medical emergencies during flights effectively with the help of digital health technologies and even how to predict them would be a step towards preparing for long flights towards other planets, too. If humanity wants to become an interplanetary species and populate the Moon and further on, Mars, first it has to figure out how to make long hours of flying not only bearable for people but comfortable and enjoyable – while staying healthy.
