Will we ever really know what’s on our plates? And if we knew, what could we do with the answer?\u003C\u002Fspan>\u003C\u002Fp>\n\n\n\n\u003C!--more-->\n\n\n\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">I often blog about trends that promise to have the biggest influence on medicine in the coming years. These posts receive a lot of feedback from people around the world. After I published predictions for how medical technology will advance in 2015, a mother left an emotional comment about her daughter of 8 years who was allergic to most foods. \u003C\u002Fspan>\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">She described how hard it was to find food her daughter can eat. She had to check labels diligently for allergic ingredients, and she had to cook very carefully. She was pregnant at the time she wrote the comment, and worried. Were there any labeling advances or technological devices that could make her life easier? In the US one out of three people has a food allergy or modifies the family diet because of a suspected food allergy in a family member. Approximately 5% of children and 4% of teens and adults have clinically proven allergic reactions to foods. \u003C\u002Fspan>\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Globally, about 35 % of adults are overweight, with half a billion being obese with a body mass index of over 30. According to \u003Ca href=\"http:\u002F\u002Fwww.idf.org\u002F\" target=\"_blank\" rel=\"noopener noreferrer\">The International Diabetes Federation\u003C\u002Fa>, around half a billion people will have diabetes by 2035; half of them will be undiagnosed. Food allergies, obesity and diabetes are only the most striking examples where knowing calorie intake and ingredients are pivotal in managing medical issues day and day out.\u003C\u002Fspan>\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Given these statistics, the \u003Ca href=\"http:\u002F\u002Fec.europa.eu\u002Fresearch\u002Fhorizonprize\u002Findex.cfm?prize=food-scanner\" target=\"_blank\" rel=\"noopener noreferrer\">European Commission launched\u003C\u002Fa> a €1 million prize challenge to develop an affordable, mobile, non–invasive solution that enables users to measure and analyze their food intake. The contest runs until the end of 2016. Hundreds of other similar initiatives are needed to help people deal with diabetes or obesity.\u003C\u002Fspan>\u003C\u002Fp>\n\n\n\n\u003Ch1 class=\"wp-block-heading\">\u003Cb>We really have no idea what we eat now\u003C\u002Fb>\u003C\u002Fh1>\n\n\n\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">We can only guess. Ingredients are listed on most of the products we buy, but every meal and every plate is different. One remedy would be having a list of exactly what ingredients and how many calories a meal contains, and what allergens and toxins might be in it. Not just the kind of meal we eat but the actual food on our plate and its specific amount. Several companies have been trying to address this.\u003C\u002Fspan>\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Canadian \u003C\u002Fspan>\u003Ci>\u003Cspan style=\"font-weight: 400;\">\u003Ca href=\"http:\u002F\u002Ftellspec.com\u002F\" target=\"_blank\" rel=\"noopener noreferrer\">TellSpec\u003C\u002Fa>\u003C\u002Fspan>\u003C\u002Fi>\u003Cspan style=\"font-weight: 400;\"> had a successful crowdfunding campaign in 2013 that raised more than $380,000. Now it aims to develop a hand–held food scanner that can inform users as about specific ingredients and macronutrients. Through spectroscopy, an analysis of how matter interacts with different wavelengths of light, it can quickly determine the chemical compounds in a given food. The company founder, Isabel Hoffmann, had a story similar to that of the mother who commented on my blog. Hoffman’s daughter got sick after the family moved to a new flat in Toronto. Months later the girl was found to have a variety of food allergies. \u003C\u002Fspan>\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">As a result, Hoffmann recruited a team to design a device that could tell what was in a particular food. That way she and others could know what to avoid. They sent out the first devices to beta testers in mid-2015. Several critics have raised questions about the device and whether the company can deliver what they promised. Backers in the crowdfunding campaign seem willing to wait and see. The first commercially available models are launching in 2016.\u003C\u002Fspan>\u003C\u002Fp>\n\n\n\n\u003Ch1 class=\"wp-block-heading\">\u003Cb>The promise and problems of food scanners\u003C\u002Fb>\u003C\u002Fh1>\n\n\n\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Another device, \u003C\u002Fspan>\u003Ci>\u003Cspan style=\"font-weight: 400;\">\u003Ca href=\"https:\u002F\u002Fwww.consumerphysics.com\u002Fmyscio\u002F\" target=\"_blank\" rel=\"noopener noreferrer\">SCiO\u003C\u002Fa>\u003C\u002Fspan>\u003C\u002Fi>\u003Cspan style=\"font-weight: 400;\">, from Israel, was founded by people with optical engineering backgrounds. They have raised over $2.7 million on Kickstarter in 2014. It uses a technology similar to \u003C\u002Fspan>\u003Ci>\u003Cspan style=\"font-weight: 400;\">TellSpec\u003C\u002Fspan>\u003C\u002Fi>\u003Cspan style=\"font-weight: 400;\">’s but is designed to identify the molecular content of foods, medicines, and even plants. It illuminates an object; optical sensors detect the reflected light; and the device analyzes it using an algorithm and a cloud–based database that is constantly updated. The company promises that in milliseconds the ingredients and molecular make–up of the foodstuff will appear on the user’s smartphone. The device was scheduled for shipment in 2015, but disappointed backers, but still hasn’t shipped. On top of the delays, experts in applied science criticized both SCiO and TellSpec for overstating what their inventions can do. \u003C\u002Fspan>\u003C\u002Fp>\n\n\n\n\u003Cdiv class=\"wp-block-image\">\u003Cfigure class=\"aligncenter\">\u003Cimg loading=\"lazy\" decoding=\"async\" width=\"512\" height=\"394\" src=\"https:\u002F\u002F1712507217.rsc.cdn77.org\u002Fwp-content\u002Fuploads\u002F2016\u002F05\u002Fscio-molecular-sensor-512x394.jpg\" alt=\"Scio food scanner scans cheese for ingredients\" class=\"wp-image-13733\" srcset=\"https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2016\u002F05\u002Fscio-molecular-sensor.jpg 512w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2016\u002F05\u002Fscio-molecular-sensor-325x250.jpg 325w\" sizes=\"auto, (max-width: 512px) 100vw, 512px\" \u002F>\u003C\u002Ffigure>\u003C\u002Fdiv>\n\n\n\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">There are two major issues. One is size, because the device must be hand–held to become popular. With current technology this means engineers have to sacrifice sensitivity and accuracy in order to achieve a convenient size. The other issue is the algorithm. \u003C\u002Fspan>\u003Ci>\u003Cspan style=\"font-weight: 400;\">SCiO\u003C\u002Fspan>\u003C\u002Fi>\u003Cspan style=\"font-weight: 400;\"> sends data to the cloud which then sends its calculation back to the device. But to simplify what the algorithm has to do, users need to tell the scanner specifics – like whether the sample is a solid food, a liquid, or vegetable. These inconveniences are the price of keeping the scanner small.\u003C\u002Fspan>\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">There aren’t any promising hand–held food scanners on the horizon besides these, but there is no reason to believe a solution will not arise in the coming years. The challenge is not when a workable device comes along but what we will do with the large amount of data it generates. \u003C\u002Fspan>\u003C\u002Fp>\n\n\n\n\u003Ch1 class=\"wp-block-heading\">\u003Cb>Big data and the Internet of things will improve nutrition\u003C\u002Fb>\u003C\u002Fh1>\n\n\n\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Let’s say a scanner tells me how many grams of sugar my fruit contains, or what the alcohol percentage of a drink is. So what? It won’t change my behavior and dietary habits unless I’m a dietitian and understand what the data means, and how it can be acted upon. Food scanners will need to progress similarly to wearable health trackers – move from raw data to automated analysis and smart suggestions to the user.\u003C\u002Fspan>\u003C\u002Fp>\n\n\n\n\u003Cdiv class=\"wp-block-image\">\u003Cfigure class=\"alignleft\">\u003Ca class=\"img\" href=\"http:\u002F\u002Fmyhealthupgraded.com\" target=\"_blank\" rel=\"noopener noreferrer\">\u003Cimg loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"196\" src=\"https:\u002F\u002F1712507217.rsc.cdn77.org\u002Fwp-content\u002Fuploads\u002F2016\u002F05\u002FMHU_promo_300x196.gif\" alt=\"Learn more about the most exciting technological changes shaping healthcare!\" class=\"wp-image-13147\"\u002F\u003C\u002Fa>\u003C\u002Fa>\u003C\u002Ffigure>\u003C\u002Fdiv>\n\n\n\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">A good food scanner should accurately determine ingredients, and compare the data to my lifestyle, dietary choices, and my genomic background. Given how different we all are genetically, two people might digest the same food at a different pace. One might be allergic to an ingredient while the other is not. So far, pure luck and experience have alerted us to these differences. It should not work like that. Eating should be a conscious process where we know what we eat, and know what we should eat for optimum health. A food scanner, supported by a smart application could fill this place.\u003C\u002Fspan>\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">But let’s not leave out an interesting side note here, namely, incorporating genetic information into food scanners. I already have the data of my complete DNA sequence at home in a digital file. Literally thousands of studies speak to the genetic aspects of nutrition, a field called nutrigenomics. I should be able to learn what foods and individual ingredients are bad for me. Genetic tests showed me that I’m sensitive to caffeine and process alcohol more thoroughly than most people (I’m Hungarian after all). \u003C\u002Fspan>\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Nutrigenomics tries to understand how nutrition affects our metabolic pathways, and what we can do to get the most out of nutrition in a personalized way. If I’ll have the opportunity to choose another type of meat or cheese as a smartphone app suggests based on my DNA, I will enjoy the meal more and take better care of my body in the long run. With access to such data, a scanner or app could tell us what products not to buy at the grocery store, what type of food makes us more productive, sleep better, or just feel healthy. Right now we’re depending on blind luck.\u003C\u002Fspan>\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cspan style=\"line-height: 1.5;\">Some people wonder if this wouldn’t be an overly technological world where devices, scanners, and apps tell us what to eat and do. I prefer to look at it from a different angle, from the benefits of finally knowing what we eat and what ingredients lead to positive and negative consequences. I see customization to my specific genetic background as another benefit, too.\u003C\u002Fspan>\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cspan style=\"font-weight: 400;\">Diabetes patients would know how many carbohydrates their food contains. But knowledge doesn’t change behavior alone, otherwise nobody would smoke by now. Knowledge supported by gaming or technologies revealing our lifestyle choices to our family members or caregivers might do. Patients with rare genetic metabolic disorders such as phenylketonuria would know what to avoid at all cost. People with allergies could avoid dangerous meals. Having a good diet would not rely on the experience we bring with us from childhood and what we have learned since then. Instead, it could be based on informed decisions. While the allergic reactions to the underground seeds kill every year about 150 people, \u003Ca href=\"https:\u002F\u002Fwww.livescience.com\u002F34559-tornado-deaths-infographic.html\">tornadoes stay in the range of 100-120 – except for some peak years\u003C\u002Fa>. And \u003Cstrong>in the case of the innocent-looking peanuts, a minimal amount can cause a life-threatening \u003C\u002Fstrong>\u003Cstrong>\u003Ca href=\"https:\u002F\u002Facaai.org\u002Fallergies\u002Ftypes\u002Ffood-allergies\u002Ftypes-food-allergy\u002Fpeanut-allergy\">anaphylactic shock\u003C\u002Fa>\u003C\u002Fstrong>\u003Cstrong> – and if there’s no EpiPen around, the attack can result in a deadly outcome\u003C\u002Fstrong>.\u003C\u002Fp>\n\n\n\n\u003Cp>Moreover, peanut allergy is more common than ever before: \u003Ca href=\"https:\u002F\u002Fyoungwomenshealth.org\u002F2013\u002F12\u002F06\u002Fpeanut-allergy\u002F\">its prevalence has tripled in the past ten years\u003C\u002Fa>. In the Western world, \u003Ca href=\"https:\u002F\u002Fwww.news-medical.net\u002Fhealth\u002FPeanut-Allergy-Prevalence.aspx\">the occurrence in some populations can be as high as 1 in 200 individuals\u003C\u002Fa>. In the United Kingdom, peanut allergy is present in between 0.4 and 0.6 % of the whole community. Approximately 1-2% of children in the United States have a diagnosed peanut allergy. However, in China, fish, shrimp, seaweed, and crab trigger food allergies, which make up only 3.4 to 5.0% of all allergies, while peanut allergy is unheard of. In Australia, the chance of getting a peanut allergy is even more unlikely: only 0.25% of the population is concerned.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Beyond the severity and prevalence of the food allergy, peanuts are especially dangerous and difficult to fight – and they generally show their true colors very early\u003C\u002Fstrong>. Peanut allergy \u003Ca href=\"https:\u002F\u002Fyoungwomenshealth.org\u002F2013\u002F12\u002F06\u002Fpeanut-allergy\u002F\">usually starts by age 2 with 75% of allergic reactions happening the first time a child eats peanuts\u003C\u002Fa>. Since then, the device became a popular tool among people with gluten sensitivities.\u003C\u002Fstrong> As their world map shows, meals were tested in many corners of the Earth: from the Siam Kitchen Express in Hawaii through the Dolce Locura in Chile until the Sushilka in Russia.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>I also had the chance to test the gluten sensor with both gluten-containing and gluten-free meals – and \u003C\u002Fstrong>\u003Cstrong>\u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Ftesting-food-gluten-home-nima-sensor-review\">the entire team had a technological sublime during the process\u003C\u002Fa>\u003C\u002Fstrong>\u003Cstrong>. It’s that simple. The developers even made it easier to screw back on the cap of the capsule. After some softer grunting sounds, the pyramid will show the result. In the peanut butter case I was obviously expecting a „peanut found” signal, and after two minutes, voilá, it appeared.\u003C\u002Fp>\n\n\n\n\u003Cp>I also \u003Cstrong>tested the sensor with a biscuit on whose packaging it was said that &#8220;it contains peanut in traces&#8221;\u003C\u002Fstrong> since I believe it might be the most important feature of the Nima to detect the amount of peanut which might already cause an allergic reaction. They explain that antibodies are large, y-shaped proteins recognizing and binding to specific target proteins – thus showing their existence clearly. That’s why they are frequently used to detect proteins in lab tests.\u003C\u002Fp>\n\n\n\n\u003Cp>As a consequence, the Nima team designed the sensor to detect traces of peanut in meals using specific proteins. For brainiacs: the Nima 20B10 and 16B1 antibodies bind to a peanut protein called Arah3. Although not the most antigenic, it is most abundant in all types of peanut and is more stable under processing conditions.\u003C\u002Fp>\n\n\n\n\u003Cfigure class=\"wp-block-image\">\u003Cimg loading=\"lazy\" decoding=\"async\" width=\"842\" height=\"870\" src=\"https:\u002F\u002F1712507217.rsc.cdn77.org\u002Fwp-content\u002Fuploads\u002F2018\u002F10\u002FIMG_6768.jpg\" alt=\"peanut sensor\" class=\"wp-image-22204\" srcset=\"https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2018\u002F10\u002FIMG_6768.jpg 842w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2018\u002F10\u002FIMG_6768-768x793.jpg 768w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2018\u002F10\u002FIMG_6768-496x512.jpg 496w\" sizes=\"auto, (max-width: 842px) 100vw, 842px\" \u002F>\u003C\u002Ffigure>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>What type of foods could it test?\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>The Nima team designed the anti-peanut pyramid to test for 10 ppm, and above (equal to 0.001g peanut in a standard serving size of 100g), thus it can show tiny amounts and traces of peanut in food as well. \u003Cstrong>No matter whether it’s about soups, sauces, ice cream, peanut butter, baked goods or packaged foods.\u003C\u002Fstrong> However, there are certain \u003Cstrong>limitations\u003C\u002Fstrong>: the sensor cannot test sesame, eggplant, paprika, tomato paste or doesn’t process alcohol and solid chocolate well.\u003C\u002Fp>\n\n\n\n\u003Cp>The Nima team was also thoughtful enough to include a pocket guide into its box &#8211; thus users do not have to remember all the food restrictions and limitations.\u003C\u002Fp>\n\n\n\n\u003Cp>Regarding \u003Cstrong>the question of whether or not the peanut sensor could recognize other nuts\u002Ftree nuts – the support services responded that peanuts aren’t nuts in a technical sense.\u003C\u002Fstrong>&nbsp;Thus the team would have to create another type of sensor for testing tree nuts – and they don’t have such plans at the moment. While tree nuts, such as almonds, cashews, walnuts grow on trees, peanuts are underground vegetables, really. They belong to the family of lentils, beans or peas. (Could you send it back, so it doesn’t pollute the environment?) A 12-pack costs $72– or $59 for premium members, which means that one test costs around $5-6. That is not cheap at all. So you have to decide when and what to test.\u003C\u002Fp>\n\n\n\n\u003Cp>On the other hand, if you are living in the US and your health insurance benefits include FSA\u002FHSA reimbursement, the peanut sensor and test capsules are reimbursable with a peanut allergy diagnosis. That’s great news, and more insurance companies should follow that great example in the future!\u003C\u002Fp>\n\n\n\n\u003Cp>All-in-all, the Nima peanut sensor can provide peace of mind to anyone suffering from peanut allergy due to its easy testing procedure, portability – and accuracy. The sensors have been the subject of independent lab testing, one of which showed the sensitivity of the peanut sensor was 100 percent and the accuracy was 99.2 percent, while \u003Ca href=\"https:\u002F\u002Fnimasensor.com\u002F2018\u002F08\u002F15\u002Fnima-peanut-assessment\u002F\">Nima&#8217;s own results\u003C\u002Fa> showed 99 percent and 97.6 percent accuracy, respectively.
