A.I. outsmarting human doctors? Simple big data analytical\nsoftware presented with clever marketing tactics? It’s difficult to assess the\nactual state of play when it comes to artificial intelligence in healthcare. Moreover,\nthere’s no database that contains all the smart algorithms worth applying to\nmedical processes. That’s the reason why we decided to collect every artificial\nintelligence-based algorithm that already received FDA approval – meaning that\nthey are proven, reliable, and accurate solutions enabled by an official\nregulator for medical use. Let’s see the infographic in details!\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>The\nfactors of algorithmic healing\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>When\nwe started to assess the universe of smart algorithms in healthcare, we took\ninto account temporal and spatial factors, accuracy and credibility, as well as\nmedical specialties where A.I. algorithms have a chance to make the care\nprocess better. \u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Concerning the timeline, we noticed an uptake in the appearance of new solutions in the last years, and you can see that also in the infographic\u003C\u002Fstrong>. In 2014, only AliveCor’s algorithm for the detection of atrial fibrillation was approved. Two years later, the FDA found further four solutions ready for clinical use, while in 2017, six new algorithms were approved by the US regulator. This exponential growth just accelerated last year, when the FDA endorsed 23 algorithms in medicine. \u003Cstrong>As the first approvals in 2019 also show, we do not expect the trend to slow down. On the contrary, we will most likely see dozens of new medical A.I. solutions on the market.\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>Looking at the spatial factors, while \u003Ca href=\"https:\u002F\u002Fwww.analyticsinsight.net\u002Ftop-6-artificial-intelligence-hubs-in-2018-an-analysis-for-their-localization\u002F\">the most important hubs for A.I. development\u003C\u002Fa> are the Silicon Valley, the Boston-New York area, Montréal, London, Bangalore, and Beijing, and the same can be applied in medicine and healthcare, \u003Cstrong>the most decisive factor for compiling FDA approved algorithms was that it is the only yardstick for credible and accurate medical software\u003C\u002Fstrong>. Fig. 1b, Esteva, Kuprel et al., 2017\u003C\u002Ffigcaption>\u003C\u002Ffigure>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>What does an FDA approval mean?\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>\u003Cstrong>Regarding the meaning of the FDA\napproval itself, the listed algorithms embrace the entire scale of approvals \u003Ca href=\"https:\u002F\u002Fkenblockconsulting.com\u002Ffda-submissions\">starting from\n510(K) submission through de novo to premarket approval (PMA)\u003C\u002Fa>\u003C\u002Fstrong>\u003Cstrong>.\u003C\u002Fstrong> The first one\nrefers to a premarket submission to demonstrate that a device aiming for market\nlaunch but not requiring premarket approval is as safe and effective as other\nsimilar instruments with PMA. The latter actually means the FDA process of\nscientific and regulatory review to evaluate the safety and effectiveness of\nmedical devices supporting and\u002For sustaining human life and the most stringent\nof the device marketing applications. \u003C\u002Fp>\n\n\n\n\u003Cp>The de novo pathway for device marketing rights was added to address novel devices of low to moderate risk that do not have a valid predicate device – for example in the case of software solutions such as smart algorithms. Upon successful review of a de novo submission, FDA creates a classification for the instrument, a regulation if necessary, and identifies any special controls required for future premarket submissions of substantially equivalent devices. However, geriatrics, orthopedics or pathology seem to be less prone to A.I. Certain medical specialties do not even appear in the list yet, such as pulmonology, dermatology, surgery, OB\u002FGyn or forensic medicine.\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>Although we should not draw definitive conclusions from only this infographic, as it constitutes a snapshot of the current state of play not revealing anything about the trends. First and foremost, computer vision is one of the fastest-growing fields in A.I. development, and medical imaging has both the data and the visuality that smart algorithms need to thrive. As a consequence,  \u003Ca href=\"https:\u002F\u002Fspectrum.ieee.org\u002Fthe-human-os\u002Frobotics\u002Fartificial-intelligence\u002Fcomputers-match-human-accuracy-in-screening-for-breast-cancer-risk\">researchers found that commercial software for automatically classifying breast density\u003C\u002Fa>, and thus detecting breast cancer, can perform on par with human radiologists. It featured the two best players in the history of the show, Ken Jennings, who had the longest unbeaten run of 74 winning appearances, and Brad Rutter, earner of the biggest prize of $3.25 million. Their opponent was a huge computer with over 750 servers and a cooling system stored at a location so as not to disturb the players. The room–sized machine was made by IBM and named after the company&#8217;s founder, Thomas J. Watson. It did not smile or show emotion, but it kept on giving good answers. At the end, \u003Ca href=\"http:\u002F\u002Fwww.ibm.com\u002Fsmarterplanet\u002Fus\u002Fen\u002Fibmwatson\u002F\" target=\"_blank\" rel=\"nofollow noopener noreferrer\">Watson\u003C\u002Fa> won the game with $77,147 leaving Rutter and Jennings with $21,600 and $24,000 respectively. It left the audience in shock and awe at the same time.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Cognitive computers have been developing rapidly\u003C\u002Fstrong> over the last few years following \u003Ca href=\"http:\u002F\u002Fwww.wired.com\u002F2014\u002F10\u002Ffuture-of-artificial-intelligence\" target=\"_blank\" rel=\"nofollow noopener noreferrer\">three technological breakthroughs\u003C\u002Fa>. One is cheap parallel computation due to a new kind of chip called a graphics processing unit (GPU). The second one is accessible big data due to massive databases, web cookies, wearable devices and decades of search results. In 1997, IBM&#8217;s supercomputer Deep Blue could beat Garry Kasparov, the reigning chess grand master that time. He said he could have performed better if he had access to the same databases as Deep Blue. So later, freestyle matches were organized in which supercomputers could play against human chess players assisted by AI (they were called human\u002FAI centaurs). Guess what! In 2014 in a&nbsp;Freestyle Battle, the AI chess players won 42 games, but centaurs won 53 games. The best potential pair is a human with technology.&nbsp;\u003Cstrong>This is the only balance that can lead to a positive future\u003C\u002Fstrong>&nbsp;with more and more disruptive innovations including ever-improving cognitive computing but an also ever-improving human intelligence and wisdom. It is not any different in healthcare. It took plenty of time until the stethoscope, the symbol of healthcare&nbsp;was accepted by the medical community back in the 19th century. The instrument was \u003Ca href=\"https:\u002F\u002Fen.wikipedia.org\u002Fwiki\u002FStethoscope#History\">invented by&nbsp;French physician René-Théophile-Hyacinthe Laënnec\u003C\u002Fa>, who published its description in 1819, but it took several decades until doctors actually started using it.\u003C\u002Fp>\n\n\n\n\u003Cp>I don&#8217;t expect any difference with A.I.&nbsp;After many of my talks, physician colleagues ask me whether artificial intelligence might replace them in their jobs and whether algorithms can eventually become better at making diagnoses. Both will happen but not the way they imagine it. \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fartificial-intelligence-will-redesign-healthcare\u002F\">Huge waves are coming to healthcare&nbsp;to transform the job of physicians\u003C\u002Fa> into something distinctly different than before. Although some of their tasks will be taken over by A.I., they will have more time for others, for example, deal with patients with real care and patience. Doctors will not have to&nbsp;struggle with being up-to-date in medical research, with administrative tasks, with consultation or making notes. As the amount of information they accumulate grows exponentially, the assistance of computing solutions in medical decisions is imminent. A.I. will open new dimensions for doctors on the personal level as well as for hospitals and other medical institutions on the structural level.\u003C\u002Fp>\n\n\n\n\u003Cp>On the institutional level, the most obvious use of A.I. will be data management.&nbsp;Collecting it, storing it, normalizing it, tracing its lineage – it is the first step in revolutionizing the existing healthcare systems. Recently, the AI research branch of the search giant, Google, launched its&nbsp;Google Deepmind Health&nbsp;project, which is used to mine the data of medical records in order to provide better and faster health services.\u003C\u002Fp>\n\n\n\n\u003Cp>It could also analyze entire healthcare systems. For example, 97 percent of healthcare invoices in the Netherlands are digital containing data regarding the treatment, the doctor, and the hospital. These invoices could be easily retrieved. A local company,&nbsp;\u003Ca href=\"https:\u002F\u002Fwww.zorgprismapubliek.nl\u002F\">Zorgprisma Publiek\u003C\u002Fa>&nbsp;analyzes the invoices and uses IBM Watson in the cloud to mine the data. For example, IBM&#8217;s \u003Ca href=\"http:\u002F\u002Fresearcher.watson.ibm.com\u002Fresearcher\u002Fview_group.php?id=4384\">Medical Sieve\u003C\u002Fa>&nbsp;will become the next generation “cognitive assistant” with analytical, reasoning capabilities and a wide range of clinical knowledge. The algorithm will be qualified to assist in clinical decision making in radiology and cardiology. A.I. assistants could also look up relevant medical information and keep doctors up-to-date in clinical research. IBM has the capacity to scan through millions of pages in seconds. Van Oijen, Ph.D. is an Associate Professor at the Academic Medical Center at the University of Amsterdam about Watson. As a clinical epidemiologist, he worked with IBM Watson for Oncology in preparation for a research proposal studying the role of Digital Decision Support tools. With several clinical colleagues of the department of Medical Oncology, they studied Watson for Oncology’s approach in approximately 400 surrogate patients with breast, lung or colorectal cancer. He believes that Watson for Oncology could result in a reduction of costs and efforts.\u003C\u002Fp>\n\n\n\n\u003Cp>They both agreed that AI cannot be a substitute for communication.&nbsp;\u003Cem>Right now, the technology is in its infancy, but improving all the time\u003C\u002Fem>, said Hennessey.
