That’s the main finding of the latest \u003Ca href=\"https:\u002F\u002Fbmjopen.bmj.com\u002Fcontent\u002Fbmjopen\u002F9\u002F3\u002Fe025267.full.pdf\">study\u003C\u002Fa> of The Medical Futurist Institute, entitled \u003Cem>Opinion leader empowered patients about the era of digital health: a qualitative study\u003C\u002Fem>, published in the \u003Cem>British Medical Journal\u003C\u002Fem>.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>The case for e-patients\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>In the last decade, rapid technological advancement has resulted in a shift towards digital health in medicine. This shift is theorized as a cultural transformation of how disruptive technologies providing digital and objective data accessible to both caregivers and patients lead to an equal level physician-patient relationship with shared decision-making and the democratization of care. \u003Cstrong>Has this change already started? Where does it stand at the moment if it already has? What does a doctor-patient partnership look like? What do physicians and patients need to do differently in the digital age? What are their greatest challenges?\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>The Medical Futurist Institute (TMFI) had plenty of questions regarding the assumed shift in the behavior of patients and doctors due to easily available health technologies\u003C\u002Fstrong>. For questions concerning the patient’s side, TMFI conducted a qualitative study by asking 11 e-patients who are considered opinion leaders worldwide about their experiences and expectations. Although there’s no strict definition yet,\nthe ‘e’ can stand for ‘electronic’, ‘equipped’, ‘enabled’, ‘empowered’,\n‘engaged’ or ‘expert’. \u003C\u002Fp>\n\n\n\n\u003Cp>Empowered patients often have access to information, medical knowledge, and their own health data. It is important for them to be in contact with other patients and become highly active, engaged, equipped and educated in this process. Sharing medical stories, pieces of advice, providing support to other patients, and looking for new opportunities in treatment options are all characteristics of an empowered patient. They can find the questions they need to ask and have the confidence to ask those questions. They think that each patient can strive to be the expert of their own condition because of social media and technology in general.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Anyone can become an e-patient, who is proactive, has the desire and the courage to ask questions, and uses technologies. The latter doesn’t even have to mean particularly unique technologies. This partnership already signifies a cultural change or\nparadigm shift: the previously rather asymmetrical conversation with a semi-god-like\ndoctor becomes reciprocal. Partnership\nalso means that physicians and patients listen to each other, express their\nneeds and desires clearly, and have a common goal.\u003C\u002Fstrong> According to one of the e-patients, this also means\nthat ‘patients have to empathize with doctors as well, like ‘I understand you\nhave seen fifty other patients before me today.’\u003C\u002Fp>\n\n\n\n\u003Cp>In this process, it is essential that patients are curious, while doctors need bravery and a willingness to make changes. Based on the interviews, ‘medical professionals need to change their perception of medical support because now there’s a sort of feeling that ‘I don’t need a machine, I know myself,’ although there is so much information that it’s physically and mentally impossible for somebody to know everything. Physicians will tell patients it is not just socially appropriate to ask questions but it is also an important aspect of proactivity. Patients will prepare more for the visits, and discussions and technologies will be part of everyday care. These will simplify but not replace contact between patients and caregivers.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>‘Ten\nyears from now every patient will be asked ‘Do you want to come in? Do you want\na video consultation? Do you want your\ninformation on your mobile phone? What is\nthe right thing for you?’ That is the hope of one of the e-patients we asked. \u003C\u002Fstrong>Another commented this: ‘And instead of doctors saying\ndo not Google your symptoms because you will feel sicker, they will actually\nsupport their patients and teach them how to Google and how to assess\ncredibility and correctness on what they find. So that’s what I hope, and I\nthink that’s what will happen.’ \u003C\u002Fp>\n\n\n\n\u003Cp>It’s an important point of the entire study, which we at The Medical Futurist also want to happen. Listing of personal health skills could enable patients to show their doctors they are active participants in their care.&nbsp;That means being a patient is not a passive adjective, but a very active verb.\u003C\u002Fp>\n\n\n\n\u003C!--more-->\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>How to present complex patient info easily to doctors?\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>The Medical Futurist reached out to the idea initiator, \u003Ca href=\"https:\u002F\u002Ftwitter.com\u002Fbetabetic\">@betabetic\u003C\u002Fa> or Lindsay Fallow, a software developer turned science journalist; who is coping with multiple endocrine problems including adrenal insufficiency and a type-1 variant diabetes, plus severe asthma and gastric issues. That means she needs to have some of her hydration and nutrition through a central IV. It becomes clear just by reading the listing of all these medical conditions how complex skill-set it must take her to manage them on a daily basis.\u003C\u002Fp>\n\n\n\n\u003Cp>She said she was walking her dog and contemplating about an upcoming appointment with a new specialist when the idea popped into her mind. She was thinking about how to present the complex and massive amount of information that she knows about her illnesses. “\u003Cem>So I was thinking, this doctor needs to know that this is something I do normally &#8211; because physicians have a habit of thinking that their patients are a bit weird when they turn up with ‘professional’ style information.\u003C\u002Fem> \u003Cem>And how crazy is it to imagine that people who have health problems wouldn’t bring all of their experience and expertise to their health &#8211; whether professional or personal?\u003C\u002Fem> Who you are, defines how you deal with anything life throws at you… yet our medical records do not store that&#8221;, she explained.\u003C\u002Fp>\n\n\n\n\u003Cp>So, she created a one-pager where she included bits and pieces about her personal and professional life. Fallow listed what kind of technologies and social media she uses, trackers such as Fitbit, Garmin or Strava. Moreover, she entered all the resources and assets that help her manage her condition starting from her supportive partner until the medical alert dog. She described what symptoms she struggles with and what her current goals are with regards to managing her illness. It is clear, concise and might be very helpful to doctors. She said that she has only tried once so far, but the initial response was very positive. Well-known health tech and patient advocate, Susannah Fox, told The Medical Futurist that \u003Cem>the juxtaposition of professional and personal lives is what grabs her when she sees the #patientCV conversation.\u003C\u002Fem>&nbsp;&#8220;We are encouraged to publicly list our work-related skill sets, such as in our CVs or on LinkedIn, but rarely our personal health skills. By owning that hard-won experience as patients or as caregivers, we own our power&#8221;, she added.\u003C\u002Fp>\n\n\n\n\u003Cp>However, Fallow believes everyone should be cautious when introducing things to physicians that are changing how ‘patienting’ – the way she calls the activity of being a patient – is done by the doctor because it could both help and do harm. The patient-doctor communication is a delicate rapport and needs to be treated with respect. On the other hand, she thinks that the Patient CV should become a core part of any healthcare plan; as 99.9 percent of the work in managing illness is done by the patient and not the medics. We cannot agree more. It’s a great tool for empowering patients and showing healthcare professionals that being a patient is not a passive condition but an active participation.
