But the recent decade has seen us propelled into a new universe, where healing doesn&#8217;t just come from pharmacies but also from app stores. This shift has been brought about by the rise of Digital Therapeutics (DTx), healing that can be administered right through our smartphones.\u003C\u002Fp>\n\n\n\n\u003Cp>The title, Drugs vs. DTx, is a classic case of the &#8220;it&#8217;s complicated&#8221; relationship status. On one hand, they stand toe-to-toe, each offering unique modes of healing, on the other, they do their best when working together.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Defining DTx\u003C\u002Fh2>\n\n\n\n\u003Cp>Digital therapeutics deliver evidence-based therapeutic interventions driven by high-quality software programs to prevent, manage, or treat a medical disorder or disease. \u003C\u002Fp>\n\n\n\n\u003Cblockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n\u003Cp>Think of it like traditional drugs, but instead of swallowing a pill, you&#8217;re tapping on an app.\u003C\u002Fp>\n\u003C\u002Fblockquote>\n\n\n\n\u003Cp>But this isn&#8217;t just a novel concept. They hold the promise of improved health outcomes, greater accessibility, and cost-effectiveness in patient care when used based on proper evidence.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">The Dance of Difference\u003C\u002Fh2>\n\n\n\n\u003Cp>Recently, The Medical Futurist published an insightful infographic, highlighting the key differences between traditional drug-based therapies and digital therapeutics. The monitoring is a whole different ball game, as DTx allows for real-time, automatic monitoring. As we discussed, it is also very different from the user experience point of view: I head to the app store instead of the pharmacy, and I use an app instead of swallowing pills. \u003C\u002Fp>\n\n\n\n\u003Cp>Digital therapeutics are more than just a novel concept. They hold the promise of improved health outcomes, greater accessibility, and cost-effectiveness in patient care when used based on proper evidence.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Privacy + Access = Wider Reach\u003C\u002Fh2>\n\n\n\n\u003Cp>In a \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-new-buzz-these-are-the-top-examples-of-digital-therapeutics\u002F\" target=\"_blank\">previous article\u003C\u002Fa>, we explored DTx and its benefits. \u003C\u002Fp>\n\n\n\n\u003Cp>DTx solutions are typically delivered through smartphone apps. In other words, via&nbsp;\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.psychiatrictimes.com\u002Fview\u002Ffrom-theory-to-real-world-practice-clinical-integration-of-digital-therapeutics\" target=\"_blank\">a channel pretty much everyone has access to\u003C\u002Fa>. Accessibility is one of the key factors for any new solution to become widely used.  \u003C\u002Fp>\n\n\n\n\u003Cp>But there is another upside: it allows a certain kind of privacy, bringing treatments to the patients’ homes, and removing stigmas typically connected to a number of therapies, either in substance abuse care or in mental health.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Great potential &amp; unique risks\u003C\u002Fh2>\n\n\n\n\u003Cp>A \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.mckinsey.com\u002Findustries\u002Flife-sciences\u002Four-insights\u002Fthe-health-benefits-and-business-potential-of-digital-therapeutics\" target=\"_blank\">report by McKinsey\u003C\u002Fa> lists great examples: &#8220;Research has shown that digital disease management can drive a 45 percent reduction in the three-month rate of major adverse cardiovascular events and a 50 percent reduction in the 30-day readmission rates for patients after acute myocardial infarction.&#8221; These impressive figures show what traditional and digital therapeutics together can do. \u003C\u002Fp>\n\n\n\n\u003Cp>Another \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.nature.com\u002Farticles\u002Fs41746-023-00777-z\" target=\"_blank\">interesting study\u003C\u002Fa> highlights an often-overlooked factor as well: sociocultural influences that can impact the efficacy of DTx. External conditions, such as demographic factors, sociocultural and cognitive abilities, strongly influence what medicine can do and this is equally valid for traditional and digital therapeutics. \u003C\u002Fp>\n\n\n\n\u003Cp>While DTx apps won&#8217;t have side effects like traditional drugs, they do have their own set of challenges. Digital barriers, privacy and data security issues are among these.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Eight key elements of DTx\u003C\u002Fh2>\n\n\n\n\u003Cp>According to the McKinsey report, impactful digital therapeutics solutions typically contain most, if not all, of the following eight elements:\u003C\u002Fp>\n\n\n\n\u003Col class=\"wp-block-list\">\n\u003Cli>Regular monitoring, measurement, and feedback through connected medical devices\u003C\u002Fli>\n\n\n\n\u003Cli>Keeping payers (patients) and providers in the loop\u003C\u002Fli>\n\n\n\n\u003Cli>Personalised coaching and support\u003C\u002Fli>\n\n\n\n\u003Cli>Gamified behavioral modification\u003C\u002Fli>\n\n\n\n\u003Cli>Building a thriving community\u003C\u002Fli>\n\n\n\n\u003Cli>Unified digital health ecosystems\u003C\u002Fli>\n\n\n\n\u003Cli>Patient education\u003C\u002Fli>\n\n\n\n\u003Cli>Advanced analytics to predict and prevent health events\u003C\u002Fli>\n\u003C\u002Fol>\n\n\n\n\u003Cp>However, if we look at it from a little distance, these all point towards giving patients better tools for health management. It&#8217;s a fascinating question. We all understand drugs. That was our trusted tool, the &#8220;go-to&#8221; remedy for all our health issues. DTx is a brand new &#8220;weapon&#8221; in our arsenal, with fresh characteristics, waiting to fit into the established framework.\u003C\u002Fp>\n\n\n\n\u003Cp>In conclusion, it’s not about Drugs vs DTx. If you have GPT-4, click on your name on the bottom left, go to settings, click on beta features and allow them with the slider.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>How to add the plugins?&nbsp;\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>Adding plugins also works similarly to adding new apps to your phone: there is a &#8220;plugin store&#8221;, and you can pick whatever you fancy. When you start a new chat, hover over the GPT-4 section and you will see 3 options:&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>1. Default chat, 2. Browse with Bing and 3. Plugins. Feel free to look for whatever you fancy!\u003C\u002Fp>\n\n\n\n\u003Cp>It is important to note that they are not necessarily very useful now. For a number of reasons, most of these plugins fall short as of yet. But they let us peek into a future when we all will have the most capable personal AI assistant at hand.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>What are the problems for now?&nbsp;\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>They either don&#8217;t work reliably enough; pose such privacy threats that I&#8217;m not willing to take &#8211; or, often, the combination of these two. Be mindful of the privacy risks, and don&#8217;t just click through everything without paying attention!&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>I’ve set up a test Gmail address and Google profile for this article, as letting OpenAI &#8211; with due respect and my heartfelt gratitude for their amazing work &#8211; and these app developers access my main profile would have felt way too much.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">So, what can you do with plugins?\u003C\u002Fh2>\n\n\n\n\u003Cp>Here are a few interesting ones:\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Zapier &#8211; Interact with Gmail, Sheets, Docs, HubSpot, Salesforce\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>It promises to “Interact with over 5,000+ apps like Google Sheets, Gmail, HubSpot, Salesforce, and thousands more”. In theory, you only need to ask it to draft and send an email using your Gmail account directly from ChatGPT and voilá, no need to switch between browser windows. Again, in theory, you can connect it to your other Google gadgets, like your documents or google sheets library.\u003C\u002Fp>\n\n\n\n\u003Cp>In practice, I straightforwardly failed when setting up automations (Zaps &#8211; as they call it) over the most simple ones. In this regard,\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fchatgpt-alternatives-for-healthcare-and-research\" target=\"_blank\"> I have the best experiences\u003C\u002Fa> with \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.perplexity.ai\u002F\" target=\"_blank\">perplexity\u003C\u002Fa>, which is typically very fast and it delivers correct answers quite often. Both the Bing option and perplexity.ai let you see the source(s) of their answers, which is great.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>ScholarAI\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>“Unleash scientific research: search 40M+ peer-reviewed papers, explore scientific PDFs, and save to reference.”\u003C\u002Fp>\n\n\n\n\u003Cp>I have mixed feelings about this plugin, notably, I rather doubt if the studies it delivers are the most relevant or the most reliable ones. Also not sure if it has access to all (or at least most) scientific journals.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>For example, when I was searching for papers with “ChatGPT” and “Healthcare” as keywords, it listed a good amount of papers, and they seem to be overlapping with the results of my other search methods. To be honest, I didn’t really experience a material difference in the quality of answers when in use and when not in use. \u003C\u002Fp>\n\n\n\n\u003Cp>If I just activate it (and don’t use the “perfect” prompt), ChatGPT still decides to use the plugin every now and then to make sense of my prompt &#8211; which is interesting to see, I still try to figure out what is confusing about those specific prompts &#8211; they are typically rather innocent looking.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Video Summary\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>“Summarize YouTube video highlights. Generate summaries from YouTube video URLs.”\u003C\u002Fp>\n\n\n\n\u003Cp>Very similar to the next one, works only on videos that do have transcripts, which might be trivial with the English language ones, but quite rare with such exotic languages as ours. The summaries are more detailed than what Shownotes offers.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Shownotes\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>“Turns long podcasts into quick summaries, finds specific info, highlights key points, and suggests new episodes”\u003C\u002Fp>\n\n\n\n\u003Cp>It was among the most popular plugins, but it also works only on podcasts\u002Fvideos that have transcripts, which is not such a huge thing, as I could already use ChatGPT in many ways to summarise that transcript for me. On top of that, I hardly see the point of summarising these podcasts in 5-10-whatever bullet points, that is just the opposite of what podcasts are about.\u003C\u002Fp>\n\n\n\n\u003Cp>I also asked it to recommend me new podcasts based on my interests, but the recommendations were not on point.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>I found one major difference between the latter two apps. I \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.youtube.com\u002Fwatch?v=w2sHCiPxPnM\" target=\"_blank\">provided a link\u003C\u002Fa> to a recording of a Terraforming Mars game and asked both apps who won. While Video Summary delivered the correct answer (the streamer won) and correctly informed me which game it was a recording of, and even reported some of the major turning points, Shownotes delivered this verdict:\u003C\u002Fp>\n\n\n\n\u003Cp>“The video is a recording of a game between the Los Angeles Lakers and the Golden State Warriors.
