Now, technological solutions line up against microorganisms, bacteria, and fungi. Here are a few examples.\u003C\u002Fp>\n\n\n\n\u003C!--more-->\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>1 in 9 in-patients will die due to infection\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>According to the \u003Ca href=\"https:\u002F\u002Fwww.cdc.gov\u002Ffeatures\u002Fhandhygiene\u002Findex.html\">US Center for Disease Control\u003C\u002Fa>, studies show that \u003Cstrong>on average, healthcare providers clean their hands less than half of the times they should. This significantly contributes to the spread of \u003C\u002Fstrong>\u003Cstrong>\u003Ca href=\"https:\u002F\u002Fwww.cdc.gov\u002Fhai\u002F\">healthcare-associated infections\u003C\u002Fa>\u003C\u002Fstrong>\u003Cstrong> (HAIs or HCAIs)\u003C\u002Fstrong>, such as MRSA (methicillin-resistant \u003Cem>Staphylococcus aureus\u003C\u002Fem>) and \u003Cem>C. diff (Clostridium difficile\u003C\u002Fem>), affecting 1 in 25 hospital patients on any given day. Statistics revealed that on average, 1 in 9 will die due to the infection, and the number of HAI caused \u003Ca href=\"https:\u002F\u002Fwww.ncbi.nlm.nih.gov\u002Fpmc\u002Farticles\u002FPMC1820440\u002F\">the death of approximately 99,000 people in 2002\u003C\u002Fa> only in the US. That’s almost the population of Seychelles.\u003C\u002Fp>\n\n\n\n\u003Cp>Still, that was only the extent of the problem in one country. \u003Cstrong>The World Health Organization estimates that there are over 1.4 million cases of HAI at any given time. Of every 100 hospitalized patients, \u003Ca href=\"http:\u002F\u002Fwww.who.int\u002Fgpsc\u002Fcountry_work\u002Fgpsc_ccisc_fact_sheet_en.pdf\">7 in developed and 10 in developing countries will acquire at least one healthcare-associated infection\u003C\u002Fa>\u003C\u002Fstrong>. However, this is likely to be a gross underestimation, due to the lack of proper surveillance.\u003C\u002Fp>\n\n\n\n\u003Cp>These are insanely high numbers, especially considering that \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fcould-have-social-media-saved-semmelweis\">Dr. Ignác Semmelweis, “the savior of mothers”, already evangelized in the 19\u003Csup>th\u003C\u002Fsup> century\u003C\u002Fa> that hand sanitizing could mitigate the effect of infections – although, at that time, he meant it only with regards to new mothers and “childbed fever”. Even healthcare providers are at risk of getting an infection while they are treating patients.\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>Several factors contribute to the appearance of healthcare-related infections. A prolonged hospital stay, as well as the severity of the underlying illness, increases the risk of catching the germs, and so does patients’ lower immune status due to their severe conditions. In addition, the \u003Ca href=\"http:\u002F\u002Fwww.who.int\u002Fgpsc\u002Fcountry_work\u002Fgpsc_ccisc_fact_sheet_en.pdf\">WHO mentions\u003C\u002Fa> poor environmental hygienic conditions and waste disposal, inadequate infrastructure, lack of equipment, understaffing, overcrowding, lack of procedure, the insufficient knowledge and application of infection control measures or the absence of local and national policies.\u003C\u002Fp>\n\n\n\n\u003Cp>If the \u003Cstrong>environmental factors in a healthcare facility, as well as the debilitated state of patients, were not enough for the rampant HAI-rage in hospitals, the increased power of resistance in microorganisms also contributes to humanity’s defeat vis-á-vis infections\u003C\u002Fstrong>. The \u003Ca href=\"http:\u002F\u002Fwww.who.int\u002Fnews-room\u002Ffact-sheets\u002Fdetail\u002Fantimicrobial-resistance\">WHO estimates\u003C\u002Fa> that in 2016, 490 000 people developed multi-drug resistant tuberculosis globally, and drug resistance is starting to complicate the fight against HIV and malaria as well. However, we have to acknowledge that \u003Cstrong>one of the main reasons for HAI is the failure of healthcare professionals to wash their hands between patients or before procedures\u003C\u002Fstrong>. The World Health Organization also considers hand washing \u003Ca href=\"http:\u002F\u002Fwww.who.int\u002Fgpsc\u002F5may\u002Ftools\u002Fwho_guidelines-handhygiene_summary.pdf\">as the primary measure proven to be effective in preventing HCAI\u003C\u002Fa> and the spread of antimicrobial resistance.\u003C\u002Fp>\n\n\n\n\u003Cp>Still, the \u003Ca href=\"https:\u002F\u002Fwww.ncbi.nlm.nih.gov\u002Fpmc\u002Farticles\u002FPMC3249958\u002F\">adherence rates of healthcare workers are rather low on average\u003C\u002Fa>. Thus the number of HAIs might be more effectively reduced. This method has many limitations – including bias and inadequate sample sizes projecting an inaccurate compliance picture.\u003C\u002Fp>\n\n\n\n\u003Cp>Monitoring through specific sensors entirely changes that. For example, medical staff can wear the Biovigil badge on their coats. \u003Cstrong>The little gadget senses whether or not the doctors or nurses washed their hands: if so, it turns green\u003C\u002Fstrong>. After a while, it reminds the wearer to rewash their hands and becomes red if too much time passed from the last hand wash. Beyond the constant reminder, the badge also helps to make transparent how committed the health facility, or its members are towards hand hygiene.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Ca href=\"https:\u002F\u002Fwww.vitalacy.com\u002F?cp=585\">Vitalacy\u003C\u002Fa> works similarly, although instead of a badge, healthcare professionals wear a wristband, which indicates when it&#8217;s time for a handwash.&nbsp;\u003Ca href=\"https:\u002F\u002Fitunes.apple.com\u002Fus\u002Fapp\u002Fiscrub-lite\u002Fid329764570?mt=8\">iScrub Lite\u003C\u002Fa> is an application for healthcare professionals for iOS who wish to monitor hand-hygiene compliance with a tap. The equipment works on a range of disinfectants that have never been atomized before, such as triethylene glycol, or TEG. The team used the gadget to atomize disinfectants onto environmental surfaces contaminated with bacteria and showed that it eliminated 100 percent of bacteria that commonly cause HAIs.\u003C\u002Fp>\n\n\n\n\u003Cp>The technology could become the new method for disinfecting hospital environments. However, it might also be used to deliver an entirely new class of medicines to patients via inhalers. The centuries-old notion that the source of ultimate medical knowledge resides in the heads of medical professionals is disappearing. \u003C\u002Fp>\n\u003C\u002Fdiv>\n\u003C\u002Fdiv>\n\n\n\n\u003Cp>Moreover, with the rise of lifestyle diseases like diabetes and obesity, healthcare costs are projected to escalate even further. As humanity can manage chronic diseases with increasing efficiency, life expectancy increases, paradoxically leading to even more severe shortage in healthcare access.\u003C\u002Fp>\n\u003C\u002Fdiv>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Responsible governments prepare for this change\u003C\u002Fh2>\n\n\n\n\u003Cp>In such turbulent times, it is the responsibility of regulators and policymakers to keep all stakeholders in line in order to truly leverage the potential of technologies for healthcare and avoid dystopic outcomes. It is a difficult task to undertake. Governments should listen to people, technology companies, drug producers, healthcare providers and shape policy according to real needs while keeping pace with innovation. Policymakers must find a balance between safety and innovation. They should promote the safe use of digital health technologies while offering regulations to separate the wheat from the chaff. Besides, they should think about all the potential legal consequences of the usage of healthcare technologies, including data protection.\u003C\u002Fp>\n\n\n\n\u003Cp>Fortunately, several governments are already consciously preparing for the future. Canada, New Zealand, Denmark or Estonia are on the way to an entirely digital healthcare system – every one of them being at a different stage of development, delivering already implemented good practices for other nations. \u003C\u002Fp>\n\n\n\n\u003Cp>Recognizing that issues vary across countries, regions, and medical specialties; we have strived to include as many exemplary cases as possible in this policy report. They are grouped together around the four most important topics: 1: patient design replacing patient centricity, 2: the regulation of digital health, 3: national policies and 4: regulating AI.\u003C\u002Fp>\n\n\n\n\u003Cp>Let&#8217;s see a few examples from the report!\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">\u003Ca href=\"https:\u002F\u002Fwww.jmir.org\u002F2022\u002F8\u002Fe39178\" target=\"_blank\" rel=\"noreferrer noopener\">Patient design vs patient centricity\u003C\u002Fa>\u003C\u002Fh3>\n\n\n\n\u003Cp>A paradigm shift is underway in the patient-clinician relationship, driven by irreversible changes in information access, yet the model under which clinicians are trained, care is conducted, and care delivery is designed has not changed significantly even though we call it “patient-centered.” It is increasingly important to understand the crucial difference between patient design and the illusory “patient-centric” approach that keeps patients in passive roles and means nothing more than “we might think about you when we make decisions”.\u003C\u002Fp>\n\n\n\n\u003Cp>Patient design \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002F3-reasons-why-patient-design-must-replace-the-patient-centricity-illusion\u002F\" target=\"_blank\">is a different concept\u003C\u002Fa>, one that invites patients as active participants – and stakeholders – in the highest levels of decision-making in healthcare. To achieve medicine’s new potential, it must be optimised around the wants and priorities of the ultimate stakeholder—the party that has the most at stake in how it all plays out: the patient. Patient design is the approach that can make it happen.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">Regulating digital health &#8211; how to handle digital therapeutics\u003C\u002Fh3>\n\n\n\n\u003Cp>Digital health applications and solutions are increasingly becoming part of our healthcare experience. Partially because they offer fast and convenient solutions for problems arising from capacity shortages of traditional medical systems (like skin-checking apps), or because they enhance the capabilities of doctors, medical personnel, or hospitals (like sepsis watch algorithms or AI diagnostics models).\u003C\u002Fp>\n\n\n\n\u003Cp>While these apps are available in many medical specialties, channelling them into state-run healthcare systems to harness the benefits on population-wide scales is challenging. \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fdiga-how-germany-channeled-digital-health-apps-into-its-healthcare-system\" target=\"_blank\">Germany’s DiGA system\u003C\u002Fa> tackles this issue.\u003C\u002Fp>\n\n\n\n\u003Cp>Based on the Digital Healthcare Act, 73 million Germans covered by statutory health insurance (approximately 90% of the total population) are entitled to use DiGAs, provided they have a prescription or an attested diagnosis. This means that digital healthcare has made it into primary care.\u003C\u002Fp>\n\n\n\n\u003Cp>Statutory health insurers reimburse the associated DiGA costs, with prices negotiated in advance with the umbrella association of German health insurance companies.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">National policies &#8211; New Zealand gets outside input into government policy\u003C\u002Fh3>\n\n\n\n\u003Cp>The Government of New Zealand has a public health strategy, a health technology strategy and they invited people to give feedback on the upcoming digital health strategy too. They create the report with the digital health sector through literature review, briefings to sector groups, face-to-face workshops and online collaboration.\u003C\u002Fp>\n\n\n\n\u003Cp>Their opinions will be incorporated into the final report and will be published under a Creative Commons license meaning that anyone can freely access them.\u003C\u002Fp>\n\n\n\n\u003Cp>The Strategy is not meant to be a detailed plan, nor a document to sit on a bookshelf, but aspirational goals and enabling strategies, priorities, frameworks, guidelines and resources that will evolve and change over time in response to the changing digital world that New Zealanders live in.\u003C\u002Fp>\n\n\n\n\u003Cp>They also expect continual revision of the Strategy as the digital future emerges, which is an excellent approach to address the changing needs of patients.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">Regulating artificial intelligence &#8211; Large language models\u003C\u002Fh3>\n\n\n\n\u003Cp>The rapid progress in artificial intelligence (AI) has given rise to advanced large language models (LLMs) like GPT-4 and Bard. These LLMs have already gained significant interest in the realm of healthcare due to their wide-ranging use cases such as aiding in clinical documentation, expediting insurance pre-authorization, summarizing scientific studies, and functioning as chatbots to respond to patient inquiries about their personal data and worries.\u003C\u002Fp>\n\n\n\n\u003Cp>Despite their immense transformative potential, it&#8217;s crucial to approach LLMs with caution. Their training methodology differs from other AI-powered medical technologies that have already undergone regulation, a distinction that becomes particularly significant in the sensitive arena of patient care.\u003C\u002Fp>\n\n\n\n\u003Cp>The latest iteration, GPT-4, introduced in March 2023, escalates the potential and risks of this technology in supporting various medical tasks. Alongside the inherent risks of misinterpreting its outputs due to inconsistent reliability, GPT-4 showcases advanced capabilities such as interpreting text within images and analysing their context.\u003C\u002Fp>\n\n\n\n\u003Cp>The task of regulating GPT-4 and other generative AI in the healthcare sector without stifling their revolutionary potential is a pressing and critical challenge. This is necessary to ensure patient safety, uphold ethical standards, and safeguard patient privacy.
