In a more general manner that’s what digital humanism is about. Instead of technological development serving the interests of big tech companies to the detriment of people by exploiting human weaknesses and by taking control out of their hands, humans should step up and say stop to technology degrading humans, creating or widening gaps in societies, disregarding diversity. Here are some ideas and principles about how that, namely digital humanism, could unfold in healthcare.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Technology vs. humans: who has control?\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Google, Facebook, Twitter, Amazon, YouTube, and co.: for years we have been hearing about how they reshape the world, but the harmful effects of tech companies of the ‘attention economy’ are still not taken seriously enough, although there are plenty of signs of how harmful they can be for societies. \u003C\u002Fp>\n\n\n\n\u003Cp>The system is failing, \u003Ca href=\"https:\u002F\u002Fwww.theguardian.com\u002Ftechnology\u002F2017\u002Fnov\u002F15\u002Ftim-berners-lee-world-wide-web-net-neutrality\">that’s what the founder of the World Wide Web, Tim Berners-Lee,\nsaid two years ago\u003C\u002Fa>.\nHe emphasized that \u003Cstrong>while digitalization\nopens unprecedented opportunities, it also \u003C\u002Fstrong>\u003Cstrong>\u003Ca href=\"https:\u002F\u002Fwww.informatik.tuwien.ac.at\u002Fdighum\u002Findex.php\">raises serious concerns\u003C\u002Fa>\u003C\u002Fstrong>\u003Cstrong>:\nthe monopolization of the Web, the rise of extremist opinions and behavior\norchestrated by social media, the formation of filter bubbles and echo chambers\nas islands of disjoint truths, the loss of privacy, and the spread of digital\nsurveillance\u003C\u002Fstrong>. Remember\nthe news about Facebook becoming a central tool for spreading propaganda\nagainst the Rohingya in Myanmar – even \u003Ca href=\"https:\u002F\u002Fwww.theguardian.com\u002Ftechnology\u002F2018\u002Fmar\u002F13\u002Fmyanmar-un-blames-facebook-for-spreading-hatred-of-rohingya\">the UN blamed the platform for it\u003C\u002Fa> – or stepping up support for Rodrigo Duterte in the\nPhilippines? Or \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Ffacebook-in-healthcare\">when the Russians tried to use social media platforms\npopulating them with fake news\u003C\u002Fa> in the 2016 US elections to divert its outcome?\u003C\u002Fp>\n\n\n\n\u003Cp>However, we don’t have to go this far, just look at the individual level. \u003Cstrong>Online companies are building technology that’s addictive and attempts to catch users’ attention at any price – even by exploiting human weaknesses and harnessing our worst psychological flaws\u003C\u002Fstrong>. According to statistics, the average person looks at their phones \u003Ca href=\"https:\u002F\u002Fwww.inc.com\u002Fjohn-brandon\u002Fscience-says-this-is-the-reason-millennials-check-their-phones-150-times-per-day.html\">150 times a day\u003C\u002Fa>. \u003Ca href=\"https:\u002F\u002Fwww.businessinsider.com\u002Fbroken-attention-economy-op-ed-2019-3\">Every time we get a notification, the brain releases a flood of dopamine as addictive as the reward signal a gambler gets\u003C\u002Fa> as they pull the handle on \u003Ca href=\"https:\u002F\u002Fwww.inc.com\u002Fbetsy-mikel\u002Fformer-google-designer-says-1-simple-trick-can-curb-your-smartphone-addiction.html\">a slot machine\u003C\u002Fa>. Likes? Shares? New e-mail? It doesn’t matter until it keeps you engaged. Are you inclined to read news tailored to your interests for a longer time? They create your very own filter bubble and you don’t have to confront conflicting opinions ever again. Do you feel more excited when watching conspiracy theories? They put content in front of you about the wildest ideas out there. \u003C\u002Fp>\n\n\n\n\u003Cp>In an excellent lecture, \u003Ca href=\"http:\u002F\u002Fwww.tristanharris.com\u002F\">Tristan Harris\u003C\u002Fa>, former design ethicist at Google and co-founder of the \u003Ca href=\"http:\u002F\u002Fhumanetech.com\u002F\">Center for Humane Technology\u003C\u002Fa>, mentioned that from over 1 billion hours of YouTube watched daily, 70 percent are from the recommendations. It’s not users actively seeking out videos about, say flat earth conspiracies, but YouTube puts them in front of watchers as their viewing results in longer engagement on the site. And users diligently click on the next video, join the recommended Facebook group or allow notifications from dozens of sites. However, if you look at smart algorithms as\ncontrol mechanisms in robots, or VR\u002FAR embodying an immersive or interactive\nenvironment to intelligent software programs, \u003Cstrong>you’ll find that the ultimate technology that we have to worry about is\nsmart algorithms and artificial intelligence. Algorithms harnessing the power\nof big data, the achievements of computer vision and natural language\nprocessing – and the capabilities to take over human agency\u003C\u002Fstrong>. \u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>No wonder that concerns about A.I. development not only revolve around smart algorithm-based automation replacing jobs but also how it may overtake the human race in the cockpit\u003C\u002Fstrong>. \u003Ca href=\"http:\u002F\u002Fwww.bbc.com\u002Fnews\u002Ftechnology-30290540\">Stephen Hawking even said that\u003C\u002Fa> the development of full artificial intelligence could spell the end of the human race. Elon Musk agreed. \u003Ca href=\"https:\u002F\u002Fvimeo.com\u002F332532972?fbclid=IwAR1Y3oWsp-KksNfGeISXkwJmL81qhCeZPm0mS-DtULN3XkLE6dB5HiZfJ0Q\">Tristan Harris said\u003C\u002Fa> that artificial social systems coupled with overwhelming A.I. and extractive incentives coming from the biggest technology companies are resulting in the exploitation of human weaknesses and the downgrading of humans. What kind of solutions are at our disposal to go against\nthe interests of technology companies and the inherent biases of their\nproducts? \u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>The\nsolution is about digital humanism\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Michael Stampfer, \u003Ca href=\"https:\u002F\u002Fwww.wwtf.at\u002Fabout\u002Foffice\u002Findex.php?lang=EN\">Managing Director of the Vienna Science and Technology\nFund (WWTF)\u003C\u002Fa> said in a\nconference about digitalization in Vienna that \u003Cstrong>the problem is not that the biggest tech companies misused neutral\ntechnologies, but that they build certain principles, values, and interests\ninto their products already in the design phase.\u003C\u002Fstrong> Who says that Facebook has\nto have a “like button” turning the platform into a slot machine or that\nnotifications should pop up whenever you haven’t used Instagram for a while\nreminding you of other friends sharing their memories, turning the platform\ninto a social competition about ‘whose life is cooler’? Stampfer believes that \u003Cstrong>the hidden interests coded into algorithms\nshould be taken out into the spotlight, and channel the values of humanism into\nthe core of technological development. \u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>That requires self-control from the part of the biggest tech companies, which might be a problematic request from enterprises competing for higher profits on a brutal market. But at least in rhetoric, \u003Ca href=\"https:\u002F\u002Fwww.theverge.com\u002F2018\u002F1\u002F17\u002F16903844\u002Ftime-well-spent-facebook-tristan-harris-mark-zuckerberg\">Mark Zuckerberg, for example, embraced Tristan Harris’ idea “time well spent” as a design goal for Facebook\u003C\u002Fa>. The latter says that the solution lies in the creation of “humane technology”, which means interfaces that are responsive to human needs and considerate of human frailties. We went through the\nprinciples and contemplated on what they mean in relation to healthcare and how\nthey could be adapted to medicine.\u003C\u002Fp>\n\n\n\n\u003Col class=\"wp-block-list\">\n\u003Cli>\u003Cstrong>Digital technologies should be designed to promote democracy and inclusion. \u003C\u002Fstrong>In the case of healthcare, that should mean technologies accessible to anyone and solutions that go beyond social, financial, or educational barriers.\u003C\u002Fli>\n\n\n\n\u003Cli>\u003Cstrong>Privacy and freedom of speech are essential values for democracy and should be at the center of our activities. \u003C\u002Fstrong>Safeguarding sensitive patient information should be considered as a high priority – even more than in the case of “regular” data. As with the appearance of huge amounts of genetic and genomic data, unauthorized access to such health information would not only mean jeopardizing the current state of patients but also meddling with their future. \u003C\u002Fli>\n\n\n\n\u003Cli>\u003Cstrong>Effective regulations, rules, and laws, based on a broad public discourse, must be established.\u003C\u002Fstrong> We have been pushing for policy-makers, regulators, and public institutions to step up their game for years. However, what \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fsenators-dont-understand-facebook-will-make-sense-digital-health\">the two-day congressional hearing of Mark Zuckerberg showed last year\u003C\u002Fa> was that the understanding of technologies among lawmakers is quite superficial – though that’s the first step of effective regulations. But if they cannot make sense of Facebook, what will they do with bioterrorism, artificial intelligence, exoskeletons, or virtual reality treatments?\u003C\u002Fli>\n\n\n\n\u003Cli>\u003Cstrong>Regulators need to intervene with tech monopolies\u003C\u002Fstrong>. Beyond the above requirement from lawmakers, \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fhealthcare-and-medicine-needs-a-global-fda\">The Medical Futurist already recommended setting up a global FDA-like entity\u003C\u002Fa>. This would enable the effective regulation of the activities of many disruptors who are by-passing the nods of regulatory agencies and just target the right population on online channels no matter in what country they are.\u003C\u002Fli>\n\n\n\n\u003Cli>\u003Cstrong>Decisions with consequences that have the potential to affect individual or collective human rights must continue to be made by humans. \u003C\u002Fstrong>We have been evangelizing for years that automated decision-making systems in healthcare should only support human decision-making, not replace it. The transparency, as well as accountability of smart algorithms, should be among the first concerns of health tech developers. \u003C\u002Fli>\n\n\n\n\u003Cli>\u003Cstrong>Scientific approaches should cross different disciplines and academic and industrial researchers must engage openly with wider society and reflect upon their approaches. \u003C\u002Fstrong>We are advocating for the collaboration of different disciplines as well as that of different groups in society as that increases the efficiency of health technological products. The challenges of diversity are way too prevalent in healthcare, too – just look at \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Ffemtech-womens-health\">the femtech market\u003C\u002Fa> – and we should do everything in order to close the gaps.\u003C\u002Fli>\n\n\n\n\u003Cli>\u003Cstrong>Practitioners everywhere ought to acknowledge their shared responsibility for the impact of information technologies. \u003C\u002Fstrong>The Medical Futurist team has been contemplating the impact of futuristic technologies on society, as well as the medical community for years. That’s why Dr. Bertalan Meskó launched a pilot course „Lessons in Digital Health” at Semmelweis Medical School in the 2017 autumn semester to experience how the principles and skill-set necessary for 21st-century doctors could be taught in practice. By now, the course was already applied in several medical schools, such as the UP College of Medicine in The Phillippines and the School of Medicine at Marmara University in Istanbul, Turkey. \u003C\u002Fli>\n\n\n\n\u003Cli>\u003Cstrong>Education on computer science\u002Finformatics and its societal impact must start as early as possible\u003C\u002Fstrong>. The Medical Futurist also believes that \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fkids-choose-stem-for-your-future\">STEM education coupled with social sciences and the arts should start as early as possible\u003C\u002Fa>. Will it be able to replace the art of making a correct diagnosis? Not anytime soon.\u003Cbr>Many times after my talks, people ask me whether algorithms could theoretically be better at making a diagnosis than doctors. With my doctor’s cap on, I must defend the art of medicine. But as a medical futurist I need to tell my honest views. Making a diagnosis is an art. We humans are not engineering products, and therefore measuring a few parameters and tweaking a few knobs will not diagnose and cure our diseases.\u003C\u002Fp>\n\n\n\n\u003C!--more-->\n\n\n\n\u003Cp>Part of the art of diagnosis is looking at the whole human being. The way the patient walks, speaks, smells, or thinks are all important to the final diagnosis.\u003C\u002Fp>\n\n\n\n\u003Cp>The other part of a correct diagnosis is learning and gathering whatever information is relevant to the patient’s case. Being a physician entails a commitment towards life–long learning. Doctors frequently dive into biomedical databases of peer–reviewed papers to find the information they need. Keeping up to date with the latest literature in our field of interest is normal routine in the life of a physician.\u003C\u002Fp>\n\n\n\n\u003Cp>The first part is what improves with experience, requires creativity and instinct. Technology might never replace that. Wearable devices and home monitoring services can measure a lot of parameters, but the physician’s impression on meeting a patient is irreplaceable.\u003C\u002Fp>\n\n\n\n\u003Cp>The other part is where everybody fails. No doctor in the world can be perfectly up–to–date on recent medical advances as there are 25 million papers on \u003Ca href=\"http:\u002F\u002Fwww.Pubmed.com\" target=\"_blank\" rel=\"noopener\">Pubmed.com\u003C\u002Fa>. No doctor can be certain they have found all the pertinent information their case requires. To do so, we do need help from the world of technology.\u003C\u002Fp>\n\n\n\n\u003Ch1 class=\"wp-block-heading\">Computers practicing medicine\u003C\u002Fh1>\n\n\n\n\u003Cp>In 1996, the IBM supercomputer Deep Blue challenged Garry Kasparov, the reigning world chess champion. Kasparov won, and headlines around the world celebrated the triumph of humanity over the computer. IBM used the experience from the match to improve Deep Blue’s algorithms, and asked for a re–match in 1997. Kasparov lost to Deep Blue this time, 2.5 to 3.5. Advanced Chess players played against one another by using chess software. The human player decides the move, but the human–software pairing is considered a team. This is a winning combination of human creativity and the power of computing.\u003C\u002Fp>\n\n\n\n\u003Cdiv class=\"wp-block-image\">\u003Cfigure class=\"alignleft\">\u003Ca class=\"img\" href=\"http:\u002F\u002Famzn.to\u002F1LRAp3t\" target=\"_blank\" rel=\"noopener\">\u003Cimg loading=\"lazy\" decoding=\"async\" width=\"300\" height=\"196\" src=\"https:\u002F\u002F1712507217.rsc.cdn77.org\u002Fwp-content\u002Fuploads\u002F2016\u002F05\u002FMHU_promo_300x196.gif\" alt=\"my health upgraded\" class=\"wp-image-13147\"\u002F\u003C\u002Fa>\u003C\u002Fa>\u003C\u002Ffigure>\u003C\u002Fdiv>\n\n\n\n\u003Cp>But there have been no such digital aids in medicine – until now. \u003Ca href=\"http:\u002F\u002Fwww.ibm.com\u002Fsmarterplanet\u002Fus\u002Fen\u002Fibmwatson\u002Fhealth\u002F\" target=\"_blank\" rel=\"noopener\">Watson\u003C\u002Fa>, IBM’s new supercomputer, aims to fill this gap in medicine. After it beat two highly skilled players in the television quiz show Jeopardy, US clinics started testing its application in the practice of medicine. The advantage Watson offers is an ability to comb through patient records, English textbooks, and millions of medical papers in existing databases – in seconds instead of decades. Its algorithms arrive at diagnostic suggestions, and assign probable success rates to them. In the end the treating physician makes the decision, assisted by all pertinent information gleaned by Watson.\u003C\u002Fp>\n\n\n\n\u003Cp>Two features of Watson are noteworthy. It employs natural language processing, meaning it can understand written and spoken language. It also uses deep question–and–answer technology. It can enter into conversation and learn more during it. Given that medical practices use different electronic medical records, Watson must be able to understand both structured and unstructured data. Some physicians take notes about a diabetes patient, mentioning “diabetes” or “T1D” in the clinical summary. Understanding natural language means distinguishing between a note that is important and one that is not in a given context.\u003C\u002Fp>\n\n\n\n\u003Ch1 class=\"wp-block-heading\">IBM Watson at the oncology clinic\u003C\u002Fh1>\n\n\n\n\u003Cp>The MD Anderson Cancer Center was one of the first leading clinics that \u003Ca href=\"https:\u002F\u002Fwww.mdanderson.org\u002Fnewsroom\u002F2013\u002F10\u002Fmd-anderson--ibm-watson-work-together-to-fight-cancer.html\" target=\"_blank\" rel=\"noopener\">announced\u003C\u002Fa> it would start using Watson. In oncology, mountains of studies are published every day, and every patient’s case is unique due to cancer’s ability to randomly mutate. Finding information relevant to a particular patient’s case is difficult no matter how much experience the treating physician has. When Watson goes over the patient’s case, it comes up with the list of suggestions for treatment and assigns a confidence value between very low and very high. MD Anderson researchers evaluated Watson’s success rates and found it to be very efficient. Because physicians rate the suggestions that Watson comes up with, it improves with each case.\u003C\u002Fp>\n\n\n\n\u003Cfigure class=\"wp-block-embed\">\u003Cdiv class=\"wp-block-embed__wrapper\">\nhttps:\u002F\u002Fwww.youtube.com\u002Fwatch?v=hbqDknMc_Bo\n\u003C\u002Fdiv>\u003C\u002Ffigure>\n\n\n\n\u003Cp>In May 2015, a collaboration between Watson, Epic – a software company focusing on electronic health records – and the Mayo Clinic began. Epic has 350 customers that exchange over 80 million medical records annually. The \u003Ca href=\"http:\u002F\u002Fwww.prnewswire.com\u002Fnews-releases\u002Fibm-watson-health-epic-and-mayo-clinic-to-unlock-new-insights-from-electronic-health-records-300077830.html\" target=\"_blank\" rel=\"noopener\">Mayo Clinic has more\u003C\u002Fa> than 1 million patient visits a year and conducts at least a thousand clinical trials at any one time. Using Watson to analyze that huge amount of data or answer the questions of patients seems like a good step forward. One Mayo Clinic oncologist called Watson’s potential to provide clinical trial information wherever is needed crucial.\u003C\u002Fp>\n\n\n\n\u003Ch1 class=\"wp-block-heading\">The verdict\u003C\u002Fh1>\n\n\n\n\u003Cp>A supercomputer becomes a cognitive computer when it tries to reproduce the behavior of the human brain through artificial intelligence. Human physiology is so complex that we can benefit from computers that mimic the way we think and pose questions. Such computers improve by learning just as physicians do – except they improve more quickly.\u003C\u002Fp>\n\n\n\n\u003Cp>So can a computer uncover relevant information to a patient’s case better? Absolutely. Can it diagnose more accurately than a physician? There is no reason to believe it won’t be able to, although we’ll have to wait years – maybe even decades &#8211; for that level of accuracy. Would such algorithms replace doctors then? I highly doubt it. Their role will change but they will always be needed. If you ask patients whether they want to be treated by a computer or a person, the majority will choose the person. We are social beings. We need to discuss our health issues not only for the sake of receiving proper treatment, but also because words alone can heal.\u003C\u002Fp>\n\n\n\n\u003Cp>And \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002F2016\u002F04\u002F28\u002Fthe-algorithm-of-empathy\u002F\" target=\"_blank\" rel=\"noopener\">there is currently no algorithm\u003C\u002Fa> or smartphone app for empathy or understanding.
