Our findings are\nthrilling: the newly established Danish National Genome Center strives to have at least 60,000 whole-genome\nsequenced in the next 5 years, while the Copenhagen Healthtech Cluster wants to\nset up a network of data registers updated so fast that it might enable helping\ndoctors real-time – perhaps even in the OR.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>With robust data systems towards integration\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>The \u003Ca href=\"https:\u002F\u002Fwww.forbes.com\u002Fsites\u002Fduncanmadden\u002F2018\u002F03\u002F27\u002Franked-the-10-happiest-countries-in-the-world-in-2018\u002F#1444087573e9\">third happiest country in the universe\u003C\u002Fa> has one of the most advanced digital health systems\nalongside an elaborated and concise \u003Ca href=\"https:\u002F\u002Fwww.healthcaredenmark.dk\u002Fmedia\u002F1611539\u002FThe-Danish-Digitalisation-strategy-2018-2022.pdf\">national digital health strategy for the next four\nyears\u003C\u002Fa>. The document\nemphasizes the importance of the cooperation of each and every healthcare actor\nthrough the easiest and fastest way, technology, with a clear purpose: to build\nan integrated network focusing on patients and looking at the person as a whole,\nnot just at the individual diagnosis.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Why is it so crucial for Denmark to build an \u003Cem>integrated\u003C\u002Fem> network? The Nordic countries as excellent examples of the social welfare state have been providing a social safety net to their citizens based on their \u003Cem>data \u003C\u002Fem>for decades\u003C\u002Fstrong> – so they have been front-runners in the collection of information on births, deaths, disease and its control through the centralized state. For example, information on causes of death has been collected since 1875, and cancer incidence has been registered for the whole country since 1943. The Danish National Patient Registry has been keeping records that date 30-40 years back, making it one of the oldest nationwide hospital registries in the world. Henning Langberg, Director of the Copenhagen Healthtech Cluster said that digitalized Danish health data is incredibly rich: crosses various fields, encompasses different periods and enmeshes every social stratum &#8211; practically about more than 5.5 million people.\u003C\u002Fp>\n\n\n\n\u003Cp>At the dawn of the digital age, \u003Cstrong>one would think that you just open the “data taps” and information will just flow freely and without hitches. However, every city, municipality, Danish region (an administrative unit below the state but above the settlements) and even hospital usually has its own solution, and one of the most difficult things is to apply innovations across regions\u003C\u002Fstrong>. Tobias Boggild-Damkvist, who has been suffering from Type 1 Diabetes since his teenage years and struggling with the management of Multiple Sclerosis, told The Medical Futurist that he has been developing \u003Ca href=\"https:\u002F\u002Fwww.linkedin.com\u002Fcompany\u002Frotohealth\u002Fabout\u002F\">medical devices\u003C\u002Fa> that empower people with diabetes to manage injections – as a person with diabetes has to inject 5-6 times a day, but can’t “use” the same spot twice in a row, the device helps “remember” injection spots. Everyone gets access to the \u003Ca href=\"https:\u002F\u002Fwww.sundhed.dk\u002F\">Sundhed.dk\u003C\u002Fa> patient portal, which allows any patient to access their own health data, make appointments with the doctors or look up their complete patient history. But that’s only one type of data registry: medical professionals, researchers or institutions get access to a wide range of clinical databases, biobanks, lab- or imaging systems – with high-quality data. \u003C\u002Fp>\n\n\n\n\u003Cp>There are \u003Cstrong>numerous efforts to leverage on all these digital bits and pieces, as well as the digital infrastructure. Looking for online solutions based on data could be especially useful for chronic patients to cut back on frequent visits to hospitals for monitoring\u003C\u002Fstrong>. For example, in Velje Hospital in Southern Denmark, doctors conducted a study for self-testing. Some patients receiving anticoagulation treatment were asked to do self-testing with an appropriate device once a week and report back on the indicated online channel. They didn’t have to go to the hospital, have the stress of waiting for the physician, do the testing and travel back home. So far 24 patients have completed the one-year weekly self-testing and reporting. It turned out that their results improved by 8 percent compared to the usual monitoring method with hospital measurements. The horizon of possibilities is as vast as the selection for public registers – and that’s exactly what the publicly funded organization aims to mine. Currently, \u003Cstrong>they are setting up their framework so that private businesses, research institutions, and individual researchers could get access from the state to look into public registers and specialized datasets for specific purposes.\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>According to Langberg, their overall aim is to \u003Cstrong>establish a close collaboration between companies and the healthcare sector so that they can come up with fresh, out-of-the-box solutions for pressing issues\u003C\u002Fstrong>. And there’s no shortage of that. As everywhere else in the developed world, the number of the elderly and of the people with chronic diseases is rapidly growing – which requires fast responses. The Danish parliament adopted the law to establish the National Genome Center last May, and Peter Longreen, CTO told The Medical Futurist that they expect to build up their dedicated supercomputing infrastructure until this May, then begin large-scale whole-genome sequencing and build up their accompanying genome database around July. \u003C\u002Fp>\n\n\n\n\u003Cp>He believes that within the next five years, they will\nat least do 60,000 whole-genome sequences, but that’s his lowest estimation. “\u003Cem>In honesty, I think we will do much much\nmore. I think we’ll do several hundred thousands of these sequences. So, in\nfive years, we’ll have a very rich genome\ndatabase, some very enriched knowledge databases, and also databases with\nsecondary data\u003C\u002Fem>,” Longreen explained.\u003C\u002Fp>\n\n\n\n\u003Cp>However, it’s not like direct-to-consumer genetic or genomic testing where you order a spit kit and look up your results online within a couple of weeks. Gert Sorensen, CEO of the Danish National Genome Center said that the patient pathway would look like the following: \u003Cstrong>when you go to your family doctor, he refers you to the hospital, and you come to the point where the hospital finds out that there could be a genetic cause for your disease. Then, the doctor would offer you to have a genetic analysis done.\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Nevertheless,\nthese vast amounts of genomic data could\nsignificantly change the course of healthcare. The ability to do more precise\ndiagnostics, to be able to stratify the population or to prescribe\nindividualized drug cocktails will shift the way towards targeted treatments,\npersonalization – effectively, towards new methods\nof healing\u003C\u002Fstrong>. And Longreen\nsays we don’t even know what else we might find: “\u003Cem>this is a domain that&#8217;s constantly evolving, so we will also see the\ndesign of databases derived from treatments based on genomic information in the\nclinic. As in Denmark \u003C\u002Fstrong>\u003Cstrong>\u003Ca href=\"http:\u002F\u002Fps.au.dk\u002Fen\u002Fcurrent\u002Fallnews\u002Fnews\u002Fartikel\u002Ftrust-may-explain-the-good-state-of-danish-economy-and-the-countrys-successful-welfare-society\u002F\">citizens have the highest levels of trust in the\ngovernment as well as in other people in general\u003C\u002Fa>\u003C\u002Fstrong>\u003Cstrong>,\nit’s not a surprise that they would rather entrust their sensitive data with\nthe state rather than private companies\u003C\u002Fstrong>.\u003C\u002Fp>\n\n\n\n\u003Cp>In the U.S., a highly individualistic and\nentrepreneurial society, where the state has traditionally a much lesser role,\nthe idea might sound odd at first. In Central and Eastern Europe, where the\nstate has not such a good reputation, giving such sensitive data to the state might not even happen (voluntarily), and\nit would also easily raise some eyebrows in other parts of the world. \u003Cstrong>But with the appropriate checks and\nbalances put in place, the Danish model could work well – especially in light\nof the recent data breaches of huge tech\ncompanies – and the institutions seen so far are doing their best to prove it\u003C\u002Fstrong>.\u003C\u002Fp>\n\n\n\u003Cp>\u003Ciframe loading=\"lazy\" style=\"border: none;\" src=\"https:\u002F\u002Fwww.opinionstage.com\u002Fpolls\u002F2537825\u002Fpoll\" name=\"opinionstage-widget\" width=\"100%\" height=\"300px\" frameborder=\"0\" scrolling=\"no\">webkitallowfullscreen mozallowfullscreen allowfullscreen\u003C\u002Fiframe>\u003C\u002Fp>\n\n\n\u003Cp>The Danish National Genome Center has been working on\na security model where people who are granted access to sensitive patient data\nwould not be allowed to download, copy or remove data from the genome database,\nwhere one patient’s genetic analysis is only available in one copy. Beyond\nthat, anyone who wants to carry out a research project has to receive approval\nfrom the National Scientific Ethics Committee.\u003C\u002Fp>\n\n\n\n\u003Cp>The situation of the Danish Healthtech Cluster is a\nbit more complicated as they are collaborating with private companies. For the\norganization, the challenge is not only to build up technical solutions to\nencrypt data in the most secure way possible\nbut also to make sure patients trust them enough that their data will only be\nused for purposes that they would like them to be used for, explained Langberg.\nHe added that “\u003Cem>we don&#8217;t expect people to\nbe willing to give access to their private data to private companies, but I\nthink the way that companies should get access to data is through collaboration\nwith the public health authorities and health system\u003C\u002Fem>.” That’s what they are working on considering even some options\naround artificial intelligence to protect patient data as best as possible. So,\nin a couple of years, patients will have the chance to know that the most\nadvanced technical solutions guard both their genomic and their general patient\ndata.\u003C\u002Fp>\n\n\n\n\u003Cfigure class=\"wp-block-image\">\u003Cimg decoding=\"async\" src=\"https:\u002F\u002F1712507217.rsc.cdn77.org\u002Fwp-content\u002Fuploads\u002F2019\u002F02\u002FDanish-Patient-Data-512x340.jpg\" alt=\"Danish digital health\" class=\"wp-image-23026\"\u002F>\u003Cfigcaption>Source: www.trifork.com\u003C\u002Ffigcaption>\u003C\u002Ffigure>\n\n\n\n\u003Cp>All-in-all, to respond to the original question, it seems to us that Denmark is heading towards the direction stated in the digital health strategy &#8211; at least when looking at efforts in precision medicine and the management of patient data. \u003C\u002Fp>\n\n\n\n\u003Cp>It’s \u003Cstrong>in the process of building up an integrated digital health framework focusing on patients and guarding their data against any security or privacy issues.\u003C\u002Fstrong> Tobias Boggild-Damkvist said in a somewhat sarcastic tone that instead of the healthcare system “\u003Cem>trying to kill you in new and exciting ways\u003C\u002Fem>,” if you dream 20 years down the road, you hope you will be able to use all your personal data for the good of your children and grandchildren. That seems to be the general direction for Denmark at the moment. Beyond the commitment to healing, this is the second most important bastion of healthcare – and the requirement at the core of precision medicine.\u003C\u002Fp>\n\n\n\n\u003Cp>In the previous centuries, healthcare focused mainly on working out generalized solutions with which the biggest ratio of ill people could be treated. If cough syrup was good for the majority of the coughing masses and only two people had a rash as an allergic reaction to it, there was no question about treating sore throat with cough syrup. The experience-based and somewhat “trial-and-error” approach of medicine made place for evidence-based medicine. As a consequence,&nbsp;physicians not only prescribed some pills because their ancestors also used these drugs, but they proved the efficacy of treatments and diagnostic methods with the help of scientific tools, e.g. clinical studies. They mapped extensively why sore throat is best treated with cough syrup; and started to analyse the side effects of it. So, people with an allergic reaction got to know that they should rather not use it, but turn to an alternative.\u003C\u002Fp>\n\n\n\n\u003Cp>We know this from the current praxis as well. The medical community use a certain method or pharmaceutical product, if evidence is strong enough. Let me tell you, how precision medicine goes even further.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>What is exactly precision medicine?\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Precision medicine enables the medical community to fulfill the age-old requirements of healing and doing no harm at the same time. As the \u003Ca href=\"https:\u002F\u002Fghr.nlm.nih.gov\u002Fprimer\u002Fprecisionmedicine\u002Fdefinition\">National Institutes of Health (NIH) said\u003C\u002Fa>, it is &#8220;an emerging approach for disease treatment and prevention that takes into account individual variability in genes, environment and lifestyle for each person.&#8221; This approach will allow doctors and researchers to predict more accurately which treatment and prevention strategies for a particular disease will work in which groups of people. If you think about your blood type, the first example of precision medicine, you already have an idea about its importance. The “one-size-fits-all” strategy will definitely start to crumble. It is the logical result of hundreds of years of medical research and accumulated knowledge. Currently, we know that everyone has a different genetic code, may react differently to pharmaceutics or may have a completely opposite reaction to treatment as assumed. So why should we treat everyone with the same drugs or with the same method?\u003C\u002Fp>\n\n\n\n\u003Cp>However, precision medicine is not only about targeted treatments, but also about a transition in the approach of physicians as well as patients. Med students and residents do rarely have the opportunity to study precision medicine in medical schools. Moreover, the highly hierarchic and experience-based teaching methods rarely incorporate it into the life of new medical professionals. But precision medicine also implies a new relationship between patient and caregiver; which rather represents an equal partnership than the superior-subordinate-like relationship prevailing currently. While there is a myriad of treatment options, patients have different genetic backgrounds, metabolize drugs differently, make different lifestyle choices and have distinct life goals. The more relevant information the patient can bring to the doctor&#8217;s attention, the more personalized they can make the treatment together. It is an insanely huge number! As if the entire population of Guatemala got the disease at once! However, not only the figures of new cases look so gloomy. In 2012, an estimated 8.2 million people died from cancer worldwide – the entire population of Switzerland, if you like.\u003C\u002Fp>\n\n\n\n\u003Cp>And while the four most common cancers &#8211; lung, breast, bowel and prostate cancer &#8211; account for around 4 in 10 of all cancers diagnosed worldwide, there are many cancer types, where the disease has so many variations as the number of patients themselves. As a consequence, it is extremely difficult to treat it with the old methods.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>…but chemotherapy is no less disagreeable\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>As it is commonly known, cancer occurs when cells refuse to die and keep multiplying in various places in our bodies, while hiding from our immune systems. Currently, the most effective treatments against cancer comprise of various forms of radiation and chemotherapy, which stop the regeneration procedure for cells. The problem with chemotherapy and radiation is that it cannot be utilized in targeted ways. It means that they&nbsp;also affect the functioning of healthy cells, which has serious, sometimes even life-threatening side effects. Although physicians are also aware of this, it is still difficult to introduce innovations. The reason is the operational logic&nbsp;of such mammoth-like bureaucratic systems as the regulatory environment around healthcare, as well as the natural suspicion of the medical community towards novelties.\u003C\u002Fp>\n\n\n\n\u003Cp>However, technology has changed the equation. As it becomes more and more disruptive and accessible for people worldwide and patients are actively seeking help against this ugly disease, solutions beyond the framework of healthcare systems have piled up. For example, companies started to offer direct-to-consumer therapies; communities started to share their experiences and their own solutions regarding various diseases. Social media communities for sharing information about cancer and cancer research are growing; and it is no different in other areas of&nbsp;healthcare. On the one hand, researchers experiment with drugs that directly attack cancer cells without damaging other tissues; for example \u003Ca href=\"http:\u002F\u002Fwww.cancercenter.com\u002Fcervical-cancer\u002Ftargeted-therapy\u002F\">in treating cervical cancer\u003C\u002Fa>. On the fast developing area of nanotechnology, Swedish researchers have developed a technique that uses \u003Ca href=\"http:\u002F\u002Fwww.kurzweilai.net\u002Fnanoparticles-that-cause-cancer-cells-to-self-destruct\">magnetically controlled nanoparticles to force tumour cells to self–destruct\u003C\u002Fa> without harming surrounding tissue, as radiation and chemotherapy does. In case of IBM, they launched \u003Ca href=\"https:\u002F\u002Fwww.ibm.com\u002Fwatson\u002Fhealth\u002Foncology\u002F\">Watson for Oncology\u003C\u002Fa> to help cancer research; Google has its Deepmind Health project; while \u003Ca href=\"https:\u002F\u002Fwww.bloomberg.com\u002Fnews\u002Farticles\u002F2016-09-20\u002Fmicrosoft-develops-ai-to-help-cancer-doctors-find-the-right-treatments\">Microsoft’s research machine-learning project, dubbed Hanover\u003C\u002Fa>, aims to ingest all the papers and help predict which drugs and which combinations are the most effective.\u003C\u002Fp>\n\n\n\n\u003Cp>The Swiss company \u003Ca href=\"http:\u002F\u002Fwww.oncompassmedicine.com\u002F\">Oncompass\u003C\u002Fa> offers oncological biopsy analysis in order to provide better personalized treatment plans for each cancer patient. The Boston-based \u003Ca href=\"https:\u002F\u002Fwww.veritasgenetics.com\u002F\">Veritas Genetics\u003C\u002Fa> offers \u003Ca href=\"https:\u002F\u002Fwww.genomeweb.com\u002Fsequencing-technology\u002F999-whole-genome-sequencing-service-veritas-embarks-goal-democratize-dna\">whole genome sequencing, interpretation and genetic counselling for $999 for anyone\u003C\u002Fa>. They also provide the $299 myBRCA HiRisk test, which gauges 26 genes associated with heightened predisposition for breast, ovarian and other associated cancers. The UK-based start-up,&nbsp;\u003Ca href=\"https:\u002F\u002Fnanoporetech.com\u002F\">Oxford Nanopore Technologies\u003C\u002Fa> promises to further raise the stakes. They offer real-time, out-of-the-lab DNA sequencing, which made it possible to \u003Ca href=\"https:\u002F\u002Fnanoporetech.com\u002Fpublications\u002Freal-time-dna-sequencing-antarctic-dry-valleys-using-oxford-nanopore-sequencer\">carry out offline DNA sequencing of environmental samples on Antarctica\u003C\u002Fa> using only a hand-held device. Thus, it is getting more and more difficult to figure out to which start-up to turn to or which product to choose. This is exactly the reason behind such initiatives as Precision Medicine for Me. This grass-root information pool collects patient organizations, advocates, start-ups and industry leaders to share with patients which treatment options are the best for them based on their personalized data.\u003C\u002Fp>\n\n\n\n\u003Cp>They do what physicians and decision-makers are supposed to do. But as regulators and the traditional healthcare setting is too slow for them, they are not waiting. They are rather trying to find their own way out of disease as soon as possible.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Where are the governments hiding?\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>There were already efforts to incorporate precision medicine into mainstream healing practices. In the US, during his presidency, \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Ftechnologies-that-will-shape-the-future-of-cancer-care\u002F\">Barack Obama announced the Cancer Moonshot\u003C\u002Fa>, an effort to find a cure for cancer; as well as the \u003Ca href=\"https:\u002F\u002Fobamawhitehouse.archives.gov\u002Fprecision-medicine\">Precision Medicine Initiative\u003C\u002Fa>, a bold research effort with the aim to offer the benefits of targeted treatments to every American. Although it’s unclear exactly how those cuts would directly impact personalized medicine, researchers and precision medicine advocates raised concerns. They believe it would have a broader impact on efforts to advance precision medicine. I believe it would be very sad to see a U-turn in the US regulatory efforts. As scientific and technological development is unstoppable, the primary task of regulators would be to foster research and enable regulated access to the latest innovations.\u003C\u002Fp>\n\n\n\n\u003Cp>Not the US is the only geographic location, where precision medicine got into the view of decision-makers. In March 2012, as swarm of healthcare experts and patient advocates established the \u003Ca href=\"http:\u002F\u002Feuapm.eu\u002F\">European Alliance for Personalized Medicine\u003C\u002Fa>. It is calling for the European Commission, the European Parliament and EU member states to help improve the regulatory environment so that Europe’s patients and citizens can have early access to personalized healthcare; and public health research receives&nbsp;appropriate support. It is true that the media, developers and supporters of digital health or technological advancement tend to get overexcited about new innovations and \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-12-most-overhyped-technologies-in-healthcare\u002F\">some fall in the trap of over-hyping\u003C\u002Fa>. Such stories as the short-lived fame and fall of the blood-testing company, \u003Ca href=\"http:\u002F\u002Fwww.vanityfair.com\u002Fnews\u002F2016\u002F09\u002Felizabeth-holmes-theranos-exclusive\">Theranos\u003C\u002Fa> do not help either.\u003C\u002Fp>\n\n\n\n\u003Cp>In December 2016, \u003Ca href=\"https:\u002F\u002Fwww.theatlantic.com\u002Fhealth\u002Farchive\u002F2016\u002F12\u002Fthe-peril-of-overhyping-precision-medicine\u002F510326\u002F\">The Atlantic magazine published an article\u003C\u002Fa> arguing against the over-hyping of precision medicine. Although the author is right about the adverse effects of overhyping, it is completely wrong about the prospects of precision medicine. It ignores the fact that what used to be a brave statement before about the future of medicine has become technically possible with new technologies. Genome sequencing costs hundreds of dollars instead of millions. Cancer cells can be extracted from the patient’s blood instead of the need for a tissue biopsy.
