Knowing the medical reality, he inquired whether those existing computers and accompanying systems would allow the use of advanced technologies such as artificial intelligence (AI). Practically, he wanted to know how I think existing hardware should be upgraded to make it ready for AI.\u003C\u002Fp>\n\n\n\n\u003Cp>To answer him, I made a seemingly unrelated analogy coming from the video game industry: cloud gaming. This approach, where the cloud handles all the processing power, has gained traction; bypassing the need for powerful PCs and consoles altogether. I told him that I believe \u003Cstrong>the same fate awaits medical professionals and hospitals with limited IT infrastructure that still want to benefit from power-hungry technologies like AI\u003C\u002Fstrong>.\u003C\u002Fp>\n\n\n\n\u003Cp>And by now, \u003Ca href=\"https:\u002F\u002Fwww.healthcareitnews.com\u002Fnews\u002Fgoogle-cloud-teams-carecloud-bring-generative-ai-small-practices\" target=\"_blank\" rel=\"noreferrer noopener\">reality has caught up\u003C\u002Fa> with this prediction: CareCloud already uses Google Cloud to help power its own operational needs and enable generative AI and search functionality in its products.\u003C\u002Fp>\n\n\n\n\u003Cp>By using Google&#8217;s offerings such as &nbsp;Vertex AI and Generative AI App Builder, the company is developing tools to bring physicians at smaller practices the same AI-enabled capabilities that are increasingly available at large hospitals and health systems.&#8221;\u003C\u002Fp>\n\n\n\n\u003Cp>Physicians working in smaller practices could ask generative AI more complex clinical questions and gain deeper insights with evidence-based recommendations derived from analyzing datasets.\u003C\u002Fp>\n\n\n\n\u003Cp>And it all makes sense. Rather than upgrading physical hardware, cloud computing technology can upgrade healthcare institutions, and provide access to advanced technologies; while circumventing the need for physical health IT upgrades. Such an approach has not been explored at large, but it holds promise for the future of healthcare as we shall see in this article.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>From the moons of Mars to the cloud\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>For the majority of its existence, the video game industry relied on consumer hardware. The bigger the storage, the better the graphics cards and the faster the processors that gamers’ PCs and consoles could pack, the superior performance they would get, allowing them to play the latest games with more features. This bigger-better-faster approach extended past the gaming machines to the games themselves. The latest instalment in the series released in 2020, \u003Cem>DOOM Eternal\u003C\u002Fem>, boasts better visuals, and lets you roam the surface of Mars but has a \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Ftwinfinite.net\u002F2020\u002F03\u002Fdoom-eternal-install-size\u002F\" target=\"_blank\">file size of up to 50 GB\u003C\u002Fa>.\u003C\u002Fp>\n\n\n\n\u003Cp>However, in recent years, video games have increasingly bypassed storage media hardware altogether in favour of cloud storage. No need for physical copies, your games follow you wherever there’s an internet connection. Platforms like Epic Games Store and Steam serve as distributors for these digital versions of games, and digital sales from such platforms first \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fscreenrant.com\u002Fdigital-game-sales-consoles-outnumber-physical-first-time\u002F\" target=\"_blank\">outsold physical sales\u003C\u002Fa> in 2020.\u003C\u002Fp>\n\n\n\n\u003Cp>Now, even gaming systems are going digital with cloud technology. With services like \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fstadia.google.com\u002F\" target=\"_blank\">Google Stadia\u003C\u002Fa> and \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.amazon.com\u002Fluna\u002Flanding-page\" target=\"_blank\">Amazon Luna\u003C\u002Fa>, \u003Cstrong>gamers don’t need high-end PCs or new consoles to play the latest games, which would traditionally require such devices\u003C\u002Fstrong>. In fact, with a high-speed internet connection, they can play those demanding games like \u003Cem>DOOM Eternal\u003C\u002Fem>, on a supported, yet underpowered device they bought a couple of years ago. All this is made possible thanks to the power of the cloud, which can similarly give a boost to existing healthcare infrastructure.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Cloud magic\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Cloud gaming works through \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fazure.microsoft.com\u002Fen-us\u002Foverview\u002Fwhat-is-cloud-computing\u002F\" target=\"_blank\">cloud computing technology\u003C\u002Fa>. Rather than having the user’s computer handle all the processing to play a demanding game, it’s done remotely via servers which provide the computing power to the user and relay it via the net, or “the cloud.” It’s basically having powerful, distant PCs do the computing, while you benefit from the same software output on an underpowered system. You might even think of it as a beefed-up Netflix. Cloud gaming subscribers choose the game they want to play on a supported device anywhere with a robust connection and the servers will run the game while the player remotely controls it.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>It’s particularly handy for those who want to play a new game but can’t afford to buy a new console to play it on. They just use their cloud subscription on a device they already own and \u003Cem>voilà\u003C\u002Fem>, new software on older hardware. The continent is already \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fafrica-hotspot-digital-health\u002F\" target=\"_blank\">a hotspot for digital health\u003C\u002Fa>, but the infrastructure might limit the adoption of more advanced and helpful technologies like AI. These require newer systems to operate, which might not be so widely available. \u003Cstrong>So, instead of fitting medical institutions with costly, top-of-the-line computers to run those, how about outsourcing the processing power and providing it on demand?&nbsp;\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>This is what’s possible with cloud computing, and it circumvents the whole health IT upgrade process in healthcare institutions with limited IT infrastructure. The technology isn’t totally foreign to healthcare; systems already employ cloud technology from \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Faws.amazon.com\u002Fhealth\u002F\" target=\"_blank\">Amazon Web Services\u003C\u002Fa> and \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fazure.microsoft.com\u002Fen-us\u002Findustries\u002Fhealthcare\u002F\" target=\"_blank\">Microsoft Azure\u003C\u002Fa>. This helps develop chatbots and health platforms to more securely manage the increased data capacity that comes with the digitisation of healthcare. In fact, health systems \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.businessinsider.com\u002Fmicrosoft-bringing-cloud-ai-tech-to-clinicians-2019-7\" target=\"_blank\">spent almost $40 billion in 2022\u003C\u002Fa> on cloud computing. However, these haven&#8217;t been employed at a large scale for demanding technologies like AI.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>AI enters the game\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>In September 2020, a first step towards such an offering was made. Google Cloud partnered with the U.S. Department of Defense (DoD) to \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.mobihealthnews.com\u002Fnews\u002Fgoogle-cloud-provide-dod-ai-enabled-cancer-diagnostic-system\" target=\"_blank\">develop an AI-based digital pathology system\u003C\u002Fa>. It involved augmented reality microscopes to help physicians, assisted by an AI to improve the accuracy of cancer diagnoses.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>While this is just one example, potentials of the technology in healthcare \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002F7-things-you-can-expect-from-a-i-in-healthcare\u002F\" target=\"_blank\">are manifold\u003C\u002Fa>. These range from optimising logistics to digesting medical and research data at unprecedented rates to assist professionals in decision-making. Thus, demanding analyses of large amounts of data can be done more efficiently, which will in turn \u003Ca href=\"https:\u002F\u002Fwww.maginative.com\u002Farticle\u002Fmed-palm-m-is-a-multimodal-biomedical-ai-from-google-research-and-google-deepmind\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">help better train AI models\u003C\u002Fa> to improve their accuracy.\u003C\u002Fp>\n\n\n\n\u003Cp>The flexible resourcing aspect of cloud-based processing infrastructure also enables healthcare institutions to leverage AI’s potential in more ways than they could if they ran it in-house. \u003Cstrong>They could use the technology to streamline tasks such as triaging with chatbots, analyse health tracker statistics and offer personalised recommendations\u003C\u002Fstrong>;\u003Cstrong> while physicians focus on tasks that require professional medical attention. All this will be possible without upgrading the local IT infrastructure.\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Before elevating healthcare to the cloud\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Just like gamers are now buying affordable processing power from the cloud to play high-performing games without spending much more on newer hardware, similar possibilities open up for healthcare institutions. But while hardware might not need upgrades, other factors will determine its successful implementation.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Similar to cloud gaming services, a high-speed internet connection will be a must, in particular for demanding AI processes\u003C\u002Fstrong>. The WHO article \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.frontiersin.org\u002Farticles\u002F10.3389\u002Ffpubh.2019.00341\u002Ffull#B10\" target=\"_blank\">further notes\u003C\u002Fa> that unstable and expensive internet connectivity and unstable power supply can’t sustain advanced technologies. There are many models and methods, but we’ll most likely regularly meet with two these days:\u003C\u002Fp>\n\n\n\n\u003Cul class=\"wp-block-list\">\n\u003Cli>Generative Adversarial Networks (GANs), for image or sound generation, think of Midjourney, Blue Willow, AIVA or Loud.ly.&nbsp;\u003C\u002Fli>\n\n\n\n\u003Cli>And Transformers, that are behind text generators, like ChatGPT or Bard &#8211; also known as Large Language Models (or LLMs).&nbsp;\u003C\u002Fli>\n\u003C\u002Ful>\n\n\n\n\u003Cp>(The Transformers-LLM relation is like the insects-beetles situation. Transformers are a type of LLM, but not all LLMs are transformers. Transformers are the most popular architecture for LLMs these days because they are very effective for natural language processing tasks, but they are not the only ones.)\u003C\u002Fp>\n\n\n\n\u003Cp>We’ve already \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fgenerative-adversarial-networks-how-can-ai-learn-so-much-while-its-iq-remains-zero\" target=\"_blank\" rel=\"noreferrer noopener\">introduced how GANs work\u003C\u002Fa>, you can easily imagine it through the forger-art detective analogy. Transformers on the other hand can be best grasped as brilliant autocomplete machines: they are trained to predict what comes next based on the previously given data.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>How do they learn?\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>There are many methods to train generative AI models, and most of the now successful models use a combination of various methods.\u003C\u002Fp>\n\n\n\n\u003Cp>GANs are typically taught through unsupervised learning &#8211; which means that the AI is not learning from labelled data &#8211; there is no one showing it pictures labelled as \u003Cem>cat \u003C\u002Fem>and \u003Cem>not-cat\u003C\u002Fem>. The &#8220;supervision&#8221; in GANs comes from the adversarial process where the generator network (the forger) tries to fool the discriminator network (the art detective), and the discriminator network tries to correctly classify whether an input is real or fake. For a simpler explanation, head over to the previously linked GAN article.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Large language models, like ChatGPT or Bard are often trained using both supervised and self-supervised learning methods.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>For instance, the GPT (Generative Pretrained Transformer) series by OpenAI is trained using a form of self-supervised learning &#8211; according to ChatGPT. The models are given a large corpus of text and learn to predict the next word in a sentence given the previous words. In this case, the &#8220;labels&#8221; are the subsequent words in the sentences themselves, so no explicit labels are needed.\u003C\u002Fp>\n\n\n\n\u003Cp>Google’s Bard on the other hand was educated through a combination of supervised and self-supervised learning, on a massive dataset of text and code, including both labeled and unlabeled data, using the PaLM 2 model.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>General-use LLMs\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>ChatGPT was the first text-generating AI algorithm that became a massive hit among everyday people, getting a hundred million users in just two months, making it the \u003Ca href=\"https:\u002F\u002Fwww.reuters.com\u002Ftechnology\u002Fchatgpt-sets-record-fastest-growing-user-base-analyst-note-2023-02-01\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">fastest-growing consumer application in history\u003C\u002Fa>. Since then there are several other widely successful algorithms, like Google’s Bard and Microsoft’s Bing &#8211; the latter is also based on GPT-4 and was customised for search.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>These algorithms were not specifically trained for medical purposes and the regulations are lacking, but they \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002F6-potential-medical-use-cases-for-chatgpt\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">can still be used\u003C\u002Fa> for a number of purposes even in medicine, such as writing emails or summarising research papers.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Based on the\u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fchatgpt-in-healthcare-what-the-science-says\u002F\" target=\"_blank\" rel=\"noreferrer noopener\"> first published studies\u003C\u002Fa>, three main areas of focus for ChatGPT emerged, namely 1. clinical use, 2. answering medical questions and assisting in education, and 3. scientific writing and research.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>This field is \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fwhat-does-gpt-4-mean-for-healthcare\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">developing at breathtaking speed\u003C\u002Fa>, and \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fchatgpt-alternatives-for-healthcare-and-research\" target=\"_blank\" rel=\"noreferrer noopener\">many alternatives are emerging\u003C\u002Fa>, both for medical and general use.
