That’s the promise of the dermatology app, and we decided to take a good look at its capabilities. The team has on-board researchers, dermatologists, and professors from several universities in countries like Germany and Australia. Their app has even been clinically tested in studies in Germany and The Netherlands, and as such, gives the app some decent credentials.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">The app is quite popular with 1.2 million downloads, and the company is growing steadily. Matthew Enevoldson, PR Manager at SkinVision, told The Medical Futurist that they are currently finalizing deals to expand their offering through large health insurance providers in Italy and France. To do so, \u003Cstrong>the app will prompt you to take a picture of the spot with your phone’s camera, and in about 30 seconds, it will give you a verdict of Low, Medium or High Risk. The app does the assessment via machine learning technology.\u003C\u002Fstrong> According to Enevoldson, the most recent data shows that their service has a sensitivity of 97% and a specificity of 78%, well above that of a GP (sensitivity 60%), a dermatologist (sensitivity 75%) and even a specialist dermatologist (sensitivity 92%).\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">However, SkinVision is not a diagnostic tool and should not be taken as the final verdict, as pointed out by the disclaimer at the end of every skin spot assessment. Nevertheless, the SkinVision team, which consists of actual dermatologists, further reviews the pictures of more suspicious spots and sends the user a ‘Doctor’s Advice’, advising the user to have that spot checked by a local dermatologist. However, we are unsure as to what warrants a ‘Doctor’s Advice’, except for High-Risk spots. In our tests, we received one for a Low-Risk mole but none (at the time of writing) for Medium-Risk spots.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">\u003Cstrong>To corroborate with the app’s assessment, we tested nevi which had already been checked by a dermatologist and knew the diagnosis of. Sure enough, the app’s algorithm was spot on with the diagnoses\u003C\u002Fstrong>, effectively identifying known low and medium risk spots.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">Once the skin lesion has been evaluated, the app provides the option to set a reminder to reassess the spot at a later time. This is particularly handy given the fact that nevi can change even if they are low-risk ones. Filling out an in-app questionnaire, users can familiarize themselves with their skin type assessment. It subsequently lists the types of skin cancer that they might be at risk of according to their skin type and the precautions that should be taken. The risk evaluation determines the user’s risk of skin cancer based on factors like familial history, number of moles they have or previous issues they might have experienced with their skin. The last feature, the UV Index, gives the local UV Index on a daily basis and offers advice like sunblock use and wearing protective clothing if the index is too high.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">Concerning the price, SkinVision is available to download for free on both Android and iOS devices but to use its services will cost you some money. For one, the picture-taking process can be problematic. For the app to take a picture, the skin spot has to be focused, centered and unobstructed\u003C\u002Fstrong>. The latter requirement can cause some issues with moles partly covered by hair. Such spots proved quasi-impossible to get the app to recognize and assess, even after trying to push all the hair aside as much as possible.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">Nevertheless, SkinVision included a manual mode where pictures can be taken and assessed by a specialist in place of the app. However, sending manual photos for assessment come at an extra €14,99 for both ‘Single Smart Check’ and ‘SkinHealth Program’ options, although this service feels like it should have been included in the packages already. This aside, the manual mode only allows pictures taken on the spot to be evaluated. However, a feature that feels lacking in the current version of the app and which could be implemented in a future update is the ability to upload and assess photos of skin spots from the phone&#8217;s photo gallery. This way, pictures sent via emails and chat apps could also be assessed.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">Additionally, \u003Cstrong>smaller nevi with a diameter of around 1 mm proved to be quite a challenge to get in focus and pictured by the app\u003C\u002Fstrong>. After numerous tries, we deemed that the app could not assess those smaller lesions.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">\u003Cstrong>Moreover, for an assessment to be made, an internet connection is required. Its evaluation of spots match those done by actual dermatologists and being further supported by dermatologists with additional advice makes the app even more worthwhile. However, there are areas where it can still improve: saving offline assessments for a later online review or better assessing spots covered by hair.\u003C\u002Fp>\n\n\n\n\u003Cp class=\"has-text-align-left\">If you have several moles that you’ve meant to get checked but have been postponing for ages, the SkinVision app is an adequate 21st-century tool to get it done. better visit. Let me show you in detail, how primary care should be carried out in the future!\u003C\u002Fp>\n\u003C\u002Fdiv>\n\u003C\u002Fdiv>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Digital health should become an organic part of primary care in the future\u003C\u002Fh2>\n\n\n\n\u003Cp>I live a fairly healthy life. I use data to improve my lifestyle and to make better decisions by optimizing \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002F6-months-sleep-tracking-experiment-learnt\u002F\">my sleep pattern\u003C\u002Fa>, \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Ffitbit-helped-change-lifestyle-great-fitbit-surge-review\u002F\">my physical\u003C\u002Fa>, \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-one-year-excel-spreadsheet-experiment-that-changed-my-life\u002F\">my cognitive or my emotional abilities\u003C\u002Fa>; and&nbsp;I had \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fmydna-review\u002F\">several genetic tests\u003C\u002Fa>. What is the reason behind all my activities? I want to live a long life and work until the very end while being fit in body and soul. And I’m ready to sacrifice energy, time and money to reach it.\u003C\u002Fp>\n\n\n\n\u003Cp>Data and digital technology is my guardian and my spear-hand in order to achieve my goals. Just look at \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-one-year-excel-spreadsheet-experiment-that-changed-my-life\u002F\">my Excel spreadsheet experiment\u003C\u002Fa> and all the stunning results I could achieve in one year! My GP also thinks that for example \u003Cem>activity trackers have a ‘placebo effect’. When you know that your steps are counted, it increases your motivation, boosts your performance and you feel healthier\u003C\u002Fem>. Thus, I believe digital technology should have a bigger impact in healthcare for making people’s lives better.\u003C\u002Fp>\n\n\n\n\u003Cp>Although it is obvious that change starts with the individual, a lasting impact can only be reached when the direct environment also resonates with such changes. When people start&nbsp;to turn&nbsp;to digital technology to live a healthier life, the best response would be to advance the closest health sector to the individual, namely primary care in order to reflect the requirements of the 21\u003Csup>st\u003C\u002Fsup> century.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">So what does primary care look like now? What should be changed?\u003C\u002Fh2>\n\n\n\n\u003Cp>Healthcare today is not proactive, but rather reactive medicine. Patients usually visit the GP, when they already have some serious problem; and since doctors are overwhelmed with tasks, they can only focus on treating acute illnesses. There is not enough data about the patient, about the possible illness, most GP’s and hospitals do not incorporate the latest proven scientific research into their practice. How could they? Data processing systems are usually not that efficient.\u003C\u002Fp>\n\n\n\n\u003Cp>Doctor shortages are a global phenomenon. The World Health Organization (WHO) estimates that there will be a \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.who.int\u002Fhealth-topics\u002Fhealth-workforce#tab=tab_1\" target=\"_blank\">worldwide shortage of around 10 million\u003C\u002Fa> physicians, nurses, and allied health workers by 2030.\u003C\u002Fp>\n\n\n\n\u003Cp>At the same time, the need for healthcare services is rising. Illnesses are becoming easier to catch, civilizational diseases such as diabetes and obesity are on the rise, while aging societies need more and more care. The result? Long waiting hours at the GP or for appointments and surgeries, pricey, long, sometimes unnecessary procedures, random patient management, unequal doctor-patient relationship.\u003Cscript>\u003C\u002Fscript>\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">How could we change it?\u003C\u002Fh2>\n\n\n\n\u003Cp>We need more data and a 180-degree attitude change! Data is essential for getting to know the patients, their situation, and their diseases as well as how to heal them completely and efficiently. Digital technology could help a lot in this respect. Healthcare trackers, sensors and wearables could serve with a swarm of useful biometric data in getting to know the health parameters and vital signs of patients and diagnosing their illnesses.\u003C\u002Fp>\n\n\n\n\u003Cp>In January, 2016 \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.buzzfeed.com\u002Fstephaniemlee\u002Ffitbit-in-the-hospital?utm_term=.ao13990DD#.ds8PGGJpp\" target=\"_blank\">a man checked into a hospital’s emergency room in Camden, New Jersey\u003C\u002Fa>. Because his heart was beating irregularly, doctors suspected he had atrial fibrillation. That is the most common type of arrhythmia, which can increase the risk of stroke. But to decide how to treat him, they needed to know exactly how long his heart had been acting up. That’s when one of the physicians noticed a Fitbit Charge HR on the patient’s wrist, and it had the answers they were looking for.\u003C\u002Fp>\n\n\n\n\u003Cp>This was one very early example of how useful data extracted from wearables could be. That’s why such events should not happen on a randomized basis, but GPs as well as hospitals should rather incorporate data from healthcare trackers, sensors, even fitness wearables into their practice on a systemic level.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">What should the first encounter with your GP’s office in the future look like?\u003C\u002Fh2>\n\n\n\n\u003Cp>\u003Cem>Somewhere in the 2030s. Adam scrolled down on his phone and asked Mimi, the healthcare chatbot about the selection of GPs in the neighborhood, where he just moved in, a very neat area in Amsterdam. Based on Adam’s preferences, Mimi offered three possible choices, and he selected one. The chatbot made an appointment for him&nbsp;for the next day. By the time, he went there, his previous doctor sent over all the necessary healthcare data about him after he consented to the necessary forms about data privacy. Adam could also take all the results of his genetic tests to the GP – data about drug sensitivity, his risks for monogenetic conditions or multifactorial diseases or mutations -; as well as the data from the health trackers, he uses for measuring his physical activity, blood pressure and tracking his sleep.\u003C\u002Fem>\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cem>Adam went to the GP’s office, which rather looked like a business meet-up; and had a very friendly chat with his new doctor, who already knew about his previous knee-replacement operation as well as his peanut allergy. Then she asked a couple of further questions about his health and examined him thoroughly: ECG, blood test, knee X-ray, etc. The whole exam was less than an hour. Afterwards, the doctor gave Adam a summary of the current state of his health and estimated his risk for different diseases. Based on this information, the doctor and Adam worked out a prevention plan to avoid these health risks and set out the parameters of a personalized healthy lifestyle. They agreed to set up various targets concerning his physical activities, his dieting habits as well as factors reducing stress.\u003C\u002Fem>\u003C\u002Fp>\n\n\n\n\u003Cp>Ideally, the GPs of the near future will rather act as health coaches. They will interpret health data if something is not clear, give advice when results are not optimal, and spot and check irregularities based on data as soon as possible.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">What happens in practice?\u003C\u002Fh2>\n\n\n\n\u003Cp>I always wished Adam’s story could be mine. For years, I have been looking in vain for a GP, who thinks the same way about prevention and digital health as I do. Finally, a few years ago I found \u003Ca rel=\"noreferrer noopener\" href=\"http:\u002F\u002Fwww.vernes.hu\u002Fen\u002F\" target=\"_blank\">her\u003C\u002Fa>. She believes in the equal-level collaboration I’m evangelizing rather than the old hierarchy of the doctor-patient relationship. And she is a data freak just like me.\u003C\u002Fp>\n\n\n\n\u003Cp>So, when we first met, I brought to her\u003C\u002Fp>\n\n\n\n\u003Col class=\"wp-block-list\">\n\u003Cli>\u003Cstrong>the results of my genetic tests\u003C\u002Fstrong>: what drugs I’m sensitive to, what metabolic conditions I carry, what major mutations I have (e.g. Leiden that increases my risk for deep vein thrombosis), and what risks I have for multifactorial conditions.\u003C\u002Fli>\n\n\n\n\u003Cli>\u003Cstrong>data sets about my health parameters\u003C\u002Fstrong> in terms of sleep quality, physical activity, blood pressure\u003C\u002Fli>\n\n\n\n\u003Cli>\u003Cstrong>major lab markers\u003C\u002Fstrong> from the past years in an Excel spreadsheet so she didn’t have to look through thousands of data points, just what really mattered\u003C\u002Fli>\n\n\n\n\u003Cli>\u003Cstrong>the need for a long-term prevention program.\u003C\u002Fstrong>\u003C\u002Fli>\n\u003C\u002Fol>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">What was my GP’s reaction?\u003C\u002Fh2>\n\n\n\n\u003Cp>Instead of jumping out of the window in fright from the data tsunami, she got curious and excited about discovering my health background. First and foremost, she examined all the available information based on my data as well as the data in the healthcare system. Then she drew up my family tree with the major diseases and causes of death, carried out a full physical examination, checked all my tests and lab markers, examined my lifestyle, and then came up with the risks for different major conditions based on international data and evidence.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">What was the final verdict and how will my GP and I work together in the future?\u003C\u002Fh2>\n\n\n\n\u003Cp>At the end of the process, we agreed upon a prevention program concerning when and what kind of health tests should I undergo. This is a difficult issue by the way. We could have a full-body MRI every year but it is expensive and so far there is no strong evidence that it prolongs life, so we said no. You really need to be considerate and sane while deciding upon a regimen of tests for the long term. She also gave me pieces of advice about my lifestyle. As I exercise 30 minutes per day on average and have never smoked, I could only slightly improve my diet and alcohol intake. But now we know what to keep an eye on:\u003C\u002Fp>\n\n\n\n\u003Col class=\"wp-block-list\">\n\u003Cli>I have a higher chance for thrombosis\u003C\u002Fli>\n\n\n\n\u003Cli>I have to be careful with certain drugs in the future because the probability of having serious side effects after taking them is high (e.g. statins for high cholesterol)\u003C\u002Fli>\n\n\n\n\u003Cli>My chance for melanoma and basal cell carcinoma is very high, so I have to do an annual dermatology checkup\u003C\u002Fli>\n\n\n\n\u003Cli>I also have to keep my BMI under 25 because I have a high genetic risk for obesity and insulin resistance\u003C\u002Fli>\n\u003C\u002Fol>\n\n\n\n\u003Cp>We meet twice a year if I have no issues. In the meantime, I keep on living a physically, mentally and emotionally active life and measure data. When something is different, I let her know.\u003C\u002Fp>\n\n\n\n\u003Cp>This is what primary care should look like globally. She doesn&#8217;t order a huge range of additional tests just because she could. We made all these decisions based on evidence. Moreover, she is eager to use the data I bring to the table.\u003C\u002Fp>\n\n\n\n\u003Cp>Digital health at its best.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">So, how could we help GPs to incorporate digital health into their everyday practice? \u003C\u002Fh2>\n\n\n\n\u003Cp>There are many factors that hinder the introduction of digital technologies on the most basic level of healthcare. As my GP said\u003Cem>, the major nemesis is lack of time. Busy physicians find it difficult to incorporate into their everyday practice to work with digital information\u003C\u002Fem>. \u003Cem>Also, there is no separate allocated time to visit your doctor only for preventive visits.\u003C\u002Fem> So, first of all, we need to make time and space for digital technologies in healthcare. She also mentioned another important factor, the compatibility issue. Medical programs used in the GP’s offices might not be compatible with the health trackers’ own software.\u003C\u002Fp>\n\n\n\n\u003Cp>My GP agrees with me that \u003Cem>in the last few years,\u003C\u002Fem> there has been&nbsp;a growing interest in\u003Cem> health prevention and active involvement in health from the patient’s side\u003C\u002Fem>. It is not a surprise that there are already examples that patients are bringing their healthcare trackers or sensors to their medical professionals to check their data – and not the other way around.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Lifestyle medicine needs to become mainstream\u003C\u002Fh2>\n\n\n\n\u003Cp>You might not have heard of lifestyle medicine before now, this field is relatively new, and even&nbsp;\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fen.wikipedia.org\u002Fwiki\u002FLifestyle_medicine\" target=\"_blank\">its Wikipedia entry\u003C\u002Fa>&nbsp;is far from being very extensive. However, it is a very important new way of practicing medicine. In layman&#8217;s terms, we could say that its goal is to support patients to live healthy as long as possible, focusing on prevention instead of treatments. \u003C\u002Fp>\n\n\n\n\u003Cp>The American College of Lifestyle Medicine (ACLM)&nbsp;\u003Ca href=\"https:\u002F\u002Flifestylemedicine.org\u002FACLM\u002FACLM\u002FAbout\u002FWhat_is_Lifestyle_Medicine_\u002FLifestyle_Medicine.aspx?hkey=26f3eb6b-8294-4a63-83de-35d429c3bb88\">initiated the steps\u003C\u002Fa>&nbsp;to establish the standards shaping lifestyle medicine as a field of its own. ACLM defines it as “the evidence-based practice of helping individuals and families adopt and sustain healthy behaviors that affect health and quality of life”.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>The field draws parallels to nutrition and dietetics but involves a more holistic approach.
