A \u003Ca href=\"https:\u002F\u002Fwww.news-medical.net\u002Fnews\u002F20231108\u002FGenetic-variants-likely-affect-sleep-quality-and-quantity-in-children-study-finds.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">recent study\u003C\u002Fa> published in the Journal of Child Psychology and Psychiatry sheds light on this. The research, involving 2,458 children, found that kids who were genetically predisposed to insomnia, based on a polygenic risk score developed for adults, experienced more insomnia-like sleep issues such as frequent awakenings or difficulty initiating sleep. Conversely, those predisposed to longer sleep durations tended to have lengthier sleep but also experienced more wakefulness during the night in adolescence.\u003C\u002Fp>\n\n\n\n\u003Cp>This study provides indirect evidence that the genes predisposing adults to insomnia may also contribute to poor sleep from toddlerhood through adolescence, suggesting the existence of a &#8216;poor sleeper&#8217; trait that persists throughout a lifetime.\u003C\u002Fp>\n\n\n\n\u003Cp>Regardless of our genes, we don&#8217;t learn as children how to achieve (the best possible) quality sleep nor understand what good sleep specifically means for us as individuals.\u003C\u002Fp>\n\n\n\n\u003Cp>We previously have written about sleep tracking multiple times, you can also \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fhow-to-measure-sleep\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">check back here\u003C\u002Fa> for example.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Let’s quickly get over a few basic questions!\u003C\u002Fh2>\n\n\n\n\u003Cp>\u003Cstrong>Is it fundamental to have eight hours of sleep every night?&nbsp;\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>No, you\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.cdc.gov\u002Fsleep\u002Fabout_sleep\u002Fhow_much_sleep.html\" target=\"_blank\"> don’t necessarily need eight hours of sleep\u003C\u002Fa> each night – actually, we don’t know yet exactly how much sleep your organism craves. But why would a standardised rule determine our bedtime when we know that everyone has a different body and different needs?\u003C\u002Fp>\n\n\n\n\u003Cp>That’s when sleep tracking comes into the picture.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Can we make up for lost sleep time on the weekend?&nbsp;\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>Well, no matter how it seems, based on research \u003Ca href=\"https:\u002F\u002Fwww.medicalnewstoday.com\u002Farticles\u002F176079.php\" target=\"_blank\" rel=\"noreferrer noopener\">that’s not happening\u003C\u002Fa>. On top of that, \u003Ca href=\"https:\u002F\u002Facademic.oup.com\u002Fsleep\u002Fadvance-article\u002Fdoi\u002F10.1093\u002Fsleep\u002Fzsad253\u002F7280269\" target=\"_blank\" rel=\"noreferrer noopener\">this study suggests\u003C\u002Fa> that sleep regularity &#8211; how consistently you keep your bedtime \u002F wake-up time from day to day is an important predictor of mortality risk and is a stronger predictor than sleep duration. Although \u003Ca rel=\"noreferrer noopener\" href=\"http:\u002F\u002Fnews.gallup.com\u002Fpoll\u002F166553\u002Fless-recommended-amount-sleep.aspx\" target=\"_blank\">Gallup\u003C\u002Fa> has reported that over the past 50 years, we have been sleeping one hour less per night than we did back in the 1950s. Moreover, many people take pride in sleeping as little as possible – especially on\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.thriveglobal.com\u002Fstories\u002F13737-how-the-i-don-t-sleep-much-culture-is-impacting-your-health\" target=\"_blank\"> college campuses or in the tech sector and Silicon Valley\u003C\u002Fa>, saying they’ll sleep when they are dead. Well, inadequate shuteye can eventually lead there – it is long known that sleep deprivation has even been used for a long time as a means of interrogation by many secret services, \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.theguardian.com\u002Fus-news\u002F2014\u002Fdec\u002F09\u002Fcia-torture-methods-waterboarding-sleep-deprivation\" target=\"_blank\">including the CIA\u003C\u002Fa>.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">What matters the most?\u003C\u002Fh2>\n\n\n\n\u003Cp>So, to make the most of the sleep issue, we have to consider&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cul class=\"wp-block-list\">\n\u003Cli>the quality of sleep (which needs to be tracked for you somehow as you are sleeping and thus unable to play a role)\u003C\u002Fli>\n\n\n\n\u003Cli>the quantity of sleep (ditto)\u003C\u002Fli>\n\n\n\n\u003Cli>and how energized you feel in the morning\u003C\u002Fli>\n\u003C\u002Ful>\n\n\n\n\u003Cp>This process is intrinsically personal and involves a lot of trial and error.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>When it comes to beginning your sleep-tracking journey, starting simple is key. As you very likely already have a smartphone, the most cost-effective way to start is \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-top-sleep-apps-to-start-your-bedtime-tracking-journey\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">with a sleep-tracking app\u003C\u002Fa>. These apps can provide a surprising amount of insight without any financial investment. This was a tough decision as the old one still works perfectly. (Yes, I’m a laughing stock in the team as well for this old-world sentimentalism.)\u003C\u002Fp>\n\n\n\n\u003Cp>Of course, the \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fsearch\u002F?term=sleep\" target=\"_blank\" rel=\"noreferrer noopener\">black-belted master of sleep tracking\u003C\u002Fa> is Dr. Meskó within The Medical Futurist premises. In his two-decade-long quest for better sleep, he experimented with various methods: standalone apps like Sleep as Android, dozens of various wearables, under-mattress sensors, and combinations of these tools. This pairing works well because each device compensates for the other&#8217;s missing data, providing a more comprehensive view of sleep patterns.&nbsp;\u003C\u002Fp>\n\u003C\u002Fblockquote>\n\n\n\n\u003Cp>He uses Withings&#8217; devices for this purpose, but everyone needs to find the combination that works best for them.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">What to make of your REM, core &amp; deep sleep cycles?\u003C\u002Fh2>\n\n\n\n\u003Cp>As we are all genetically different, we need different factors during our sleep to feel rested the next day. But to make observations for yourself, you need to have info about your sleep phases.\u003C\u002Fp>\n\n\n\n\u003Cp>Most apps (and wearables) divide your nighttime into four segments:&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cul class=\"wp-block-list\">\n\u003Cli>Awake\u003C\u002Fli>\n\n\n\n\u003Cli>REM sleep\u003C\u002Fli>\n\n\n\n\u003Cli>Core\u002FLight sleep\u003C\u002Fli>\n\n\n\n\u003Cli>Deep sleep\u003C\u002Fli>\n\u003C\u002Ful>\n\n\n\n\u003Cp>REM is named after the rapid eye movement that happens during this stage. During REM the brain is incredibly active, this is the part of our sleep where we dream, process and store memories. (Sleep experts claim REM is not the only phase for doing that, but let’s settle with this thought for now).\u003C\u002Fp>\n\n\n\n\u003Cp>What we (the apps) call the core\u002Flight sleep period is defined as \u003Ca href=\"https:\u002F\u002Fwww.ncbi.nlm.nih.gov\u002Fbooks\u002FNBK526132\u002F#:~:text=The%20human%20body%20cycles%20through,wave%20patterns%2C%20and%20eye%20movements.\" target=\"_blank\" rel=\"noreferrer noopener\">two different sleeping stages\u003C\u002Fa> by sleep experts, but let’s just stay with the app classification. This is the light sleep period we spend approximately half of our nights in, and this is the stage in which you want to be awakened by a smart sleep alarm.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>And lastly, we also have deep sleep periods, which is the time when the body does maintenance. Also known as restorative sleep, the brain activity is reduced, the heart rate and respiratory rate drops, and we grow muscles and regrow tissues in deep sleep. Deep sleep typically happens in the first half of our nights and these segments last 20-40 minutes in each sleep cycle for an average person. But there is great personal variance in this as well.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">The good, the bad and the eminent one\u003C\u002Fh2>\n\n\n\n\u003Cp>The reason that finally convinced me to invest in a new watch while having a perfectly working old one was anxiety. As a new contestant in the Sleeping Terribly arena, I wanted to see what happens on those nights when I feel like I don’t sleep. I was aware of the fact that I was not actually awake the whole night, but it certainly felt like it. I slept less on the night in the first chart, but felt perfectly fine the next day, while the night in the middle left me like a zombie.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>In contrast, the last chart shows a night with excellent sleep, enough hours and enough deep sleep, with almost exactly 50% spent in light sleep and 25% in REM and deep sleep. Christmas holidays are less stressful than getting back to work, what a surprise… \u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">Getting started\u003C\u002Fh2>\n\n\n\n\u003Cp>According to Dr. Meskó, it is easy to start our journeys toward better sleep. Here is what he suggests based on his experiments. \u003C\u002Fp>\n\n\n\n\u003Col class=\"wp-block-list\">\n\u003Cli>Start tracking your sleep however you can (with pen and paper, an app, your watch, sensor, or whatever you have).\u003C\u002Fli>\n\n\n\n\u003Cli>Each morning, give a score between 1 and 10 to your sleep, based on how rested you feel, it is also fine if you just use an old-school notebook, nothing fancy is needed!&nbsp;\u003C\u002Fli>\n\n\n\n\u003Cli>Don’t get discouraged that things seem unchanged for a while.\u003C\u002Fli>\n\n\n\n\u003Cli>And then suddenly, start noticing patterns, and get closer to what makes you feel the best in the morning. For Dr. Meskó it is to have at least one long &#8211; 60-minutes + &#8211; deep sleep period. But everyone is different!\u003C\u002Fli>\n\n\n\n\u003Cli>Start noticing the factors that ruin your sleep. It may be stress, alcohol consumption, intense workout in the evening, too much screentime, screentime right before falling asleep, dinner too late, etc, etc.&nbsp;\u003C\u002Fli>\n\n\n\n\u003Cli>Learn the ideal sleep quantity, as (for example) having the desired amount of deep sleep alone won’t make you feel perfectly rested if you don’t have enough hours. The optimal amount of sleep varies a lot, it can be 7 hours for one person and 8 and a half for another.\u003C\u002Fli>\n\n\n\n\u003Cli>Find the right time for getting up. It is very different getting aroused from deep or light sleep, making a huge difference in how you feel in the morning &#8211; or, as Dr. Meskó declared: a smart sleep alarm is \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-smart-sleep-alarm-is-the-holy-grail-of-health-tracking\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">the holy grail of health tracking\u003C\u002Fa>.&nbsp;\u003C\u002Fli>\n\u003C\u002Fol>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">I’m still having bad nights, so what’s the point?\u003C\u002Fh2>\n\n\n\n\u003Cp>Tracking sleep won’t make all our nights perfect. I have bad days due to stress. Dr. Meskó has a newborn in the house, also not ideal for sleep quality. It reached the milestone of having over 100 million users in only 2 months, and it became the \u003Ca href=\"https:\u002F\u002Fwww.theverge.com\u002F2023\u002F11\u002F6\u002F23948386\u002Fchatgpt-active-user-count-openai-developer-conference#:~:text=ChatGPT%20was%20widely%20seen%20as,2004%2C%20Twitter%20took%20over%20five\" target=\"_blank\" rel=\"noreferrer noopener\">fastest-growing consumer application\u003C\u002Fa> ever. In less than a year, it has over 100 million users per week, and millions of developers are building on its API.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>ChatGPT was the first widely available solution for interacting with AI and it has changed everything. Artificial intelligence, the mystic tool available only to a select few, became an everyday tool for anyone – overnight. And of course, such a mainstream hype around AI made us think \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fwhats-next-for-ai-in-healthcare-in-2023\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">what more we can expect\u003C\u002Fa> for the rest of the year.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Enter medical alternatives with their specific challenges\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>As healthcare is an industry where decisions can be a matter of life and death, we can’t just start using random large language models for diagnosing patients or doing the paperwork. Many of these algorithms are quite reliable, but even the \u003Ca href=\"https:\u002F\u002Fgithub.com\u002Fvectara\u002Fhallucination-leaderboard\u002Ftree\u002Fmain\" target=\"_blank\" rel=\"noreferrer noopener\">best of the models hallucinate\u003C\u002Fa> now and then. That is why we need healthcare-specific models.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Only a few weeks after the release of ChatGPT, Google\u002FDeepMind announced Med-PaLM, a \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fmedpalm-new-ai-medical-chatbots-will-soon-be-better-than-waiting-for-a-doctor\u002F\" target=\"_blank\">large language model specifically designed to answer healthcare-related questions\u003C\u002Fa>, based on their 540-billion parameter PaLM model. Med-PaLM can’t be tested by the general public, but you can read the \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Farxiv.org\u002Fpdf\u002F2212.13138.pdf\" target=\"_blank\">researchers&#8217; paper here\u003C\u002Fa>.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>In a few months, a study published in Nature revealed that the more advanced Med-PaLM 2 answered medically related questions with 92.6% accuracy. Although \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.nature.com\u002Farticles\u002Fs41586-023-06291-2\" target=\"_blank\">the detailed data shows\u003C\u002Fa> that MedPaLM still has room for improvement, the results are impressive. But they allow us a glimpse into future use cases we can expect in a few years.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Regulatory issues became the hottest topic\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>As generative AI algorithms became extremely popular and we started to see \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002F17-healthcare-companies-that-already-integrated-chatgpt-infographic\u002F\" target=\"_blank\">real-life healthcare integrations of the technology\u003C\u002Fa> in the healthcare segment, \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fwhy-and-how-to-regulate-chatgpt-like-large-language-models-in-healthcare\u002F\" target=\"_blank\">regulatory issues became very prominent\u003C\u002Fa>. It soon became evident that the capabilities of LLMs expand beyond text-based interactions, to analysing images, documents, hand-written notes, sound, and video. Thus we need to regulate the future, not just the present.\u003C\u002Fp>\n\n\n\n\u003Cp>The Medical Futurist and Dr. Eric Topol published a paper “\u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.nature.com\u002Farticles\u002Fs41746-023-00873-0\" target=\"_blank\">The Imperative for Regulatory Oversight of Large Language Models (or Generative AI)&nbsp;in Healthcare\u003C\u002Fa>” in Nature’s npj Digital Medicine. They analysed the challenges and the possibilities of LLM regulations to make them accessible for healthcare use.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>The question is indeed tricky, we need a solid framework for constantly changing algorithms to make them safe, but this framework needs to be flexible enough not to stifle innovation.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>And this is just one facet. Privacy issues are also a major concern when it comes to AI models, with multiple layers of problems. We discussed these when we analysed \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fhere-is-what-even-healthcare-ai-companies-do-with-your-data\u002F\" target=\"_blank\">what AI companies do with your data\u003C\u002Fa>.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Aiming for a high-level understanding\u003C\u002Fstrong>\u003C\u002Fp>\n\n\n\n\u003Cp>With so many things happening at once, a steadily growing part of humanity started feeling lost or left out. However, throwing our hands in the air and declaring that “AI is not for me” will definitely not be a winning attitude for our future careers.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>However, the good news is that everyday healthcare professionals don’t need to become computer scientists or designers of deep learning neural networks. Most of us only need a “scientific” understanding, like we have with our cars or an fMRI machine. Several things already arrived, for example, multimodality has become present.&nbsp;OpenAI integrated all the different “modes” of ChatGPT into a unified user interface, which allows you to upload visual prompts for example.&nbsp;&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>And most recently, \u003Ca href=\"https:\u002F\u002Fwww.wired.com\u002Fstory\u002Fhow-to-use-google-gemini-ai-bard-chatbot\u002F?utm_source=The+Medical+Futurist+Newsletter&amp;utm_campaign=83766d1589-EMAIL_CAMPAIGN_2022_02_01_COPY_01&amp;utm_medium=email&amp;utm_term=0_efd6a3cd08-83766d1589-&amp;mc_cid=83766d1589&amp;mc_eid=UNIQID\" target=\"_blank\" rel=\"noreferrer noopener\">Google announced Gemini\u003C\u002Fa>, its natively multimodal algorithm, which, according to the company, is “a brand new breed” of AI. I’m very happy to witness this whole AI revolution and have learned a LOT in the past 12 months. Sure, artificial intelligence can’t immediately solve all our problems, and we have to settle a huge heap of regulatory, ethical, and moral questions besides technological advances, but I remain hopeful.&nbsp;\u003C\u002Fp>\n\n\n\n\u003Cp>Can’t wait to see what 2024 brings!
