Their work at the clinic did not differ much from that of any other orthopaedic hospital in the world. Well, maybe they are somewhat closer to famous tennis courts. But when COVID-19 hit, they needed to re-think their entire workflow. They shifted to telemed solutions in the matter of days. Jonathan Bell was at the centre of it all. He helped us understand the practicalities of such a direct transformation.\u003C\u002Fp>\n\n\n\n\u003Cp>“\u003Cem>The aim was, initially, just to try and replicate our usual services remotely\u003C\u002Fem>” – he said about the shift they launched in a mere five days. “\u003Cem>Once systems were in place, we went fully remote for all admin and clinical staff\u003C\u002Fem>.” \u003C\u002Fp>\n\n\n\n\u003Cp>Before the pandemic, Wimbledon Clinics barely provided any telemedicine. Being a sports clinic, much of their work was insurance-based – insurers refused to reimburse for telemedicine and telephone calls.“\u003Cem>About 70-75% of our work is based on insurance. I did electronic pdf’s of all forms. I set out to run my whole practice from an iPad Pro\u003C\u002Fem>.” \u003C\u002Fp>\n\n\n\n\u003Cp>This needed to be worked out with every single department: from radiology to the pharmacy. It took the hospital where we work another 6 weeks to match us in terms of the \u003Cem>offering.”\u003C\u002Fem>\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cem>“That put the practices in my mastermind group way ahead of their competitors, and gave us a step in the patient pathway that has proven to be very effective.”\u003C\u002Fem> In other words, they created the online physio capability so that they were able to offer a treatment pathway, not just a diagnosis. \u003C\u002Fp>\n\n\n\n\u003Cp>“\u003Cem>I have rehabilitated knee replacements and ACL reconstruction successfully almost entirely through Zoom physios\u003C\u002Fem>” – Bell explained. \u003C\u002Fp>\n\n\n\n\u003Cfigure class=\"wp-block-image size-large\">\u003Cimg loading=\"lazy\" decoding=\"async\" width=\"700\" height=\"414\" src=\"https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2020\u002F07\u002Fdo-patients-trust-telemedicine-01.png\" alt=\"Source: https:\u002F\u002Fhitconsultant.net\u002F2015\u002F07\u002F15\u002Ftelemedicine-patients-wouldnt-trust-a-diagnosis\u002F\" class=\"wp-image-29407\"\u002F>\u003Cfigcaption>Source: \u003Ca href=\"https:\u002F\u002Fhitconsultant.net\u002F2015\u002F07\u002F15\u002Ftelemedicine-patients-wouldnt-trust-a-diagnosis\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">HIT Consultant\u003C\u002Fa>\u003C\u002Ffigcaption>\u003C\u002Ffigure>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Step Four: The Right Platform and Practice\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>The remote platforms they tested were \u003Ca href=\"https:\u002F\u002Fwww.skype.com\u002Fen\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">Skype\u003C\u002Fa>, \u003Ca href=\"https:\u002F\u002Fzoom.us\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">Zoom\u003C\u002Fa> and \u003Ca href=\"https:\u002F\u002Fwww.webex.com\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">Webex\u003C\u002Fa>. Initially, Skype was the best as most users had it on their computers. But they soon found Zoom working better. With people getting more and more familiar with the app, the workflow also became smoother. The lockdown situation also enabled physicians to become more flexible with time. \u003C\u002Fp>\n\n\n\n\u003Cp>Clinicians ran all video from their own equipment at home. “\u003Cem>We have a company that books our new patient appointments and we trained them to offer the video calls. We wrote a piece for them to help overcome patients objections to video as initially patients were reluctant to accept that a video call would have any value.”\u003C\u002Fem> \u003C\u002Fp>\n\n\n\n\u003Cp>They sent a \u003Ca href=\"https:\u002F\u002Fwww.jotform.com\u002Fhow-to-create-a-survey\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">Jot form survey\u003C\u002Fa> to every patient allowing them to share the satisfaction data. “\u003Cem>Our patient pathway included new consultation, ordering of investigation, sending out prescriptions, ordering of ultrasound-guided injections. We also had a list of physios who could offer video physiotherapy.”\u003C\u002Fem>\u003C\u002Fp>\n\n\n\n\u003Cfigure class=\"wp-block-image size-large\">\u003Cimg loading=\"lazy\" decoding=\"async\" width=\"731\" height=\"331\" src=\"https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2020\u002F07\u002Fbenefits-of-rmm-service.jpg\" alt=\"Source: one.comodo.com\" class=\"wp-image-29409\"\u002F>\u003Cfigcaption>Source: one.comodo.com\u003C\u002Ffigcaption>\u003C\u002Ffigure>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>The Conclusions\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>The results were unquestionable. Most patients enjoyed the comfort and the uniqueness of telehealth. About 95% of the people who answered, said the video consultation was “worthwhile” or “very worthwhile.” Bell’s team even analysed the different types of calls. Follow-ups, new patients, getting results – all scored similarly in terms of satisfaction. \u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>Video consistently scored a bit better than the telephone.\u003C\u002Fstrong> But there were a few negative comments, too. “\u003Cem>We had a few patients who said they didn’t value the experience at all. In one the clinician hadn’t spent enough time and detail as I suggested above\u003C\u002Fem>.”\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>The biggest surprise for them was the extremely high level of satisfaction\u003C\u002Fstrong>.\u003Cem> “I asked if patients would have valued the remote experience the same outside of lockdown. 60% said that this would have worked at any time.”\u003C\u002Fem> This made Bell’s team offer video consultations as a permanent feature.\u003C\u002Fp>\n\n\n\n\u003Cp>\u003Cstrong>And the downside? As Gernsback explained it, the device would have made it possible to “feel at a distance”. He basically described the function and aim of telemedicine – going a “tiny bit” further than technology would allow it today.\u003C\u002Fp>\n\n\n\n\u003Cp>But the need for healing from a distance was already there (more than) a century ago. Patients who could or would not want to go into cities or doctors who yearned for consulting a specialist colleague, they all wished for something to make their lives easier.
