Why is the former a concept we need to embrace in the future of medicine, and why is it time to forget about the latter? These are the questions we examined \u003Ca href=\"https:\u002F\u002Fwww.jmir.org\u002F2022\u002F8\u002Fe39178\u002F#ref14\" target=\"_blank\" rel=\"noreferrer noopener\">in our latest paper published in the Journal of Medical Internet Research\u003C\u002Fa>, authored by The Medical Futurist Dr Bertalan Mesko and patient scholar \u003Ca href=\"https:\u002F\u002Fwww.linkedin.com\u002Fin\u002Fepatientdave\u002Frecent-activity\u002Fshares\u002F\" target=\"_blank\" rel=\"noreferrer noopener\">Dave deBronkart\u003C\u002Fa> – widely known as ePatient Dave. \u003C\u002Fp>\n\n\n\n\u003Cp>Patient centricity has been the buzzword of the past two decades, especially often used in the 2010s, from pharma companies to healthcare providers, it was loudly advertised everywhere. As nice as it sounds, it proved to be little more than “greenwashing” of the old model, as the only thing it actually meant was “we \u003Cem>might \u003C\u002Fem>think about you when \u003Cem>we make\u003C\u002Fem> decisions”. \u003C\u002Fp>\n\n\n\n\u003Cp>Patient centricity, as was used so far, is nothing but an illusion. It keeps patients in a passive role, their voices have no power since the decisions are made by the traditional stakeholders of the medical ecosystem. Sociologist Sherry Arnstein bluntly called this tokenism.\u003C\u002Fp>\n\n\n\n\u003Ch4 class=\"wp-block-heading\">Patient design is a different concept, one that invites patients as active participants – and stakeholders – in the highest levels of decision-making in healthcare. In the 21st century, this is not true anymore. Patient empowerment has been evolving for decades, but information liquidity and access to technology made it explode in this century—and it became visible to the naked eye. \u003C\u002Fp>\n\n\n\n\u003Cp>The following examples will underline how this paradigm change happens, and what profound impact the involvement of the patient perspective can bring to medicine. \u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">1. Patient design in physical products \u003C\u002Fh3>\n\n\n\n\u003Cp>Most products born from patient design have a personal story behind the innovation: the suffering of a patient or a relative, very often a child. This explains how different a motivation these innovators have compared to “ traditional research teams”. All they want is a solution \u003Cem>now\u003C\u002Fem> (or as fast as possible) that works for \u003Cem>them\u003C\u002Fem>. \u003C\u002Fp>\n\n\n\n\u003Cp>An archetypal example is how Sarah Olson, a mother worked tirelessly to develop a \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Flevityproducts.com\u002Fcompany\u002Four-story\u002F\" target=\"_blank\">catheter stabilisation device (called LECS)\u003C\u002Fa> that since have become a class 1 medical device. Her son, Levi was born with Spina Bifida and had to undergo 15 surgeries before he was 4. With so many operations came extended periods of catheterisation, and with that, extremely painful side effects. \u003C\u002Fp>\n\n\n\n\u003Cp>Watching his child suffering surgery after surgery became unbearable, so Sarah consulted doctors and did her own research to find a solution. She soon learned that no device already on the market would help her son. Levi’s doctor told Sarah that if she wanted a different solution, she would have to invent it herself. Sarah did just that — this is how LECS I was created.\u003C\u002Fp>\n\n\n\n\u003Cp>Similarly inspiring story is that of Tal Golesworthy, a boiler engineer with Marfan syndrome, who was faced with a constant threat of an aneurysm rupture, and almost certain death. While he was obviously repelled by the thought of passing away prematurely, he also did not find the traditional medical answer – a major surgery, and life on anticoagulant drugs, which themselves create hazards – compelling. \u003C\u002Fp>\n\n\n\n\u003Cp>Thus, giving it some thought, he came up with a theoretical solution and summoned a multidisciplinary team of various engineers, cardiac surgeons, a radiologist, a morphologist and a catalysis chemist to see if they can indeed find an alternative treatment. \u003C\u002Fp>\n\n\n\n\u003Cfigure class=\"wp-block-image size-large is-style-default\">\u003Cimg loading=\"lazy\" decoding=\"async\" width=\"768\" height=\"432\" src=\"https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2021\u002F03\u002Ftmf_article_249-01-768x432.png\" alt=\"\" class=\"wp-image-33403\" srcset=\"https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2021\u002F03\u002Ftmf_article_249-01-768x432.png 768w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2021\u002F03\u002Ftmf_article_249-01-370x208.png 370w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2021\u002F03\u002Ftmf_article_249-01-1536x864.png 1536w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2021\u002F03\u002Ftmf_article_249-01.png 1920w\" sizes=\"auto, (max-width: 768px) 100vw, 768px\" \u002F>\u003C\u002Ffigure>\n\n\n\n\u003Cp>The team developed a revolutionary new surgical procedure which has since been used in hundreds of cases and introduced \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fexstent.com\u002Fsurgical-centres\u002F\" target=\"_blank\">in a number of countries and surgical centres\u003C\u002Fa>. Their method stabilises the aorta from the outside, it drastically shortens the time spent on the operating table, leaves the vascular system intact, requires no stopping of the heart, no cooling of the patient and no anticoagulants in the aftermath. Click over to \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.ted.com\u002Ftalks\u002Ftal_golesworthy_how_i_repaired_my_own_heart?language=en\" target=\"_blank\">his inspiring TED Talk\u003C\u002Fa> to learn about how he repaired his own heart. \u003C\u002Fp>\n\n\n\n\u003Cp>These stories illustrate how profoundly different scopes these patient-driven innovations have: they are not interested in unifying theories, they want a solution that works for their particular situation – now. Yes, many of these breakthroughs are able to help others in the future, but that is a secondary benefit. When your child screams in pain, when your heart might burst tomorrow, you will not be consoled by the promise of a cure in 15 years. \u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">2. Patient design in research\u003C\u002Fh3>\n\n\n\n\u003Cp>In research, patient voices are also calling for researchers to change priorities to match patients’ urgent needs. \u003C\u002Fp>\n\n\n\n\u003Cp>The father of a son with suicide ideation \u003Ca href=\"https:\u002F\u002Fwww.nytimes.com\u002F2022\u002F02\u002F22\u002Fus\u002Fthomas-insel-book.html\" target=\"_blank\" rel=\"noreferrer noopener\">told Dr Thomas Insel after a speech\u003C\u002Fa>, “Our house is on fire, and you’re telling us what you learned about the chemistry of the paint.” The scientific literature may contain volumes about “the paint,” but Insel realized “this gap between our scientific progress and our public health failure.” He left academia to pursue product development to solve real-world problems.\u003C\u002Fp>\n\n\n\n\u003Cp>When the husband of \u003Ca href=\"https:\u002F\u002Fwww.esmo.org\u002Fabout-esmo\u002Fprofiles\u002Fbettina-ryll\" target=\"_blank\" rel=\"noreferrer noopener\">Bettina Ryll, MD, PhD\u003C\u002Fa>, was dying of melanoma, she switched hats and observed the clinical trials process as a family stakeholder and was incensed to discover that researchers chose their work priorities without consulting the people who were dying. Today, she advocates in Nature for researchers to do just that.\u003C\u002Fp>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">3. Involvement in clinical design\u003C\u002Fh3>\n\n\n\n\u003Cp>Compared to the previous examples, involving patients in the design of healthcare spaces might seem to have a subtler impact. An\u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fpeek-into-the-future-of-hospitals\u002F\" target=\"_blank\" rel=\"noreferrer noopener\"> early adopter \u003C\u002Fa>is the Netherlands’ Oral and Maxillofacial Surgery Department of Radboud University Medical Centre Nijmegen, where they redesigned the rooms in the whole department based on advice from patients to enable a more balanced patient-physician relationship and a comfortable atmosphere. \u003C\u002Fp>\n\n\n\n\u003Cfigure class=\"wp-block-image size-large is-style-default\">\u003Cimg loading=\"lazy\" decoding=\"async\" width=\"768\" height=\"432\" src=\"https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2021\u002F04\u002Ftmf_article_259-01-1-768x432.png\" alt=\"\" class=\"wp-image-34109\" srcset=\"https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2021\u002F04\u002Ftmf_article_259-01-1-768x432.png 768w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2021\u002F04\u002Ftmf_article_259-01-1-370x208.png 370w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2021\u002F04\u002Ftmf_article_259-01-1-1536x864.png 1536w, https:\u002F\u002Fcdn.medicalfuturist.com\u002Fwp-content\u002Fuploads\u002F2021\u002F04\u002Ftmf_article_259-01-1.png 1920w\" sizes=\"auto, (max-width: 768px) 100vw, 768px\" \u002F>\u003C\u002Ffigure>\n\n\n\n\u003Cp>They prioritised round tables over square ones for more friendly conversations and suggested brightly lit rooms with warm colours. \u003C\u002Fp>\n\n\n\n\u003Cp>If we start to think about hospital design a little more, it is obvious how often it reflects the old-world hierarchy of patients and doctors. Why do we almost exclusively restrain patients to bed rest, even if their condition would allow them to be more active, and we have medical literature proving how prolonged immobilisation is actually damaging? Why not have social spaces, activities and more space for therapy in our hospitals to encourage more social interactions and physical activity, and thus: faster recovery? \u003C\u002Fp>\n\n\n\n\u003Cp>Healthcare carries many concepts that are no longer valid but are ingrained so deep that we mostly never even stop to think about them. institutions providing medical services. However, with digital health technologies that democratise access to care, the point of care will not be limited to healthcare institutions. It will rather be split into two; with hospitals becoming health centres for disease prevention, acute care and surgical needs, while monitoring patients’ vitals will be done closer to patients themselves.\u003C\u002Fp>\n\n\n\n\u003Cp>We explore these aspects of the future of hospital design in this two-part series. In the \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-future-of-hospital-design-inside-the-point-of-care\" target=\"_blank\" rel=\"noreferrer noopener\">first article\u003C\u002Fa>, we looked at design adjustments to accommodate digital health technologies to enhance the workplace of caregivers while providing a better patient experience. It allows caregivers to evaluate the patient’s blood sample on the spot and wirelessly transmit the results to colleagues. Another handheld device is the FDA-approved \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Ffirst-medical-tricorder-viatom-checkme-pro-review\u002F\" target=\"_blank\">Viatom CheckMe Pro\u003C\u002Fa>. It measures a number of vital signs from ECG through blood oxygen level to body temperature. \u003C\u002Fp>\n\n\n\n\u003Cp>Equipping first responders with such tools will save valuable time. Thereby they can perform more tests and deliver appropriate care at the point-of-care. As Dr. Anderson told The Medical Futurist, healthcare of the future should occur across spatial scales; to include the micro-level of our homes and communities all the way up to the macro scale of the hospital campus within the urban context. And one space we can already conquer is emergency medicine, which already bridges hospitals and patients.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Patients as the point-of-care\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>As discussed in the previous section, portable diagnostic devices bring data to the point-of-care; but patients themselves can adopt personal health sensors and wearables to make more informed decisions about their health.\u003C\u002Fp>\n\n\n\n\u003Cp>A smartwatch embedded with an ECG monitor like the Apple Watch can monitor their cardiac activity and \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fcan-your-smartwatch-send-you-to-the-hospital\u002F\" target=\"_blank\">notify them\u003C\u002Fa> of suspicious signs. They can serve as guides for patients adopting such tools. Physicians can instruct patients regarding how to analyse the metrics measured and when to seek medical attention.\u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Redesigning telemedicine for the receiving end\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>In the \u003Ca href=\"https:\u002F\u002Fmedicalfuturist.com\u002Fthe-future-of-hospital-design-inside-the-point-of-care\" target=\"_blank\" rel=\"noreferrer noopener\">first article\u003C\u002Fa> of this series, we discussed the increasing importance of telemedicine; and how designers must factor in the practice when planning future hospitals. “\u003Cem>As a result of COVID-19 and funding changes, virtual visits have now become more accepted by providers and consumers,\u003C\u002Fem>” said Dr. Anderson.\u003C\u002Fp>\n\n\n\n\u003Cp>Already, \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fmedcitynews.com\u002F2021\u002F03\u002Ftelehealth-is-ready-for-its-second-dose-introducing-virtual-care-2-0\u002F\" target=\"_blank\">95% of providers\u003C\u002Fa> are willing to use telemedicine. This indicates a trend that telemedicine is on track to become the new norm. To accommodate this need, Diana Anderson suggests “\u003Cem>dedicated spaces for virtual consultations. In particular, chronic care patients could heed to the advice; as even cancer patients continued their treatment \u003Ca rel=\"noreferrer noopener\" href=\"https:\u002F\u002Fwww.timesrecordnews.com\u002Fstory\u002Fnews\u002Flocal\u002F2020\u002F04\u002F11\u002Ftelehealth-help-cancer-patients-continue-treatment\u002F5125094002\u002F\" target=\"_blank\">thanks to telemedicine\u003C\u002Fa> during the pandemic. \u003C\u002Fp>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Democratising access to care\u003C\u002Fstrong>\u003C\u002Fh2>\n\n\n\n\u003Cp>Ultimately, such redesigning of hospitals from the inside out aims to achieve a core principle of digital health: to democratise access to care. With health metrics obtained from wearables, patients are more empowered in managing their own health and can engage in an equal-level partnership with their physicians.\u003C\u002Fp>\n\n\n\n\u003Cp>But while medical-grade tools become available at the comfort of one’s home, hospitals themselves won’t be obsolete. Rather, they could focus their resources on acute care treatment, surgical units, laboratories and imaging centres. Dr. Anderson and Cath Lewin share a similar view, seeing such high-tech facilities to be adequate for ‘very sick’ patients. “\u003Cem>Those who are not critically ill could be supported in more expansive home-based care models and community programs, she suggests. In this model, virtual care will play a key role in the monitoring of chronic conditions, prevention and wellness\u003C\u002Fem>.” \u003C\u002Fp>\n\n\n\n\u003Cp>They also added a utopistic concept. “P\u003Cem>erhaps our homes could contain a designated ‘health space’ or room\u003C\u002Fem>;\u003Cem> so that as we progress through life stages, we maintain our connectivity to care.\u003C\u002Fem>”\u003C\u002Fp>\n\n\n\n\u003Chr class=\"wp-block-separator\"\u002F>\n\n\n\n\u003Ch5 class=\"wp-block-heading\">As for the telemedicine service or app itself, a feature-rich and user-friendly interface could help.
